While cleaning material up from a completed job a part that was unsecure moved striking the left middle finger tip causing a laceration requiring stitches.

EE failed to follow known LOTO process for equipment in lab. EE opened doors to the unit reached inside to clean a press head when the machine activated and moved pinching their arm between a fix and moving part, this cause an abrasion to the forearm requiring first aid. 5/2 EE asked to be seen by Dr resulting in restricted duty from Dr.

Employee was exposed to caustic fluid during clearing of a plug and cleaning of the area to their feet when their boots and socks became saturated with the fluid causing chemical burns.

3rd party driver was delivering copper slag to the plant. They had dumped a load of copper slag and was parked. Driver was standing near the rear of the truck and flagged down another driver for help complaining of chest pains. EMS was called, and driver was transported by emergency services to a local hospital where they passed away.

Employee was poking through a poke hole clearing limestone plug, a hinged door was open above poke holes. another employee was hitting on side of chute at same time. The hinged door fell down pinching the employees right pinky between door and bar. this cause a laceration and the nail to be partially removed requiring stitches to be repaired.

EE was coming out of the scrubber when EE's lanyard hook struck EE in the front tooth.

Falling debris in side of cyclone

The worker was removing V-belts from pulley in order to remove a cover to repair a seal on the quarry water pump. When moving a v-belt to a lower groove, it trapped the tip of the glove on the finger area, and when it moved again, pinched the workers finger inside the glove, between the v-belt and the pulley.

At 9:58pm the control room operator received an alarm from the thermal cameras on the shredded tire storage pile showing a potential hotspot/fire. The control room operator dispatched crew members to evaluate the situation, and based on their evaluation, the fire department was called. The fire lasted approximately 31 min, no damage occurred except to the tire pile.

Heavy Equipment Operator was walking around equipment performing pre-operation inspection while EE slipped on an ice patch, due to winter storm. The miner landed on EE's right side, flaring up an old shoulder condition. The injury was first aid until 02/14 when physical therapy was prescribed.

The miner was working on the relocation of a steel hopper on a conveyor belt. The skids of the hopper got stuck on the conveyor belt frame, so the miner approached to inspect when the hopper had a sudden move pinching the tip of EE's left Index finger in between one of the hopper columns and the conveyor structure, causing a cut on the skin of EE's Distal Phalanx.

Maint. employee struck shin on concrete ledge on 1/27/22. Employee had pain off/on for next few months until outage treated with OTC meds/ice. Outage employee walked/climbed stairs a lot, pain still there. MRI on 4/1/22 showed tear in patellar tendon, PT prescribed.

Employee was using prybar to open gate and smashed finger between prybar and gate causing laceration and fracture.

EE tripped over step in walkway causing laceration to left knee. Laceration required stitches to close.

Employee was walking and ducked under I-beam. As they did that, right knee dislocated.

Employee hanging barricades for preheater plug when plug flushed through line, and out cracks in cooler doors. Dust got into EE eyes, causing cornea abrasions in right. Meds prescribed.

L2 pre heater tower tertiary cyclone flushed with hot clinker material into the clinker pit. This caused the grease on the deep bucket elevator chain to catch fire. The fire traveled up the belt conveyor outside of the pit, where flames were seen, and emergency services were called. The fire was extinguished by the local fire department within 34 minutes. No injuries occurred.

DOI 08/28/2020 ee spryd w/hydr oil did not penetrate skin. On 09/02 ee sought med trmt bc of hnd nbness, given 1st prescr. Sent to specialist given 2nd prescr.

Process engineer was looking up at process fan and dust got into left eye causing a corneal abrasion requiring prescription medication.

Employee was pushing on a silicone patch on the bottom of a screw conveyor housing. Silicone was covering a hole. Employee's right index finger went through the hole and made contact with the screw, partially amputating the underside of the index finger under the nail.

Employee got left ring finger caught in between a handrail and forklift fork, causing a fracture and laceration.

The incident that led to the smashed finger occured while the subject was attempting to remove several bolts and leaned a hand against the fan blade and in doing so the fan blade moved because weight was applied to it. Began to free spin and that movement is what caught the subjects finger between the case of the fan and the blade.

EE was leaving work for the day and as they pushed the building door closed, felt pain in their right shoulder. Tried otc and self-care until 12/18/2019 when was seen by dr and given prescription meds.

Employee was shoveling beside and under a ramp. EE pushed the shovel under the ramp and mashed EE's right thumb between the side of the ramp and the shovel. This "strike" broke the employees right thumb. As a result of the break EE cannot use EE's right thumb for a short period.

Employees raising an aerial lift to clean & did not see a conduit bank. Lighting insufficient but were using hard hat lights. Employee struck hard hat on conduit bank causing employee to strike chin on the left side causing a laceration. Employee was transported to hospital & received stitches for the laceration. Employee released to go back to work same day with no restrictions.

Employee was lifting hinged bearing cap on the belt winder. The cap sprang back quickly, smashing the employee's right middle finger. The right middle finger sustained a small laceration and fractured bone.

EE was removing a feed belt. Belt became bound during the removal. EE reached up and grabbed the rigging with both hands to release the tension. The belt clamp ripped out of the belt at this time. The rigging slid through the employee's left hand, pulled gloves off, lacerating EE's left palm, requiring stitches.

