Employee was working in a dusty environment. Later in the shift, they felt their eye irritated. At the end of shift, they flushed with eye wash solution. The next day, the eye condition remained the same, irritated. The following morning, they notified management that they wanted to go the clinic to have it checked out. At the clinic, employee was prescribed a steroid.
A 3/8" plate being installed by contractor broke off and hit contractor in face.
Employee was walking to the 506 feeder in the 2 barrel, employee attempted to step over a large rock and EE's foot slipped and slid under the conveyor, twisting EE's knee to the side.
Gear tooth broke on hydraulic motor causing the tire manipulator to jump striking employee on the right side of the body.
Employee was assigned to load shot rock and #2 base into CAT 775 haul trucks. Employee started feeling discomfort in the lower back around mid-morning, it was not until EE got out of the loader to use the bathroom that EE felt a more intense pain coming from the lower right side of EE's back. This was about 10:00 AM.
Employee stepped from bed of pickup truck and slipped. EE fell to ground and broke left fibula.
EE was at a Martin Marietta (customer) job site and was using a customer provided 1" drive impact wrench to loosen/tighten bolts while changing out blow bars on a crusher. Protruding pin from impact wrench extension caught EE's glove & twisted EE's R hand, bending thumb way back. EE sustained strain to hand, causing soft tissue/ligament damage/swelling. MRI ordered.
Employee was testing for a leak on a crane hydraulic pump motor and a seal blew out cutting the employees finger with hydraulic fluid. Employee was treated and released to full duty. Finger became infected 2 months later.
Employee was unrolling a spring return fueling hose and reported feeling a pull on the left side of his ribs.
Employee was inspecting transmission filter on a loader during preventive maintenance. Upon completing inspection, the access cover dropped into its closed position pinning employee's right hand between the access cover and the supporting structure. The result was two fracture fingers.
Employee was pulling cable while walking up the steps. The cable got caught in the counterweight of the drive, pulling the cable away from employee resulting in a laceration on his right thumb and right torso.
Driver climbed up on top of the bulk truck bed with out wearing a fall protection harness/lanyard and also failed to put up the pneumatic handrail up as required. He was on top of of the truck attempting to close the hatch and lost his balance falling off causing serious injuries. He was transported to the hospital and has a broke right leg and right wrist.
EE was trying to lift trailer & it fell on his leg.
EE was using a grinder with a cup style steel wire brush on a conveyor frame structure. He noticed about an hour after completing his work that he had a knot forming on his right forearm and pain began to occur. He went to an urgent care center and they determined that a piece of the wire brush was lodged in his arm.
While an employee was cleaning out a 20" slurry line that was clogged with material. Water pressure and sand blew out of a ball valve that was being used to unclog the material. The water pressure and some steam that came out of the ball valve, landed on the employee causing minor 1st degree burns on his abdomen and leg areas. This also caused a contusion to his left shoulder.
As employee was shoveling material off a catwalk, he strained his right wrist.
As employee was exiting the drive section on cv-204b, his foot was caught on a grease line that was near by. As he tried to free his foot from the grease line,he tripped and fell, rolling forward, and landing on his back. This caused a strain to his lower back area.
While employee was removing cross key from rail car, his left hand pinky finger was caught between the cross key and the wheel in a pinch point and caused a laceration to his finger.
As the employee was walking towards the plant stairs, he twisted his right knee as he lost his footing on the large rock material that was laying on the ground. This caused a strain to his right knee.
As employee was climbing down ladder to go to MCC room, the employee slipped off the ladder and hit his left hand ring finger on the ladder. This caused a fracture to his left hand ring finger.
Troubleshooting - mobile crusher. The employee was standing on a platform working on some hydraulic lines. When he stepped off he slipped and fell on some dust build up, and twisted his ankle. (Fell against hydraulic leg.)
While employee was placing nuts on a U-bolt clamp bracket, for a cone crusher accumulator cylinder. The cylinder slipped off the support blocks, pinching his left index finger between the cylinder and bracket.
Employee was opening and closing hopper doors on rail cars. While employee was holding up locking mechanism the door sprung open catching his finger between door and locking mechanism. This Resulted to a laceration to print area on middle finger,which required stitches.
Employee of customer Alamo Transit Trucking Company caught left ring finger on truck, pulling finger from hand.
As employees were changing out bearings on an 8'x 24' screen box. Employee smashed his left hand index finger between the bearing housing and an I-beam near by. This caused a fracture to the index finger.
Employee was using a come-along hoist to lift a motor and install v-belts, when hoist slipped and motor fell landing on his right foot causing a fracture.
While the employee was moving back a roll-away chute from a screen box. A piece of grip strut catwalk that was standing against the handrail behind him fell and struck employee on the right side of his back. Causing a fracture to a rib.
EE WAS USING WINCH TO RAISE TRAILER. WHEN HE RELEASED THE HANDLE, IT SWUNG BACK AND HIT HIS RT. HAND CAUSING A CONTUSION.
EMPLOYEE WAS REMOVING THE CABLE CLAMPS OFF OF A WIRE ROPE CABLE BEING USED TO PULL A CONVEYER BELT. HIS HAND WAS CAUGHT BY THE CABLES, RESULTINGIN A FRACTURE TO HIS LEFT INDEX FINGER.
