Operator fell due to loosing grip and footing while climbing down the stairs of Front Loader. Recent rainfall caused wet/muddy conditions in the job site area which contributed to the incident. Operator sustained minor injuries to leg, hip, and elbow due to the incident.

Employee was attempting to refill the injection pod that was under 20psi of pressure. The employee attempted to relieve the pressure by slowly removing the fill port plug on top of the pod. The plug flew off, hit a metal beam above the pod and ricocheted back & struck employee on the head.

Employee bagging on production line reported lower back pain. Told supervisor EE was ok. Reported to Supervisor a 2nd time that EE slipped. Co-worker reported that the employee had another Operator crack EE's back prior to the reported incident as well. Employee was treated at Hospital same day and issued a DR. excuse to return to work in 2 days without any restrictions.

Employee was cleaning around grinding mill #5 when mill puffed a cloud of dust into the air due to back pressure in the mill system. Natural barium sulfate (barite) material got into employee's eyes and back. Employee flushed eyes with water and eye saline. Employee was treated for eye abrasion and prescribed for eye drops.

Employee was clearing bridging wet Aluminum Trihydrate material from bucket elevator inlet chute with a shovel. The area was restricted access with barricade and signage. Shovel got caught to rotating buckets of elevator causing fingers to pinch between the shovel handle and inlet chute. EE received treatment from hospital ER for finger closed fracture and nail avulsion.

EE was holding a tapered bull pin size 13 1/2" x 2" diameter with a channel lock pliers. Another ee was striking the pin with a sledge hammer. They were dismantling the drive shaft of a bead mill. The pin ejected out, hitting ee on his left side of upper lip causing a cut and bleeding. Medical treatment was given with four stitches.

The miner was working on installation of a new dust collector. He had to reposition his safety harness by standing on a "X" brace of the dust collector to adjust himself. He slipped on the angle iron of the "X" brace about 8 ft and his right foot hit the end of the brace. X ray revealed that he had fractured bone on his fifth (smallest) toe of his right foot.

The miner struck the back of his head on a hopper steel frame while inspecting the support legs at the bottom of the hopper. He was wearing a hard hat. There was no swelling or bleeding. Employee did not report the incident to his supervisor until 9/27/12 after he went in to ER for dizziness and vomitting. Employee is undergoing medical test to determine cause of symptoms.

The employee was trying to get the material to flow from a silo by hitting onto the conical bottom of the silo using a plastic mallet. The mallet slipped from his hand and his right hand knuckles hit the metal ring on the silo. The employee did not request for medical treatment until 11/21/2011 and was on restricted duty.

While performing equipment repairs, the employee overextended his left arm, dislocating the same shoulder he had surgically repaired before. He popped it back in place and said he did not need medical attention. His shoulder started to hurt again on 4/8/08, so he went to see a doctor that day. The diagnosis is a dislocated shoulder with pain. He was placed on restricted duty.

Injured party and co-worker were working together to pick up the fork extension for a forklift. Co-worker was driving forklift & injured was holding the extension to help load. As co-worker was moving forward to pickup extension, injured's hand got stuck between fork extension & another piece of equipment injuring left hand distal finger. Left distal tip amputation.

A contractor employee was packaging material at his assigned bagging station when he slipped and fell on a wet spot on the floor where he was working. The treating physician diagnosed the injury as a right hand and right hip contusion. He was allowed to return to work on restrictive duty.

A janitor working through a cleaning service company was found deceased in the main office building at approximately 4:15 a.m. At this time, the cause of death has not been determined by the medical examiner. However a heart attack is suspected, since he had previously suffered a heart attack approximately 20 years ago according to his wife.

Due to a wet area on the floor because of rain, the employee's right foot slipped and twisted as he stepped out of the control room. He tried rest and applied ice on his foot right after the injury, but had to see a physician the next day due to increase pain. The attending physician diagnosed the injury as an ankle sprain and placed him on restricted work duty.