Employee was tapping a finish mill liner over with a ball pin hammer when a shard of metal broke off striking ee on right knee, causing a very small cut. EE thought ee was fine but swelling would not go down. Employee was taken for Xray on 12/7 that showed imbedded metal in right knee. Antibiotic given. Ortho referral to remove the metal. Placed on restrictions.

Employee was walking on catwalk underneath the kiln when something blew into EE's eye. Diagnosed with scratched left cornea. Given meds/drops.

Employee's finger was smashed while attempting to re-install a large guard. Right ring-finger tip was amputated from middle of fingernail upwards.

Service line Truck driver lined truck up improperly underneath spout.EE accessed top of truck using safe access platform. When forcing open hatch against the spout, ee slipped. EE struck nose/face against the handrails, causing broken nose/lacerations. Truck company refuses to gain contractor ID number and file injury themselves.

At approximately 3:25pm, a minor explosion was heard/detected in the Line 2 AFR hopper/belt/elevator system. Crew responded and tried to extinguish fire however, they could not safely extinguish the fire. Fire department called by the control room operator, and two fire departments responded (Midlothian//Ovilla). The fire was extinguished approximately 54 minutes after it started.

Employee was installing kiln trunnion bearing, while ee working on the bearing pedestal platform. As ee stepped down from the platform, right knee made contact with a piece of angle iron attached to the platform, causing bruise. Treated with OTC meds until Xrays/MRI taken. Ortho prescribed anti inflammatory and phys therapy on 9/5/17.

Center alternative fuel pile caught fire inside AFR shed for reasons unknown. Subsequent piles inside shed also caught fire. Fire depts called, MSHA hotline called. No injuries.

EE was inspecting dust transport screw. The system went positive as the employee was turning to close the screw inspection port cover back on, covering the employee in bypass dust. Employee suffered chemical burns to both eyes.

Employee performing start of shift checklist. Walked by pre-heater door and door blew open. When door opened dust blew out into employee's face and eyes. Employee was wearing safety glasses.

Pinch between two steel beams while rigging. Amputation of right middle finger tip.

On 6/17/16 we were told the employee worked their normal shift sometime in late May and the following morning their right wrist was sore. Their wrist was not struck by or against any object. Employee has continued to work normal schedule. On 6/22/16 doctor diagnosed the condition as tendinitis and applied a cast.

Employee was installing a 6" water pipe. As the pipe was set into place, both index fingers became caught between the pipe and the stand, causing fractures to both fingers.

Steel erection. Drilling/enlarging flange hole. Drill bit caught in/on the flanges hole material, binding/locking the drill. The rapid twisting motion rotated the drill handle left and shocked the operator's hand consequently forcing loss of hand control. Resulted in wrist sprain and Fx of metacarpal

Employee was installing doors on an airslide in a finish mill building. A gust of wind blew through the building doors, causing dust to get into the employees left eye. Employee suffered a scratched cornea and was given antibiotic and numbing drops.

Right index finger caught between steel members during rigging operations. Pinch point. Worker was wearing gloves. Improper hand placement. Sustained laceration requiring sutures to close. Returned to work without restriction.

The employee was standing on outside of Line 1 Spray Tower & using jackhammer with 12' bit through 3'X3' access door to clean inside the tower. Inside the structure a chunk of material fell on the jackhammer bit causing the bit to force the jackhammer out and down. The jackhammer struck the employee's right ankle causing a fracture.

Laceration under chin - Employee using jackhammer while standing on 24" platform. Stepped back without looking and fell backwards. Let go of jackhammer and the jackhammer struck him on the chin.

Employee was using a jack hammer to clear a large chunk of material. The chunk broke free striking the jack hammer causing it to twist his wrist. Employee thought his wrist was sprained was given first aid and returned to work. He called our office on 08/25/2015 and told us his wrist was not better. We took him to the doctor for an examination and learned that his wrist was broken.

Employee was removing jackhammer from catwalk approx 6ft high. As the employee pulled the jackhammer down to the ground level, the jackhammer oiler struck the employee in the mouth, causing a chipped tooth.

Right thumb caught in grinder. Right nail pulled off during incident. Required two stitches to stop bleeding.

Laceration caused during smash by large metal plate to right pinky finger, palm side.

EE was attempting to climb onto an equipment(TA-40). Weather conditions were icy, wet, and cold. As he attempted to climb on equipment his foot slipped on the stirrup and he caught himself on the rails to prevent him from falling. He heard something pop in his right shoulder.

Employee was returning from getting water in Maintenance Office. Upon re-entering the loader cab he put his right hand where door closes and utilized his left hand to shut the door. He thought he could move his right hand before the door shut. The door shut on his right middle finger. He reopened the door and removed his finger.

EE was assisting another ee in shoveling material from a hopper that had stopped up with material. As he shoveled material he felt pain in his right shoulder.

Employee was building chute and cut their right forearm on piece of sharp angle iron.

Employee was climbing fixed ladder on burner floor to bring down cardox equipment. EE was 4-5 rungs up (4-5ft), when hard hat came off. At the same time, the right foot missed the next rung and the employee fell. The employee tried to stop the fall and fell forward. EE's right elbow and hand made contact with the floor.

Employee was getting off of the Maintenance truck. As he was coming off the truck there was a oil spilled on the bumper causing him to slip and fall to the floor. Fell to the floor on right side of body.

Employee was attempting to loosen liner plate in finish mill using a 8# sledge hammer. Employee struck liner plate and a small fragment of metal spalled from the liner plate and struck employee in the right upper leg. Fragment was removed by emergency room personnel and wound was closed using a single staple. Employee released to full duty without restrictions.

