Employee's right thumb was caught between a post hole driver and a ground rod while driving rod into ground. Employee received a laceration which required medical attention and stitches.

Employee was walking to work site to start connecting a water line to a portable pump. The employee stepped into a uneven surface, causing EE to fall, twisting EE's left knee.

EE was trying to put the crane pad back on the crane deck. When EE threw pad up on the crane deck, the pad landed on their left middle finger resulting in a laceration.

Miner was attempting to pull a piece of conveyor skirting from a chute when EE lost balance and fell backwards onto the catwalk. Miner sustained a lacerations to EE's left hand requiring 4 sutures to close.

Miner was using a pipe wrench to loosen spray nozzles on wet screen. When the threads loosened, the miner's right hand came into contact with the spray bar above causing a laceration requiring stitches to close. Miner was released to full duty.

Miner states their knee buckled at ~5:30am while dismounting CAT 775 haul truck at end of night shift on 06/11/2017. Miner did not report the incident and went home. Miner called their supervisor at ~4:30pm on 06/11/2017 to report the right knee was swollen. Miner went to local ER diagnosed with knee strain. Miner was released back to work with restriction 06/12/2017.

Miner was replacing a bearing. Miner's finger was smashed between the new bearing and the head pulley, while moving the new bearing into position for installation. The miner was wearing gloves.

EE operating CAT 775 HT when he encountered a dip in haul road. EE reported that jarred his back. EE saw doctor, released w/o restrictions, 11/10/14. Follow-up appointment 11/17/14, released w/o restrictions. EE saw doctor 12/04/14 given restrictions for work.

On 03/02/2014 miner reported that his eye had become irritated at the end of his shift while picking up for the day. High Easterly winds prevailed most of the day on 03/01/2014. Miner was seen at local ER. No object found in his eye. Given medication for scratches and irritation and released to work.

Miner was shoveling spilled material inside the BC-01 tunnel. As he moved from one side of the belt to another, he stepped on loose rock spraining his left ankle.

EE was installing belt wiper on BC02 to divert stone. EE used his personal pocket knife to "mark" the belting in order to punch holes in the belting for mounting. While EE was "marking" the belt, his knife closed, causing a laceration to his right ring finger. EE was seen at local hospital, required 6 stitches to close wound, and released without restrictions.

Assembly of D9T with Blade Bush bars that were standing upright on the ground. As the employee tried to move one bar the other bar fell down hitting his left ankle pinning it between the bar and ripper shank causing an open fracture on the ankle

Employee was removing forks from a rough terrain forklift. As the pin securing the forks to the mast released, the forks tilted back to the mast catching his middle, right finger between the fork and the mounting. Employee received 5 stitches, and was released to return to work w/out restrictions.

Employee removing v-belt guard on blower fan at dry plant. Had loosened all bolts, top section of guard, and bottom section of guard when his finger felt "really warm". Removed his glove, saw blood on his right ring finger. Received 8 stitches.

Miner was grinding in front of welders while re-lining a pan feeder. He had his back to the welders. Welding flash came from behind his face shield and reflected off of the inside causing burns to the miner's eyes.

Miner was walking around his loader when he twisted his ankle. Injury was initially a first aid injury. On 11/15/2008 the miner left work early, telling his supervisor that his ankle hurt. He went to the emergency room and was told to take off work through 11/17/2008.

EE was helping replace a bearing on a speed sensor on a conveyor that was at ground level. He was walking up a small (approx. 4 foot) mound of material. His right foot slipped and he fell to same level. He struck his right shoulder on the ground, causing a right side shoulder sprain.

EE WAS INSTALLING FLEXIBLE ELECTRICAL CONDUIT ON A JUNCTION BOX. THE INNER METAL CLADDING WAS EXPOSED AND EE'S HAND SLIPPED, CONTACTING THE SHARP EDGE. THE METAL CUT THROUGH THE LEATHER GLOVE AND CUT EE'S LEFT MIDDLE FINGER. EE RECEIVED 5 SUTURES.

EMPLOYEE WAS USING A 24" CRESCENT WRENCH TO LOOSEN A RUSTED TURNBUCKLE THAT WAS SECURED IN A BENCH VISE. AS HE PULLED BACK ON THE WRENCH HANDLE WITH BOTH ARMS, THE WRENCH CAME OFF THE TURNBUCKLE CAUSING THE EMPLOYEE TO LOSE HIS BALANCE AND TURN HIS LEFT ANKLE. THE LEG FRACTURED AT THE BASE OF THE LEFT FIBULA.

EE WAS REPLACING CENTER PIN ON 988 CAT LOADER. USING CUTTING TORCH TO CUT CENTER PIN THAT WAS STUCK IN A BEARING. HEAT FROM TORCH CAUSED GREASE IN BEARING TO EXPAND AND IGNITE SPRAYING EE WITH BURNING GAS. EE SUFFERED BURN ON FACE FROM HOT GREASE.

WHILE UNLOADING ANFO EMPLOYEE BENT OVER AND LEANED DOWN TO HIT THE LOWER PADS ON THE TRAILER TO LOOSEN THE ANFO WITH RUBBER MALLET. AS HE STEPPED BACK TO STAND UP HE STEPPED ON A ROCK CAUSING HIM TO FALL BACK AND TWISTING HIS RIGHT ANKLE. HIS ANKLE STARTED TO SWELL AND TURN PURPLE.
