EE was adjusting the weight on a vibrator in the dryer burner room when EE's foot slipped a little causing EE to put all EE's weight on right shoulder causing EE to dislocate right shoulder. EE was taken to the ER where the shoulder was treated and EE came back to work the next day.

On Saturday morning at around 6:15 am EE was trouble shooting the burner at the dryer. After several trips in and out of the burner room EE, tripped and stumbled. While falling EE's hardhat came off. EE fell on left knee and striking head on a metal cabinet causing a laceration on EE's scalp resulting in five staples. DR returned employee to full duty.

While working in the Air Separator (GSAS-01) at the Grinding Mill, a maintenance EE's hands slipped of a Whizzer bar EE was removing. There was a sharp piece of metal protruding which lacerated the EE's right thumb between the 2 knuckles. EE received 4 stitches to their thumb.

At roughly 3:30 PM on Tuesday December 1 the 2nd shift Wet Process Operator stepped on a rock getting out of vehicle and sprained left knee. Employee was taken to the ER. The result was a sprained left knee and ankle. Employee is to return to work in 3 days.

Thursday April 30th while installing dryer flights at the rotary dryer (DR-01) the maintenance employee strained back. EE did not want to go to the doctor and finished the shift. Friday May 1st the employee was able to come to work, stretch it out and make it thru the day. Thursday May 28th the employee stated EE needed to go to the doctor because back was injured.

At approximately 8:45 am an employee slipped on algae injuring left shoulder on the concrete pad at PU-42 while working on a guard. Employee reported the incident at 10:00 am and finished the shift. Monday afternoon the employee reported needing to go get checked out. The doctor put the employee on lost time for 2 days, after which no restrictions.

Maintenance EE was replacing a flow divider for the mill hydraulics. EE loosened the hose for the flow divider and hydraulic fluid splashed ee in the face and went under safety glasses and into eyes. EE flushed eyes and was taken to the ER. ER flushed eyes again and prescribed prescription eye drops. EE returned to work with no modified duty or lost time.

Ee went to check on the 510 air valve in the tunnel. Ee rested EE's hand on the upper ledge of the tunnel entrance. A small amount of sand fell from the ledge getting into the EE's right eye. Eye was flushed. EE went to the emergency room the next day and received a prescription for eye drops. Ee returned to work. No modified duty.

Employee was carrying a 5 gallon diesel can about to enter a door to the mill. The employee set the can on the ground to go inspect a noise at another location. When they started to walk away they slipped on a patch of ice with the left foot. They fell on their right side injuring a shoulder.

Employee was attempting to remove a bushing from a sheave positioned on the ground. While repositioning to its side the sheave began to slip and the employee got right index finger caught between the sheave and adjacent structural steel support. The right index finger was pinched and caused a laceration that required 10 stitches. EE was released to returned to work.

The employee was attempting to turn left on the southbound side of the haul road, when the 1/2 ton pickup ee was driving was struck on the driver's side by a CAT 773 truck that was coming up from behind ee. Employee was taken to the hospital and checked out. Employee was released to light duty. Damage to pickup was substantial on drivers side. Minimal damage to CAT 773 truck.

Employee was working on replacing a conveyor belt with 2 other employees. A few days later EE felt a crook in neck but did not report it. EE went to personal doctor on 22 June 2016 (a chiropractor) and received treatment. The mine operator was notified on 29 June 2016 that EE went to the doctor and wanted to fill out a injury report. Dr diagnosis pinched nerve in back.

EE was using a bobcat loader to stand up a pipe secured to the loader bucket. Once set the rigging was removed, EE was backing the loader away, the pipe tipped toward the loader, EE reached out to catch the pipe and the tip of his right little finger was trapped between the pipe and loader frame causing a small laceration requiring stitches and fracture to the tip of the finger.

EE was helping oiler change the cutting edges on a motor grader he had been operating. As EE unscrewed the last nut (edges come bolted together) the cutting edges fell faster than expected, pinching his left pointer finger, resulting in the loss of a small part of finger tip (approx. 1/4" X 1/2"). All PPE was being worn/used at the time, including gloves.

EE states his left wrist began hurting and he believed it to be a muscle ache. It continued on & off Sat, becoming constant on Sun. and began to swell. EE did not wish to seek medical care, but on 2/19/14 he went to clinic-diagnosed w/tendonitis. He has been on conservative treatment until 6/18/14. On 6/19/14 he had outpatient surgery to correct pain. Lite duty 6/20/14.

This incident did not immediately involve either modified duty or lost time. For more than three years EE has had incidents on this job with his left ankle. Yesterday(05/06/2014) he came in with a notice from his doctor stating that he would be out of work for at least 30 days following surgery on his left ankle to correct the problem.

EE was retrieving a come along and belt brackets from the tail section of a elevator he reached down to pick them up and felt that he may have pulled muscle in his lower stomach. EE did not report the alleged incident until 11/18/2013 and did not seek medical attention until 09/05/2014, restrictions were prescribed for a possible hernia at which time the incident became reportable.

AS IT BECAME DISLODGED, HIS SAFETY GLASSES FELL OFF OFHIS FACE. THAT TIME HE PULLED BACK TOWARD HIS FACE PUSHING THE WIRE IN HIS EYE.