Employee was cleaning shot cast nozzle and material blew into employee's right eye.

Employee was moving metal disk weighing about 300 lbs by rolling it on edge. The disk fell over and struck employee on outside of left foot. Sustained "Jones fracture" to fifth metatarsal.

Employee was attempting to use magnetic block to reset switch on BMH reclaimer, when reclaimer was moved by another employee and caught employee's finger between sensing block and rail block. Right fingernail evulsion and laceration. Employee treated and released. NO LOST TIME.

Zemer maintenance crew was performing a lift of a roll of belt. During the lift miner's hand became entrapped between the lift chain and the roll of belt resulting in an injury to his right thumb.

Employee was pushign brick on conveyor. He trapped his right index finger between two bricks and sustained a crush/pinch injury to the finger tip and nail.

Miner was trimming belt from cable with a knife and cut index finger. Miner was taken to doctor and received three stitches in finger. Miner returned to work the same day and had no lost time.

Employee was cleaning slots in division screen of #3 F/M using jackhammer. Small piece of metal went up under safety glasses sticking him on the right side of nose and a piece of metal ended up in left corner of right eye.

While using a jackhammer in cooler his left hand was on the handrail (scaffolding) and he was pulling back on the jackhammer with his right hand. When the jackhammer came loose the handle pinned his left hand between the handle and the handrail of the scaffolding.

EMPLOYEE WAS ATTEMPTING TO FREE BLOCKAGE IN AIR SLIDE GATE WITH STEEL BAR WHEN BLOCKAGE RELEASED AND GATE SLAMMED CLOSED, TRAPPING HIS FINGER BETWEEN THE STEEL BAR AND THE AIR SLIDE HATCHWAY. HE WAS TRANSPORTED TO A CLINIC AND SUTURES WERE TAKEN TO CLOSE THE WOUND.

EMPLOYEE DROPPED A 5-GALLON BUCKET CONTAINING SAMPLE DISCARDS THAT WEIGHED ABOUT 50 POUNDS. HE TRIED TO CATCH THE BUCKET BEFORE IT HIT THE FLOOR AND IN DOING SO STRAINED HIS LOWER BACK. LATER IN THE DAY HE REQUESTED MEDICAL EXAMINATION. TREATING PHYSICIAN DIAGNOSED A LUMBAR STRAIN AND PRESCRIBED MEDS AND PHYS THERAPY.

Employee was placing liner plates in finish mill using a pry bar. The pry bar slipped and struck him in outer surface of left ear.

A cooler had broken down. They were passing 5 gallon buckets full of material from person to person. EE said he grabbed a bucket and turned too hard to hand it to the next guy. He felt a pain in his right groin area.

Employee was moving a bucket of rock, and lost footing on uneven surface. He twisted his lower back causing a strain.

EE WAS CLEANING TOWER. WHEN HOT BUILD UP PIECES CAME BACK OUT DOOR & GOT IN HIS EYE.

Working on preheater with jackhammer, working in a rush. Poking on preheater and twisted in and upward motion & felt sharp pain on left side.

EE entered the mill chamber to check air quality alarm went off and the EE exited the area and became sick shortly after. EE was treated with oxygen and directed to stay home for 2 days.

Employee was installing overhead grate and stretched a muscle in his upper back. He was placed on restricted duty for three days.

Employee was loading cement into a rail car and fell off the side when his fall protection harness take-up reel locked up unexpectedly, causing him to lose balance. He sustained a laceration to his right thumb requiring sutures and a small cut/contusion to his right cheek.

EMPLOYEE WAS DRILLING HOLES IN A PLATE OF 1/4" STEEL WITH A HAND DRILL MOTOR. THE DRILL BIT BOUND UP IN THE STEEL, CAUSING THE DRILL MOTOR TO KICK BACK, FORCING THE EMPLOYEE'S RING AND PINKY FINGERS BACKWARDS. HE WAS DIAGNOSED WITH A FRACTURE TO THE RIGHT RING FINGER.

EMPLOYEE WAS CLIMBING A LADDER TO CLINKER SILO 4 WHEN HIS FOOT SLIPPED OFF THE BOTTOM RUNG OF THE LADDER. HE TRIED TO CATCH HIS FALL BY GRABBING THE LADDER AND HYPEREXTENDED HIS LEFT SHOULDER, RESULTING IN A ROTATOR CUFF STRAIN.

Employee went to Stage #4 to blast an air cannon. He blasted the cannon and the sample port cap came off and sprayed employee with hot materials.

Employee was working in raw mill with dusty work environment, did not notice any problem until he woke up the next day with irritated right eye. Employee was examined by clinic physician and a dust particle was removed from the surface of the eye.

Employee was operating a jackhammer on the bypass at L2 preheater when he felt a sharp pain in his lower back. He immediately put the jackhammer down and started working with an air lance. The pain continued and employee was taken to doctor.

Employee was walking out of Finish Mill 3. He had oil/grease on his boots. He stepped into a puddle of water and slipped, twisting his ankle. Employee initially refused any medical attention, but decided to go to a doctor when his ankle began to swell on 11/3/05. His doctor ordered that he not wear his safety footwear until 11/7/05, which restricted his normal duty assignment.

Employee was attempting to slide a metal panel when the corner slipped and pinched his fingertip. He reacted by pulling his finger away which caused a laceration on the fingertip. The laceration required two sutures to close.

