May 9, 2024 at 0800 hours when a manlift operator ran EE's manlift, an 8008 JLG, serial number 0300310942, into the canopy covering the C35 conveyor belt coming out of C35 tunnel. The operator was pinned between the roof edge and the man basket hand rail.

EE was taking door off coal hopper when they removed top bolt the bolt on the bottom of the door was loose causing the door to swing down onto the thumb

EE was repairing a hinge on a metal storage box and felt discomfort in left shoulder area. Clinic was called and EE opted for self-care. On 07/15/2024 EE was seen by a medical professional and was returned to work with no restrictions and continued self-care. On 07/22/2024 EE returned for a follow up visit and was given restrictions and scheduled for an MRI.

At appromently 8:30 an employee was using a chop saw in the maintenance shop when the ee notice a fire on the wall. The employee extinguished the the fire and continued to work, at 8:47 the employee noticed the fire had reingnited. MSHA was called at 9:12

Employee was driving John Deere Gator from parts room. EE said had a fitting in EE's right hand. The fitting fell and EE bent over to pick it up from the floor board while driving over covered drainage ditch. Per employee the "bump" from driving over drainage ditch cover and EE bending over caused EE's back to jar.

On 3/9/2021 the miner was prying a log out of the cooler discharge EE felt discomfort in EE's right lower back. The miner reported this to EE's supervisor and stated that EE didn't want to go and have it checked out. On 3/16/2021 the miner reported to EE's supervisor that the pain had increased over EE's long change off and would like medical attention.

While cleaning raw material build up from feed hopper skirt board with a scraping bar the employee attempted to strike an over size chunk of material and smashed right index finger and thumb on the corner post of the hand rail.

Employee was replacing an A/C blower motor at MCC-3. While removing electrical tape and zip ties with a knife the knife slipped lacerating the employee's left thumb.

On 1/12/16 employee was cleaning up a material spill. EE thought EE had pulled a muscle in EE's stomach. Employee reported the injury to EE's shift supervisor. However, the supervisor did not report the injury to the plant manager. On 1/25/16 employee went to EE's primary doctor. The doctor informed the employee that EE had a hernia.

Employee was using a cutting torch to cut a return roller bracket off the C-51 conveyor system. He moved his left hand in front of the torch, burning himself. Employee reported to leadman but the leadman did not report to plant manager. Therefore plant manager did not tell safety department until 3/16/15.

EE's were installing a dogbone, the dogbone slipped off the shaft smashing hand of other EE. The bone slipped of the cooler side shaft. The injured employee reached up to keep the bone from striking another employee in the head, when he did, it caught his hand in between dogbone and chain.

Employee was lifting a heavy equipment tire that was mounted on a dayton rim from the ground. The tire was lying flat on hard ground. Employee was lifting one side of tire to get it vertical to roll to the mobile shop when he felt a twinge in his groin area.

Employee received a call that work on C-7 was completed. Due to excessive noise during the call, he moved to the platform on south side of cooler to hear better and due to poor lighting, tripped over the dam sections in center of platform, falling on his right shoulder.

Employee had his annual HCI hearing test here at the Plant on 4/26/12. The results showed a STS had occurred and that the STS met the criteria for a recordable shift on Form 300. We sent him for a retest to our company doctor on 10/2/12. We did not know that this needed to be filled out on 7000-1. This was informed to us on 11/7/12 during a MSHA Plant Inspection.

Employee on 3/13/12 approximately at 08:00 AM was preparing ls160 bobcat tire and rim for repair. He lifted the rim on to work table and picked up a 4 lb. sledge hammer to repair bent rim when he felt a sharp pain in his mid back area.

Employee was checking the water pump for refueling. When he came up on the pump he noticed the gas cap was off. As he was trying to put the cap back on the pump caught fire. Employee ran to the nearest fire extinguisher and as he was running back up a sloped area he felt something give in his right calf.

IE was working along with another EE to increase production of our precoat material. While catching several samples, IE picked up bucket of material with both hands and set it down above the scales. He then picked bucket back up and started pouring the rock and felt a twitch in his left shoulder.

EE was using a 4 lb. shop hammer and a 1 3/4" combination wrench to loosen nuts on coal mill bowl.EE struck the wrench with the hammer and this caused the wrench to bounce up and strike EE on top of his left hand.

EE's were cleaning the raw shale buildup from C-34 tripper chute. While cleaning the chute, IE was using a pneumatic chipping gun and when IE tried to change his footing he slipped and hit his left calf muscle on the inside of the chute.

