Employee was sorting and stacking Pump hoses and fittings onto a pallet. Employee lost their balance and fell backwards onto tongue/hitch area of a cargo trailer. Employee hit their back on the tongue/hitch area of the trailer.

EE1 put bolt-inner bearing housing. EE2 loosened outer fastener;bearing house pulled out.EE1 said stop;EE2 re-tightened fastener. Bolt in-place for alignment of shaft/flange. Vibration/movement from impact wrench may have let shaft spin on bearing. Pendulum effect occurred;paddles contacted EE1. EE exited tub w/o help; walked 300' to truck. EE1 stated legs hurt;driven to hospital.

I was traveling west on the haul road with a load of track pads for the dragline in the bucket of the loader. The pads fell out and I ran over them bouncing me up and into the ROP cutting my left eye brow and the back of my head. I was taken to the ER and the cut above my eye was glued shut. The one on the back of my head was ok.

The employee was changing a screen on a portable shaker plant. He lifted a screen, the screen slid on the frame of the plant. He tried to catch it and twisted his body injuring his lower back.

The employee and a contractor were bradding a bolt that holds the track pin in on the dragline. The employee was holding a hammer on the pin and the contractor was hitting it with another hammer. The contractor hit the side of the machine and struck the employee on the left little finger breaking it.

Employee was lifting a converter chain for a dragline. He felt a sharp pain in his back and left leg. We took the him to the hospital. He recieved two shots Demerol, and Phenergan. He was also given two percriptions Cyclobenzaprine, and Hydrocodone. The employee was placed on light duty.

Employee was in the process of changing a drag cable on the dragline #160. He was working between the two drums installing a wedge. When he went to climb out from between the two drums he pressed down on the lagging to lift himself out and a sharp edge on the lagging cut through his gloved hand and made a one inch laceration on his right hand. He had to receive stitches.

Employee was walking on the sidewalk going to the QC lab when he tripped over some metal edging on the corner of the sidewalk. This caused the employee to land on his left hip. The employee broke his left hip.

Employee was stepping over the 902 freshwater pump in order to get to the 901 freshwater pump to get it started. He stepped over the pump and shifted his weight onto his left leg. He lost his footing, his left foot slipped out from under him. When this happened he lost his balance and fell to the ground striking his right knee.

EMPLOYEE GOT OFF OF HIS LOADER TO RETRIEVE A PIECE OF METAL THAT HE SAW IN THE MATERIAL BEING LOADED. AFTER HE RETRIEVED THE PIECE OF METAL HE WAS DESCENDING DOWN THE PILE OF MATERIAL. HE LOST HIS FOOTING AND HIS FOOT FOLDED UP BEHIND HIS LEG THUS CAUSING HIS KNEE TO POP. PAIN WAS FELT IN THE KNEE WHILE WALKING.

Employee was doing a walk-thru inspection when he came upon a recent gravel spill on a catwalk. He began cleaning the catwalk and then decided to finish his inspection and return later to finish cleaning the spill. As he continued up the catwalk he twisted his knee and ankle on a rock that was on the catwalk. He sustained a sprained right knee and ankle.