Employee was installing pump on water truck when pump slipped and struck his left thumb, causing fracture near nail bed.

Employee claims he was using an oxyacetylene torch to cut duct work from top of finish mill. Pieces of hot slag allegedly entered employee's shirt through a gap in the sleeve and caused burns to forearm area. Employee failed to report injury and burns became infected, causing employee to seek medical treatment a week after injury.

Changing bags inside compactment area and hot dust got on arms; sweat from body mixed with dust and caused a burn on arms and hands.

While wearing protective dust suit in 100+ degree weather, employee climbed up/down tank truck 4 times to assist with positioning of truck for loading of CKD. On 4th trip, employee felt nauseous and dizzy.

Employee was leaning over a wheelbarrow cleaning next to a tall pallet of bricks when one of four bricks fell from top of stack & struck him in lower back.

Employee was using a 4' metal bar to poke through the inspection port at the Return Airslide at Finish Mill #2 to clear a material plug when he hit the fifth digit on his right hand on the edge of the port hole opening.

Lifting 20 ft extension ladder, ladder flipped over and employee twisted his wrist.

Employee claims that on or about 5/27/05, he lifted and held a section of grating at the Line 1 Scrubber Pit with his left arm while cleaning out the material with his right hand and arm. During this process, employee claims he felt a sharp pain in his left arm. Employee did not report injury until 6/29/2005.

AT APPROX. 9:20 AM ON MAY 20, 2005, AN EXPLOSION WAS HEARD COMING FROM LINE NUMBER ONE. THE DEFLAGRATION WAS OBSERVED TO BEING AT THE ELEVATOR BOOT & CONTINUED TO THE TOP OF THE ELEVATOR. THIS EXPLOSION WAS CAUSED BY THE IGNITION OF A DUST CLOUD INSIDE THE ELEVATOR.

Employee was shoveling material out from under 311-BC1 belt conveyor when he claims he struck a secured bolt with his shovel, causing his right wrist to hyperextend. Employee did not report injury until he felt initial pain on 05/04/2005.

Employee was working inside the raw mill roll removing outer bearing race using a wedge and hammer. Employee claimes he was driving the wedge between the bearing race and cage when a piece of race came out and struck him in right thigh.

EE was at bottom of bypass dust silo performing inspection on 2 bypass dust extraction systems. EE thought isolation gate was closed. EE claims he opened port downstream of gate on airslide to verify gate was closed. When EE opened port on airslide, material immediately began flushing from silo. EE inhaled bypass dust during incident.

Injured employee & co-worker used strap attached to forklift to lower Pfeiffer grinding segments into open box. While lowering segment, strap stuck on front of one fork. Injured employee claims that when he used his right hand to pull strap off fork, segment weight shifted in box applying a load on strap causing strap to pinch employee's finger between strap & fork.

Employee claims he was getting set up to cut wood on the welding table in the maintenance shop. When he set circular saw on welding table, an 8' x 2', 1/2" thick piece of steel that was sitting on the welding table fell on the top of his left foot. The piece of steel was hanging off welding table and the weight of the saw was enough to drop steel off table.

EE WAS CLEANING BYPASS AREA. JACKHAMMER GOT STUCK IN BUILDUP. WHEN EE PULLED ON THE JACKHAMMER, THE LOCKING LATCH THAT HOLDS THE BIT OPENED AND THE BIT FELL OUT OF THE JACKHAMMER AND STRUCK THE TOP OF EE'S RIGHT HAND.

Employee claims he was avoiding a mud puddle as he was walking through the RMS building and ran into a fixed pipe protruding from the south side wall. The pipe hit his upper left front tooth causing 1/3 to break off.

Employee claims that while air lancing sand silex, he got sand in both eyes.

Employee claims he injured his upper back and neck while shoveling rock around the 311-BC1 belt. Employee reported the soreness to his supervisor on 3/26/2005 and requested medical attention on 4/04/2005.

EE shoveling off a catwalk. Attempting to move an iron plate covering hole in catwalk and fell through hole, approx. 14 ft to pavement.

Climbing out of roll crusher holding on to a brace going across roller when brace slid off the side & EE fell backward onto roll crusher.

EE WAS CLEANING UP IN THE KILN WHEN HE RECD. SOME TRASH FROM HIS COWORKER WHICH WAS BEING PASSED UNDER THE KILN RIG WHERE A PALLET OF BRICKS WERE. THE TRASH THAT WAS BEING PASSED TO EE SNAGGED ON A BRICK CAUSING IT TO FALL APPROX. 4 FT., STRIKING EE'S RT. FOOT CAUSING HIS 4TH DIGIT TO RECEIVE A LACERATION AND A FRACTURE. EE IS STILL UNDERGOING CARE WITH ORTHOPEDIC SPECIALIST.

Working around mill & wind blew dust up & into his right eye.

Working on a piece of rebar, trying to bend the rebar & employee's rt. leg moved & twisted ankle.

Employee was working on removing plugged gate when wind blew dust into his right eye.

Bent over to pick up jackhammer & bit. When he raised up he felt a sharp pain in his side.

He was working on the tower using a jack hammer and a bar. the bar was inside the door on the tower and he reached for the jackhammer to put it on the bar and he felt a pain in his back.

