EE STATES HE SLIPPED ON WET GROUND WHILE ENROUTETO PUMP LOCATION & SHIFTED HIS WEIGHT TO MAINTAIN BALANCE. AT THIS TIME HE FELT A SLIGHT PAIN IN LEFT KNEE. NOT FEELING IT WAS SEVERE HE CONTINU ED WITH HIS TASKS. UPON REPORTING HE DECLINED TREATMENT. STATED ON 6-13-02 PAIN STILL OBSERVED, TREATMENT INITIATED. PLACED ON RESTRICTIONS 6-27-02.