EMPLOYEE WAS CUTTING PIECE OF SKIRT RUBBER ON CONVEYER FRAME WITH UTILITY KNIFE, WHEN SLIPPED AND CAUSED MINOR LACERATIONS TO ABDOMEN AREA, RESULTING IN 4 STITCHES.
EE WAS HELPING CHANGE RETURN IDLER OTHER EE TO WELDING TO FRAME IDLER SLIPPED FROM FRAME AND CUT EE ON EAR AND CONTUSION TO THIGH.
EMPLOYEE WAS CUT BY POCKETKNIFE AS HE HELPED ANOTHER EE OPEN BOX, RESULTING IN THREE STITCHES.
EMPLOYEE PUSHED ON PRY BAR WITH FOOT; HIS FOOT SLIPPED, AND THE TIP OF THE BAR PUNCTURED HIS FOOT.
EE WAS LIFTING BELT MATERIAL ONTO CONVEYOR AND STRAINED BACK AND GROIN. INCIDENT REPORTED ON 10/15/02
EE'S HAND WAS PINCHED BETWEEN 6"X6" BLOCK OF WOOD AND BRASS RING BEING TAPPED. RECEIVED TWO STITCHES, RELEASED TO FULL DUTY.
EE INDICATED LOWER BACK PAIN AT END OF SHIFT OF OPERATING FRONT-END-LOADER, DIAGNOSED WITH LOWER BACK STRAIN. BECAME RECORDABLE ON 6/26/02 (RESTRICTED DUTY BEGAN)
EE PLACED HAND BETWEEN SCREEN WALL AND SCREENINGDECK. WHEN DECK SLIPPED, HIS HAND WAS PINCHED BETWEEN, RESULTING IN A BREAK AND LACERATIONS, TO 3 FINGERS.
EMPLOYEE WAS OPERATING HIS HAUL TRUCK BEING LOADED IN PIT. A LARGE ROCK HIT BED CAUSING CERVICAL STRAIN.
EE LOST BALANCE AND FELL FROM 6' STEPLADDER, BRUISING BACK.
CHANGING SCREENS AND PRY BAR REBOUNDED AND HIT RIGHT FORHEAD. RECEIVED STITCHES.
MOVING OIL FILTERS AND FELT PAIN TO LOW BACK. ON RESTRICTED WORK ACTIVITY.
KNEELING IN SHOP TO SPLICE CONVEYER BELT AND HIS KNEE POPPED. OLD FOOTBALL INJURY- LOST WORK DAYS.
CLEANING A GUARD THAT HAD SOME UNFINISHED EDGES, CAUGHT FINGER ON EDGE. LACERATION AND STITCHES.
DRIVING LOADER IN THIRD GEAR OVER ROUGH TERRAIN. LOOKING BACKWARD AND FELT STRAIN TO LOW BACK.
WHILE WORKING AT EQUIPMENT SHOP, EE WAS TAKING TRANSMISSION FILTER OFF HAUL UNIT AND HIT LEFT ELBOW. THIS WAS NOT REPORTED TO ANY SUPERVISOR UNTIL 7-13-01. EE WAS ON MODIFIED DUTY FROM 7/16-7/ 27.
FREQUENTLY PUSHING CLUTCH ON LOADER TO LOAD TRAIN AND FELT PAIN TO LEFT KNEE. MODIFIED DUTY ANTI INFLAMMATORY GIVEN.
WHILE BENDING OVER TO PICK UP A LADDER, FELT SHARP PAIN TO LOWER BACK.
TURNING ON HIGH PRESSURE WATER HOSE. WATER SHOT INTO FACE.

DRIVING HAUL UNIT ON UNPAVED TERRAIN AND FELT PAIN TO LOW BACK. INJURY-1-19-01, REPORTED INJURY-2-6-01.
DRIVING FORK LIFT UNDER CONEYOR AND CLUMP OF DIRT FELL ON FORKS. DIRT PARTICLES INTERED RIGHT EYE. DID HAVE PRESCRIPTION MEDICAITON.
POWER WASHING SILO AND WATER AND DIRT BLEW INTO LEFT EYE.
CLEANING UNDER TAIL PULLEY WHEN WATER AND FINE MATERIALS SPALSHED INTO LEFT EYE.
EE WAS DOING SAFETY HOUSE KEEPING. IN THE PROCESS HE WAS RELOCATING 2 RE-RAILERS (APPROX 60 LBS EACH) AS HE WAS TRYING TO REPOSITION RE-RAILERS INTO STATIONARY BACKHOE BUCKET THE RE-RAILER SLI PPED AND PINCHED HIS INDEX FINGER AGAINST THE OTHER RE-RAILER CAUSING A LACERATION. THE EE WAS WEARING SAFETY GLOVES AT THE TIME OF THE ACCIDENT.
CLIMBING DOWN LOCOMOTIVE, SLIPPED AND HIT LEFT KNEE ON ROD.
CHANGING FILTER ON 988 LOADER AND WRENCH BROKE LOOSE AND TWISTED LEFT ELBOW.
COLSING DOORS ON RAIL CARS. EE WAS SWINGING A 10 LB SLEDGE HAMMER TO CLOSE DOOR LOCK. EE STATED HE FELT A SHARP PAIN TO HIS MUSCLE BY HIS COLLAR BONE. INJURY: 3/14/00 REPORTED: 3/19/00 MODIFY DUTY:3/20/00 LOST TIME: 4/3/00 (STILL OUT)