WHILE JACKHAMMERING A WATER LINE BROKE, EE FELL BACKWARDS WHILE AVOIDING PRESSURIZED WATER. RT. HAND CAME DOWN ONTO THE CUT CONCRETE SPLITTING THE SKIN IN 2 PLACES.

STABLIZING A HOSE WITH HIS FOOT, THAT WAS USED WITH THE VACUUM TRUCK. THE HOSE MOVED QUICKLY & THREW HIM OFF BALANCE WHICH HURT HIS BACK.

EE was working on the feedshelf of the preheater and turned to quick. when he turned it pulled his back on the left side.

WHILE FIRING THE CANNONS ON LINE #2, EE HAD TO LOOK UP AND REACH OVERHEAD. WHEN ONE OF THE CANNONS FIRED IT DISLODGED LOOSE MATERIAL THAT FELL INTO EE'S LEFT EYE.

EE WAS CUTTING WITH A TORCH & WAS HOLDING TORCH AT HIS SIDE. ANOTHER EE CLIMBED THRU SOME BRACING & HIS LEFT ARM CAME INTO CONTACT WITH LIT TORCH.

EE WAS POKING OVERHEAD WITH AIRLANCE AND CHUCK BROKE LOOSE & HIT HIS RIGHT KNEE.

LABORER WAS USING AN 8' JACK HAMMER. AS HE TURNED TO HIS RIGHT SIDE HE FELT A PAIN.

EE WAS CLEANING TAKE OFF DUCT. IT WAS FULL OF BUILD UP SO IT BLEW OUT AND COVERED HIM ON HIS GLOVES & COAT. THE MATERIAL GOT IN HIS GLOVE & BURNED HIS RIGHT HAND.

EE was pulling rope through conduit. EE had arm extended and rope caught on bolt and EE arm was overextended.

EE WAS CHECKING THE TOWER AND THE AIR CANNON WENT OFF - CAUSING HOT MATERIAL TO FLY OUT AND BURN HIS FACE & CHEST.

WORKING ON CUTTING UP A BIN. LOOKED UP IN THE BIN AND SOME DEBRIS FELL DOWN AND INTO EE'S RIGHT EYE. EYE PROTECTION WAS IN PLACE. HARD HAT WAS IN PLACE.

EE WAS WORKING IN THE KILN. HE HAD BEEN ASKED REPEATEDLY TO MOVE BACK FROM THE KILN RIG WHILE THE BRICKS WERE BEING KEYED IN AGAINST THE RETAINER RING. TWO BRICKS CAME LOOSE FROM THE RING, ONE STRIKING EE ON THE SHOULDER AND ANOTHER HIS LEFT INDEX FINGER-CRUSHING IT. EE IS STILL UNDERGOING CARE.

THREE WORKERS WERE REMOVING A FINISH MILL SHELL DOOR USING AN OVERHEAD CRANE TO SECURE THE DOOR ONCE THE CRABS HOLDING IT WERE REMOVED. AFTER EXERTING PRESSURE ON THE DOOR SO THAT IT WOULD FAL L A FEW INCHES INTO THE MILL, ONE WORKER GOT TOOCLOSE TO THE CRANE SHEAVE, WHICH HIT HIS SHOULDER. HE LOST HIS BALNCE & FELL HEAD FIRST 4-5' UNTIL CAUGHT BY HIS LANYARD: STRUCK PROTUDING NUTS

MOVING PLATES INSIDE THE MILL AND GOT FINGER PINCHED BETWEEN THE PLATE AND THE WALL. 5TH FINGER FRACTURE (RIGHT SIDE).

EMPLOYEE WAS LIFTING A LARGE PIECE OF SHALE FROM CONVEYOR BELT (320 RE1). WHEN HE TURNED TO LAY IT DOWN, HE STRUCK A MOTION SENSOR CAUSING A PUNCTURE TO THE CENTER OF HIS RIGHT SHIN.

EE WAS DIGGIN IN A DITCH AND PICKED UP A ROCK TO THROW IT OUT OF DITCH. HE TURNED WRONG AND STRAINED LOWER BACK.

EE WAS GRINDING ANGLES WITH HAND GRINDER. ALL PPE WAS WORN. WHEN EE COMPLETED TASK, HE REMOVED FACE SHIELD AND SOME METAL SHAVINGS FELL BEHIND SAFETY GLASSES INTO LEFT EYE.

EE WAS USING AN OXY/ACTY TORCH TO CUT HOLES IN BRACING BEAM WITH ALL PROPER PPE. EE BENT OVER TO OBSERVE HOLE AND A PIECE OF SLAG FELL OFF BEAM INTO LEFT EAR. EE STATED THAT HE WAS OK AND DID NOT NEED MEDICAL TREATMENT. 13 DAYS LATER THE EE REPORTED AN INFECTION IN THE LEFT EAR AND WAS SENT TO DOCTOR FOR EVALUATION.

USING JACKHAMMER TO REMOVE BUILD UP IN THE CHUTE BELOW THE LINE #1 RAW MILL TRIPLE GALES. WHILE TRYING TO PULL THE JACKHAMMER OUT OF THE CHUTE A CHUNK OF MATERIAL FELL FROM ABOVE HITTING THE J ACKHAMMER BIT CAUSING THE JACKHAMMER TO FLY UP & STRIKE HIM IN THE MOUTH.

EE WAS USING A GRINDER TO SMOOTH OUT 1/8" STEEL PLATE. PLATE CAUGHT IN GRINDER AND PULLED LEFT THUMB INTO GRINDING WHEEL CAUSING SKIN ABRASION AND PARTIAL LOSS OF THUMB NAIL.

