EMPLOYEE PERFORMED WORK ON THE CAT 910 PARKING BRAKE AND WAS TESTING IT TO SEE IF THE PARKING BRAKE WOULD HOLD. HE WAS OPERATING THE CAT 910 ON OR NEAR AN INCLINE. THE MACHINE OVERTURNED. HE W AS FOUND LYING BESIDE THE MACHINE. HE SUSTAINED FATAL INTERNAL INJURIES.

While replacing an HSI crusher lift cylinder, the component shifted, pinning the employee's finger against the crusher wall. The employee sustained an open fracture of the left middle distal phalanx. Treatment included stitches, a splint, and medication with work restrictions.

EE was reinstalling the AC belt on a pickup truck. In order to place the belt on, EE was using a wedge tool to guide the belt back on. EE was leaning on the core body/radiator and as EE went to move away from the hood, felt a pop in left rib cage. EE sought medical attention the next day and was diagnosed with sprained left rib.

1st EE & 2nd EE were welding/grinding single sand screw to mend it. 1st EE was under sand screw to weld when 2 EE began grinding causing sparks & debris to fly towards 1st EE. 1st EE removed self until 2nd EE finished. 1st EE was in full PPE; face shield safety glasses gloves hard hat & fall protection. EE reported it on 2/19. Diagnosed with conjunctivitis & prescribed eye drops.

EE was tasked with replacing troughing idler cans on a conveyor (C-12). They were using a metal prybar to lift the belt when they felt a pull in their back. This resulted in a lower back strain.

Pit loader operator in a 992K loader, drove parallel to the muck pile. A large boulder slid down the pile and struck the right front loader tire. The tire blew out and during the movement the employee's right hand was injured by an unknown object in the cab. Employee suffered from a laceration to the right thumb and a strain to the right thumb / palm area.

EE was going to start replacing ball valves, as EE stepped down a set of stairs, EE's right foot slid forward causing the knee to overextend. EE fell to the ground. EE sustained a right knee sprain.

Employee was working as a groundsman, performing house keeping duties, under the primary crusher when he experienced symptoms of heat exhaustion. Another employee assisted him to the office to cool down. He sought medical attention at the local clinic, diagnosed with heat illness and was released to full duty.

2 of 3 utility owned power lines came loose from pole and fell to ground. The lines continued arcing for over 30 minutes, preventing the fire department from extinguishing the small brush fire. The Utility Repair Crew replaced the lines and system parts up to the (utility owned) pole mounted transformers. No Injuries, or damage.

An employee was pulling an air hose to the screen tower to work on the screen deck. There were two hoses coupled by a quick connect, the quick connect hit a section of the hand rail, it became uncoupled at this time. The hose whipped upwards into his face causing a small laceration to his right eyebrow.

Employee was using a pry bar to loosen a broken liner in the 130/150 universal impact crusher. When the liner was hit with a sledge hammer the bar slipped and cut him on his right side cheek. The employee needed stitches.

EE was performing maintenance on 82 ton Linkbelt Crane. EE completed job on crane, was getting off truck and stepped onto 4' ladder he was using to access truck deck and slipped and fell to the ground. He fractured three bones in his left ankle. Ladder was still in place when he looked up from his fall. Surgery on ankle to repair fractures on 2/7/08 @ 21:30 hours.

Two employees were in the process of moving a large bearing (approximately 100 pounds) from a work bench to the shop floor. Injured lost his grip and the bearing landed on the top of his foot. He was diagnosed with a few broken bones in his foot as a result of this accident.

Loading sticks in the sun all day, 97-98 degree's outside, went to go to the truck and legs started to cramp up, then the entire body locked up, fell to the ground, sent to the hospital by ambulance - body was dehydrated.

WHILE ASSISTING ANOTHER MECHANIC, THE OTHER MECHANIC HIT THE EXPANDED METAL CAGE ON A SKID STEER LOADER IN AN ATTEMPT TO KNOCK OUT AN INDENTATION ON THE HARD HAT. THE HARD HAT PUSHED ON HIS SA FETY GLASSES CAUSING A LACERATION ON HIS FOREHEAD. STITCHES REQUIRED.