EE removed troughing idler from frame of C-13 and carried it about 40 feet. When he lifted the idler over inside of handrail to throw to the ground, he felt a tearing pain in his right shoulder.

At approximately 3:30 PM on 8/6/10 EE & co-worker were assigned the task of cleaning of SC#1 belly pan. EE was working at removing buildup of raw shale from the upper section of the belly pan with a pneumatic chipping gun when a large piece of raw shale broke loose and slid down the sloped belly pan landing on the top of his right foot.

At approximately 9:00 PM on 4/14/2010 while working at the emergency hopper, employee was using a 12 ft. long, 1/2" diameter air lance to help wet raw shale from sticking to the hopper walls. Employee was stretched out pulling on the lance when he felt a sharp pain in his lower back.

At approximately at 8:00 AM on 2/5/2010, Injury was pulling auger sections out of the extruder along with #1 EE, #2 EE and #3 EE. Injury was taking the augers and tossing them on the ground. He had tossed 3 augers and switched places with #2 EE. He started feeling discomfort after 10 minutes of switching with #2 EE.

Employee was in the quarry pit area setting off a shot and there was flying rock from the shot. EE was running to get away from flying rock when he slipped and fell due to the ground being wet from rainy conditions. There is usually never any flying rock from shots, but the wet conditions hindered the shot. EE suffered a strain in his left upper leg.

EE was air-arc gouging on frame damage to a scraper. The area was very small and at or above head level causing his head to be surrounded by billowing air, fumes dust and grit. EE felt that he had gotten metal in his eyelashes and then in to his left eye. The first real discomfort he felt was on Saturday, 5/2/09.

Injury was cleaning Screen #6 on a very windy afternoon. Material blew in his right eye. His eye began to bother him on his way home. He reported this the next morning on 4/7/09 to his supervisor. He was sent to company doctor and 1 stuck piece of material and 1 loose piece of material were removed.

Injury was at the coal unloader placing the vibrator on the railcar. He was using a ratchet and pipe to tighten up bolts, while performing this job, he complained that his right shoulder was hurting and could feel a burning sensation in it.

While dismounting Haul Unit #157 from the left side door, the wind closed the door on injured's right hand before he could remove his hand out of the way. The wind was gusting about 15-20 MPH from the north. When he caught his hand in the door it caused partial amputation of his right pinkie finger.(Per call-no bone that means no amputation)

EE was walking out of the loadout building when he stepped on a grain scoop with the front part of his right foot. Stepping on the scoop bent his toes and his foot had some pain. EE reported this incident to his supervisor the next morning, but thought that his foot would get better. EE came to supervisor on 7/17/08 and said he needed to have his foot checked by a physician.

Employee was loosening motor mounting bolts on Triple Roll Crusher using a socket and 3/4" ratchet. He exerted force and didn't remember any type of slipping or jerking, but began to feel a pressure in his back.

EE was helping Maintenance work on C-34 conveyor belting when his left hand was pulled in to pulling block between 3/8" cable and sheave. The cable tightened causing his left hand and fingers to be pulled in to block, lacerating and breaking his middle finger and parital amputation to his pointer finger. (first joint)

EE was working 12 hours on the loader & operator fatigue could have caused him to not have proper posture while running the loader. Cause of injury is unclassified because of insufficient data, but he said his right shoulder was hurting him. EE reported to supervisor and decided on 7/14/08 that he needed to go to doctor to see what was wrong with his shoulder.

EE was welding a patch above the jaw crusher and he drove a piece of 4" channel iron into the gap between the jaw Pittman and the hopper above the serve as a seat. When repositioning his body, the channel slipped out allowing him to fall into the jaw crusher, catching his fall with his left hand, wrenching his shoulder.

Employee was working the the Plant mud pumps. He went to shut off agitator & lowered it to stand on while accessing the pump pipe and checking valve. While manipulating valve cable clamp, pipe fell pulling him off balance, causing him to fall.

Employee was working in the maintenance shop on the 2" pipe assembly, which was in a vice. He was using a 24" pipe wrench and the wrench slipped striking his left knee, causing his knee to swell with extensive pain and bruising.

The impact knocked his hard hat off and knocked him down to the ground.

His fingers were under the mount when the pry bar slipped, dropping the assembly on his finger, amputating the last joint on his left index finger.

EE was on top of kiln removing shims from under pads on #3 tire. After old shims were removed, compressed air was used to clean rust, scale, slag, etc. from under pads before installing new shim. EE heard a pop when this happened causing a strain to his right hand.