EE WAS GRINDING IN WELDING BAY IN THE SERVICE BUILDING WHEN HE FELT STING (THOUGHT IT WAS FIBER FROM WIRE WHEEL).

EE RELEASED THE HAND BREAK BETWEEN RAIL CARS. THE BREAK HANDLE OR CHAIN SWUNG BACK AND HIT HIS HAND.

EE WAS CLEANING CHUTE. HE LOST HIS FOOTING AS HE WAS ENTERING THE CHUTE AND HIS LEG HIT THE EDGE, CAUSING LACERATION.

SEVERAL MEN (ABOUT 10) MANUALLY MOVING A RAMP INTO PLACE. HE FELT STRAIN IN BACK.

CLEANING A GATE WHEN IT ACTIVATED THE BAR AND JACKHAMMER AGAINST HIS LEG PINNING HIM.

HOLDING REBAR DOWN WHEN ANOTHER EE MASHED HIS LITTLE FINGER BETWEEN CONCRETE & HAMMER.

PULLING STAKES LOOSE AND NAIL PENETRATED SHOE AND WENT INTO FOOT.

PLANT MGR WAS A PART OF IEEE CONFERENCE TOURING THE HOLCIM MIDLOTHIAN PLANT WHEN EE STRAYED FROM THE GROUP TO LOOK AT A FLY ASH FEEDER UNDER AN INTERSTICE SILO AND STEPPED OFF AN ELEVATED PLATFORM AND FELL APPROX 42" TWISTING LEFT KNEE. SURGERY WILL BE REQUIRED & THERE IS CONCERN FOR A PRE-EXISTING BLOOD DISORDER

WAS AT TOP OF BLENDING ELEVATOR, WALKING DOWN STAIRS AND TWISTED HIS LEFT ANKLE.

EE WAS ON THE 3RD STEP FROM THE BOTTOM OF A 10' EXTENSION LADDER. HE WAS REACHING UP TO CLOSE A 2"GATED AIR VALVE. USING A 12"WRENCH TO CLOSE VALVE TIGHT, VALVE BONNET CAME OFF, RELEASING THE PRESSURE STORE IN APPROX. 30'OF 2"HOSE. EE WAS STRUCK BY WRENCH OR VALVE BONNET AND FELL FROM THE LADDER. HE TRIED TO BREAK HIS FALL WITH HIS HANDS AND FRACTURED BOTH WRISTS.

EE WAS RACKING IN A 5KV SWITCH TO FINISH MILL AUXILLARY DRIVE. DUST IN THE SWITCH HOUSING MUST HAVE BEEN DISTURBED, CAUSING AN ARC ACROSS CONTACTORS. THE SWITCH EXPLODED OPENING THE DOOR & HIT TING THE EE'S HAND. THE EE FELL BACKWARD AND STRUCK HIS HEAD GETTING A SLIGHT CONTUSION.

EE WAS SIGNALING THE TRUCK CRANE WHILE IT WAS LIFTING GEAR GUARD FOR KILN DRIVE GEAR LOOSE DUST AND OTHER PARTICLES FELL INTO EYE AS THE WIND WAS BLOWING. THE EE WAS WEARING SAFETY GLASSES.

LIFTING WELDING LEADS TO CARRY ON SHOULDER AND FELT A POP AND SOME PAIN BETWEEN SHOULDER BLADES.

HE WAS WORKING ON STAGE 4 CONE CLEANING SANDY MATERIAL WHEN A PIECE OF HARD BUILD-UP FELL FROM UPPER STAGE AND HIT HIS HEAD.

WHILE CLOSING THE MILL REJECT DOOR, FELT SHARP PAIN IN BACK. DID NOT REPORT THE MUSCLE STRAIN UNTIL AFTER SURGERY.\

INSTALLING FIRST LEVEL OF NEW STRUCTURE AND REACHED OUT TO STABILIZE LOAD. FELT A TWINGE IN NECK AND NECK GOT STIFF OVERNIGHT.

AN EXPLOSION OCCURRED IN THE LINE 1 PREHEAT TOWER ALTERNATE FUEL ELEVATOR UPON RESTART FROM MAINT. NO ONE WAS INJURED IN INITIAL BLAST. EXPLOSION VENTING OPERATED AS DESIGNED. ELEVATOR WAS SHU T DOWN & WATER APPLIED FROM FIXED SPRAY SYSTEM. RESIDUAL FIRE WAS NOTED IN SEVERAL BUCKES & ELEVATOR WAS RESTARTED TO CLEAR MATERIAL & EXTINGUISH AT BASE OF ELEVATOR. EE WAS TOO CLOSE TO OPEN

CLEANING RETURN ROLLER ON CONVEYOR BELT, HE GOT DUST IN EYE.

EE WAS SHOVELING COAL AND DIRT WHEN HE FELT A PAIN IN HIS UPPER BACK.

EMPLOYEE DISREGARDED DIRECTIONS AND REACHED INTO OPERATING CONVEYOR TO CLEAR MATERIALS. HIS HAND CAUGHT BETWEEN BELT AND ROLLER.

EE WAS LAYING ON HIS SIDE IN THE SHOP TO CUT MATERIAL WITH OXY/ACE TORCH. HE WAS WEARING A WELDING HOOD. MOLTEN MATERIAL SPLATTERED AND RICOCHEYED OFF ADJACENT SURFACE AND FELL INTO HIS LEFT EAR.