The employee was removing a mill slurry hose normally not under pressure, when the hose sprayed TiO2 slurry all over his face and in his right eye. He washed his face and flushed his eye with water but decided to see a doctor after his eye developed a thick mucus the next morning. The doctor washed his eye and prescribed an ointment for 7 day use.

His physician prescribed a few days of light duty and will continue to evaluate him until he is able to release him to resume normal duties.

While walking to the main office, the employee was struck by another employee with the rear end of his automobile as he was backing up out of the parking area. The injuried employee was struck on the right side hip/leg area causing her to fall on her back side and elbows. The injuries sustained include fractures of the left wrist and tail bone as well as scrapes to both elbows.

On Sunday, 12/18/05, the employee notified his supervisor he had had to go to the hospital emergency because his left knee was hurting due to an old injury. Four days later the employee told his supervisor he might have actually injured himself at the job site on 12/17/05 while stepping off a front end loader and will need an MRI to determine the severity of the injury.

The injured person was trying to pry open the chamber door to a Drais mill, when the pry bar he was using slipped. The injury occured when he over extended his right elbow. The doctor diagnosised the injury as a lateral epicondylitis of the right elbow.

THE EE WAS CLOSING OFF A COMPARTMENT GATE OF A HOPPER RAILCAR USING A LARGER METAL BAR, WHEN THEBAR SLIPPED AND FELL ONTO THE TRACKS BELOW THE RAILCAR. HE DID NOT LET GO OF THE METAL BAR BEFOR E IT HIT THE TRACK AND THEREFORE SMASHED ONE OF HIS FINGERS ON HIS RIGHT HAND. THE EE SUFFERED A FRACTURED FINGER TIP OF THE RIGHT HAND AND WILL HAVE TO BE ON RESTRICTED WORK DUTY.

EE WAS PACKAGING 50# BAGS OF PRODUCT WHEN HE FELT A SHARP PAIN IN UPPER MIDDLE SECTION OF HIS BACK. HE SAID HIS BACK POPPED AT THE TIME & THAT HE FELT THE PAIN FROM THE MIDDLE OF HIS BACK UP T O THE LEFT SHOULDER BLADE. THE PHYSICIAN'S DIAGNOSIS WAS A "THORACIC STRAIN." HE TOLD EE TO TAKE A DAY OFF & THEN RE-EXAMINED HIM NEXT WORKING DAY (11/4) & RELEASED HIM FROM RESTRICTED DUTY.

THE EE WAS ASSISTING OFFICE PERSONNEL MOVE SOME BOXES USING A DOLLEY WHICH NORMALLY IS NOT INE OF HIS DUTIES. THE DOLLEY THEY WERE USING NEEDED TO BE LIFTED UP PAST A 3" STEP GOING INTO THE BU ILDING. THE EE REACHED UNDER THE DOLLEY AND TRIED TO LIFT IT UP PAST THE STEP BUT PULLED TOO HARD CAUSING LEFT SHOULDER TO DISLOCATE. PLACED ON RESTRICTED DUTY TWO WEEKS.

HOWEVER, HE DID NOT REPORT THE INJURY UNTIL LATER THAT AFTERNOON AT APPROX. 2:30PM. AT THAT TIME HE WAS SENT TO SEE A DR. WHO DIAGNOSED A LUMBAR STRAIN AND PLACED EE ON RESTRICTED DUTY.

THE INJURED PERSON WAS HELPING ATTACH A BUCKET ON TO A SMALL BOBCAT LOADER. WHILE HE WAS TRYING TO PUSH DOWN A LATCH WITH HIS FOOT, THE OPERATOR WAS IN THE DRIVER SEAT AND LEAN FORWARD ACCIDEN TALLY PUSHING THE FOOT PEDAL THAT ACTIVATED THE TILT MOTION WHICH PINCHED THE INJURED PERSON FOOT BETWEEN THE BUCKET AND THE FRAME OF THE HEADER.
