An OTR technician at a quarry installed two spare tires and a new wheel on a Komatsu WA900 loader. While demounting the scrap tire/wheel in the storage yard, EE removed the O-ring with EE's left hand. Residual pressure caused the bead seat band to spring upward, striking EE's left thumb and index finger. This amputated the tip of EE's left index finger and crushed EE's left thumb.

Employee was walking to check on pit pump and tripped resulting in a shoulder strain. Accident became reportable on 11/15/2024 when employee had surgery.

Employee was cleaning the inside of a Komatsu 785 haul truck bed using a hose and ball valve nozzle opened about half way. This wasn't enough pressure to clean out the bed, so EE opened the ball valve to full open causing the water pressure to increase. This increased the force behind the nozzle causing the hose assembly to lift striking and leaving lacerations under EE's chin.

EE was using a bandsaw to cut a 350KCmil wire. While cutting outward, the saw caught and snagged the wire, and the saw turned toward the EE. This motion made a minor laceration on the EE's right index finger, resulting in three stitches.

A haul truck was being loaded in pit. The muck pile began to slough causing wall material behind the pile to shift. This material slid down onto and covered the rear of the truck enough that the operator was unable to drive away. EE safely dismounted the truck to meet the loader operator on the ground. Three days later reported had back pain and sought medical care.

An over-the-road truck driver (non-miner) with CKJ Transport LLC. of North Texas was on site at the North Bridgeport Quarry when they fell while mounting truck cab. Driver injured left foot and suffered abrasion on the back of their head.

While inflating the RRI on a 777 Haul truck, the tire manipulator was holding the tire in place on the rim. While making adjustments to rim components to allow the tire to seat, the manipulator slipped off striking the technician on the right shoulder and pushing EE into the hub where EE sustained a broken right collar bone, shoulder blade and fractured rib.

EE1 was shoveling at the primary plant. As EE1 was lifting the shovel into motion, a rock fell and struck the EE1's left 4th digit finger tip. EE was wearing impact resistant gloves at the time of the incident. EE was later seen at clinic and diagnosed with a fracture to the tip of the finger.

EE's were adding a 90 degree steel elbow to an existing poly elbow. EE#2 operated the crane pulling the elbow towards the other elbow. EE#1 positioned his hands on the elbow flange pushing down to guide the elbow to connection. The choker chain slipped from the original location catching his left thumb between the choker and the metal flange.

IW was removing a carrier roller from a 390 excavator. The track was off the machine. IW struck the roller flange with a metal hammer and this metal on metal caused a piece of metal to chip off and hit IW in stomach.

Employee descending stairs using handrails. Last step was rotted/oxidized and broke when the employee stepped on causing him to be jarred when his weight was thrown forward. Employee maintained an upright position by holding onto the handrails but suffered neck strain.

Employee was walking down a flight of stairs (outdoors) on a catwalk when he felt a sharp pain behind his L knee. Was provided first aid and returned to work on the date of the incident. Since has had trouble and was eventually scheduled for surgery on May 14, 2010. After medical review over the last six months, the meniscus tear has been accepted as a workplace injury.

When trying to position a jack stand to balance the truck properly, the mechanic tugged on the heavy stand to get it in position. When doing this he smashed his finger between the truck frame and the metal stand. Employee took eight stitches to the left ring finger.

Employee was experiencing chest pain, numbness, and tingling in his left arm at start of shift. He attempted to climb on a Caterpillar D10N bulldozer and felt a pain in his left arm. He was taken to the Doctor. Diagnosis concluded ruptured disk in the neck area.

One employee was on a loader using a lubricant. When finished, he tossed the can down to another employee. The can deflected off the employee's hand and hit him below the eye causing a laceration.

EE WAS DRIVING A HAUL TRUCK AND HE BACKED UP TO BE LOADED. THE LOADER OPERATOR'S HAND SLIPPED AND HE HIT THE BOOM LEVER INSTEAD OF THE BUCKET LEVER IN THE LOADER. THE FULL WEIGHT OF THE BUCKET HIT THE TOPSIDE OF THE UNIT CAUSING THE EE TO JARR HIS BACK. JARRING EFFECT SPRAINED THE EE'S LOWER BACK.

EE RECEIVED A FRACTURED TAILBONE WHILE EXITING THE BUCKET AREA CATWALK ON A BUCKET TRUCK. HE HAD BEEN REPLACING LIGHTS IN THE SHOP AND WAS USING THE BUCKET TRUCK. HE HAD TO REPOSITION THE TR UCK AND IN DOING SO, THE TRUCK WAS RAINED ON. BOTH FEET SLIPPED OUT IN FRONT OF HIM AND HE FELL ON THE TRUCK'S TOOLBOX.