ELECTRICIANS WERE INSPECTING A THERMOCOUPLE ON THE LINE 1 ALKALI BAGHOUSE INLET. THEY DESCENDEDA LADDER TO REACH THE THERMOCOUPLE. WHILE WALKING ALONG THE WALKWAY TO THE THERMOCOUPLE, EE WAS EXSPOSED TO A COUPLE OF "PUFFS" OF BAD SMELLING HOT GAS. BAGHOUSE AND DUCT IS NORMALLY UNDER A SLIGHT VACUUM. HE FELT NO DISCOMFORT AFTER THE EXPOSURE, BUT REPORTED IT TO HIS SUPERVISOR.

THE EE WAS WELDING OVERHEAD WHEN A HOT BEAD OF METAL WENT INTO HIS LEFT EAR AND BURNED THE INNER CANAL BACK TO THE EARDRUM.

EE USING SLEDGEHAMMER TO REMOVE PINS FROM HOPPER DISCHARGE CHUTE. TEMPORARY PLATFORM SHIFTED PUTTING HIM OFF BALANCE. AS HE ATTEMPTED TO STOP HAMMER IN MID-SWING, HE PULLED A MUSCLE IN HIS SHO ULDER.

PROD HELPER WAS RACKING OUT THE MATERIAL FROM THE PYROCLONE ELBOW INTO THE DISCHARGE CHUTE. FALL MATERIAL PUFFED OUT OF CHUTE. WINDY CONDITIONS BLEW THE DUST UNDER THE FACE SHIELD AND SAFETY G LASSES.

LINE 1 SCRUBBER TOWER WAS PROPERLY ISOLATED FROM PROCESS GAS (KILN EXHAUST GAS) IN PREPARATION FOR OPENING THE MANDOORS TO VISUALLY INSPECT THE DEMISTING PADS FROM THE OUTSIDE OF THE VESSEL. THERE WAS QUITE A BIT OF STEAM COMING FROM THE MANDOOR. THE UPPER DAMPER WAS OPENED IN A EFFORT TO CREATE A DRAFT AT THE DOOR AND CLEAR THE STEAM, SUFFERED ASTHMA ATTACK.

EE WAS GRINDING W/4" GRINDER THAT HAD A WIRE WHEEL ATTACHED. EE WAS WEARING A PLASTIC PONCHO TO PROTECT FROM WEATHER. GRINDER GOT CAUGHT IN PONCHO & EE LOST CONTROL CAUSING GRINDER TO STRIKE H IS CHIN. THE FACE SHIELD HE WAS WEARING PREVENTED MORE SERIOUS INJURY.

EMPLOYEE WAS USING A PIPE WRENCH TO LOOSEN A BOLT. HE WAS PUSHING HARD ON THE WRENCH WHEN HE FELT A "POP" IN HIS BACK.

EE DOES NOT RECALL WHEN HE BUMPED HIS LEG. HE NOTICED SWELLING ABOUT 2 HOURS BEFORE HE REPORT-ED THE INCIDENT.

TWO EMPLOYEES WERE CARRYING A METAL PLATE. WHEN THEY LOWERED IT TO THE GROUND, ONE LET LOOSE OF THE PLATE AND THE OTHER DID NOT GET HIS HAND OUT QUICKLY ENOUGH, TRAPPING HIS LEFT THUMB BETWEEN THE PLATE AND METAL DECK.

WHILE MOVING RAILCARS, RAN INTO A RAILCAR THAT EE WAS ON. INJURED HAD JUMPED OUT OF THE WAY OF THE ONCOMING RAILCARS AND WAS HOLDING ONTO THE LADDER WHEN THE CARS COLLIDED. THE COLLISION CAUSED INJURY TO HIS SHOULDER, NECK AND BACK.

EE WAS INSPECTING THE AIR VALVES ON STRING OF 8 CEMENT CARS BEING FILLED WITH CEMENT IN SILO 3. WHILE HE WAS INSPECTING THE CARS. THE LINE OF CARS WAS STRUCK BY CARS BEING DEILVERED BY BNSF. E E JUMPED CLEAR OF THE CARS, LANDING ON THE GROUND.

EE WAS SITTING IN TRACKMOBILE, ATTACHED TO A STRING OF 8 CARS. HE WAS MOVING THE CARS SOUTH WARD ON TRACK 3 AS THEY WERE FILLED. WHILE HE WAS WAITING TO MOVE THE CARS, THE LINE OF CARS WAS STRUCK BY CARS BEING DELIVERED.

EE WAS USING JACKHAMMER TO CLEAR PREHEAT TOWER ON LINE #1 WHEN HE FELT A STRAIN IN HIS LOWER BACK.

EMPLOYEE WAS HOLDING BIT AND JACKHAMMER IN HANDSWHILE CHANGING THE BIT. AS HE WAS CONNECTING THEBIT, HE TWISTED AND FELT A SHARP PAIN IN HIS BACK.

EE WAS LIFTING A BUCKET OF WATER TO DUMP IN THE SINK WHEN HE FELT A STRAIN IN HIS BACK.

EE WAS WALKING ALONG ROAD IN PROCESS AREA & WAS STRUCK BY A GOLF CART COMING AROUND A CORNER.

