EMPLOYEE WAS CLEARING ROCK OFF A CONVEYER BELT. HE STARTED TO WALK BACK DOWN ON THE CATWALK AND SLIPPED ON SOME LOOSE GRAVEL. HE FELL, BRUISING HIS LOWER BACK AND BUTTOCKS. HE INITIALLY DECLIN ED MEDICAL HELP BUT THE NEXT DAY INFORMED HIS SUPERVISOR THAT HE FELT HE NEEDED TO BE SEEN BY A PHYSICIAN.

EE WAS STANDING ON THE CATWALK BY THE CONE CRUSHER. HE STEPPED ONTO THE LEDGE OF THE CRUSHER AND WHEN HE JUMPED BACK DOWN, HE MISSTEPPED AND TWISTED HIS ANKLE. EE STATES HE MIGHT HAVE HAD GREA SE ON HIS BOOT WHICH CAUSED HIM TO LOOSE HIS FOOTING. HE REFUSED MEDICAL TREATMENT BUT WENT TO THE EMERGENCY ROOM LATER THAT NIGHT. HE RECEIVED A SPRAINED ANKLE FROM THIS INJURY/ACCIDENT.