EE RAISED HIS LEFT HAND TO PROTECT HIMSELF FROM BEING STRUCK BY A FALLING AIR LANCE (10' LONG, 1INCH STEEL PIPE). HIS LEFT THUMB WAS BENT BACK, STRAINING THE MUSCLES IN THAT AREA.

EE WAS WALKING IN THE FINISH MILL BUILDING WHEN HE STEPPED ON A ROCK AND TWISTED HIS KNEE.

EE COMPLETED LOADING RAILCAR AND WAS TAKING PHOTO OF CAR A TOP. WHILE FOCUSING CAMERA, EE MOVED TO GET A BETTER PICTURE. HE STEPPED ON WALKWAY AND TWISTED HIS RIGHT ANKLE. THERE WAS SUFFICIENT LIGHT AND EE WAS WEARING SAFETY.

EE WAS WORKING ON A CATWALK. STEPPED BACKWARD OFF A STEP TOT HE LOWER CATWALK 10 INCHES ONTO SMALL PIECE OF REFRACTORY AND TWISTED HIS LEFT ANKLE.

EMPLOYEE WAS ATTEMPTING TO LATCH THE BELT COUNTERWEIGHT IN THE UP POSITION WHILE THE BELT WAS BEING ADVANCED. HE PINCHED HIS RIGHT THUMB BETWEENTHE COUNTERWEIGHT AND THE CLAMP.

EMPLOYEE JAMMED LEFT WRIST WHEN HE PULLED A LEVER UP TO RELEASE TRAIN COMPARTMENTS. EE HAD NO WORK RESTRICTIONS UNTIL 3/1/01.

EE WAS OPENING RAILCAR DOORS WITH A SLEDGE HAMMER AT THE GYPSUM PIT WHEN HE NOTICED PAIN IN HIS NECK AND LEFT SHOULDER.

EE WAS A PASSENGER IN THE #4100 TRACKMOBILE THAT WAS BEING DRIVEN BY ANOTHER EE WHO WAS DRIVING IN REVERSE AND DID NOT SEE #4000 TRACKMOBILE THAT WAS STOPPED ON ADJACENT TRACK NEAR SWITCH. DRI VER TRIED TO STOP BUT COLLIDED CAUSING PASSENGER EE TO INJURE HIS CHIN.

EE WAS A PASSENGER IN THE #4100 TRACKMOBILE THAT WAS BEING DRIVEN BY ANOTHER EE WHO WAS DRIVING IN REVERSE AND DID NOT SEE #4000 TRACKMOBILE THAT WAS STOPPED ON ADJACENT TRACK NEAR SWITCH. DRI VER TRIED TO STOP BUT COLLIDED CAUSING PASSENGER EE TO INJURE HIS RIGHT WRIST.

EE WAS DRIVING #4100 TRACKMOBILE IN REVERSE AND DID NOT SEE #4000 TRACKMOBILE STOPPED ON ADJACENNT TRACK NEAR SWITCH. EE TRIED TO STOP BUT COLLIDED CAUSING EE TO BRUISE RIB.

EMPLOYEE WAS HOLDING ON TO A SHACKLE TO STABILIZE LOAD THAT WAS BEING LOWERED WHEN LOAD SHIFTED CATCHING THIRD FINGER BETWEEN THE SHACKLE AND LOAD.

EE WAS WALKING CATWALK FROM TOWER 1 TO BLENDING SILO WHEN HE SLIPPED ON SLOPED GRATING INJURING HIS LEFT WRIST AS HE TRIED TO CATCH HIMSELF. WEATHER CONDITIONS: ICY.

EMPLOYEE WAS STANDING ON WEST SIDE OF BY PASS DUST BIN WHEN A STRONG GUST OF WIND BLEW FINE WOOD CHIP PARTICLES INTO RIGHT EYE. APPROX 40 MPH WIND GUST.

EE WAS CLEARING A PLUG ON STAGE IV VESSEL AND DUST BLEW INTO LEFT EYE.

WORKING ON TEBTITUARY INLET DUCT CLEANING BUILDUP FROM DUCT, DUCT BECAME POSITIVE, BLOWING HOT AND FLAMES FROM CLEANING PORT.

EE USED RIGHT HAND TO PUSH DOWN HUB HANDLE ON GATE IN SILO #5 WHEN HAND SLIPPED CAUSING CUT TO THIRD DIGIT ON RIGHT HAND.

EMPLOYEE TRIPPED OVER SLIDE DOOR ON ELEVATOR (LINE #2 PREHEATER TOWER) FALLING TO HANDS AND KNEES. STRAIN TO BACK. MODIFIED DUTY.

DUST ACCUMULATED ON THE BRIM OF EMPLOYEE'S HARD HAT & BRIM OF SAFETY FACE SHIELD BRACKET. WHEN HE LOWERED THE FACE SHIELD, DUST FELL INTO HIS EYES.

EE GOT DUST IN RIGHT EYE WHILE INSTALLING GEAR BOX AT #1 FINISH MILL. PRESCRIPTION

EE FELL OUT OF GOLF CART THAT WAS BEING DRIVEN BY ANOTHER EE CAUSING LACERATION TO LEFT ELBOW CONTUSION TO RIGHT HIP AND LOWER LEFT SHIN.CONTRIBUTING CONDITIONS WERE UNEVEN ROAD SURFACE, LOOSE GRAVEL, SHARP TURN AND TRAVELING TOO FAST. MODIFIED DUTY.
