A crew of 5 workers were installing an 8'x10' process door when the lifting lug on the door became detached. This resulted in the door free falling approximately 22" to the ground and ultimately falling over onto one of the crew members.
Employee was crossing over a 36" pipe in a sitting position and became unbalanced and fell to ground level. Employee experienced pain in left forearm and was treated by Doctor. Treatment include applying a sling to the arm and prescription pain medication.
Contracted service employee (seasonal) was performing landscape maintenance/mowing on site and was stung by bee/yellow jacket. Reporting under our Mine ID as contractor is an independent contractor and does not meet the requirements to have their own mine ID under 30CFR Part 50. However, the reportable should not count against 4100320 as the person is not employed by Alcoa.
Employee says that yesterday, 6/11/2018 around 9am, right leg began to hurt just below the knee and on the outside part of the calf. Employee says doesn't know the exact time the pain started and isn't sure on exactly what cause it. Employee says the pain feels like a constant Charlie horse and is having difficulty walking on the leg.
Employee was operating machinery walking along side via remote when EE noticed something in EE's eye. Eye protection was being worn at the time. Employee reported to security for evaluation and was taken to medical clinic for further evaluation. Employee had an abrasion on the eye and was treated with medication.
Employee was moving a heavy plate (spacer) used to open a door. The employee felt pain when they lifted the plate with right hand but pain dissipated and completed shift. The following morning they reported pain in the hand/wrist.
Index finger bent backwards while pulling cable off when cable suddenly started going the opposite direction due to cable on tugger was wedged in a spot and started backspooling. Employee went to Doctor on 7/25 and was put on restrictions starting 7/26.
Red line case. On 2/16/2017 employee got in and out of the bucket of the HV bucket truck 5-6 times. Employee did not feel bad at that time but reported the injury on 2/22 after knee got worse.
Employee was scoping a job in a manlift. Employee was reaching out and up to simulate what needed to be done (not lifting anything) when EE felt pain in back. The pain did not continue and EE completed the job. After leaving work and over the weekend the pain progressed and reported pain when arrived to work on Monday 2/13.
Redline case- On 9/15/2016 employee was pouring a coup of coffee, set cup down was holding coffee pot in right hand, felt sharp pain in right wrist and dropped coffee pot. No injury resulted from the incident. On 9/23/2016 employee was diagnosed with carpal tunnel.

Cleaning a screen box on tray 16, when inserting box finger was smashed between insert and box
While attempting to loosen the nut the pneumatic wrench caused the straight wrench to turn pinching the employees hand between the straight wrench and the adjacent bypass valve. The injured cleaner received a contusion to the top of the hand.

EE was grinding tacs off of the north elevator door, the grinder jumped and caught the glove and pulled thumb between guard and grinding disc causing laceration to left thumbnail.
Employee was hammering overhead to break loose a knocker, felt a "pinch" in employee's back.

ee was repacking a pump, the packing gland was stuck under the shaft he applied pressure to pull it out and felt right wrist pop and pain to right wrist
employee was welding on his knees, felt stinging on his knees and discovered that he was kneeling in caustic residue, minimal burn to left knee, 2nd degree burn to right knee
packing was leaking on valve, caustic sprayed out in face and ran down goggles and into left eye causing chemical burn

EE was swinging a 4 lb hammer and missed nut striking index finger on left hand causing fracture and laceration.
Employee states that he was beating on multiple press feed valves and repeatedly tried to seat valves as daily routine, pain in elbow started 6 months ago.
Employee was picking up a hose and felt sharp pain in mid low back.
employee was draining a heater, hit knocker valve with hammer, right index finger was trapped between hammer and T-handle causing laceration and fracture of right index finger.
EE was putting #15 caustic on, cleared the plug free from coat line, went to verify caustic line was fully open, the caustic blew past the head gasket, he turned to run and the first step he felt a pull in right calf.

EE was pulling a 6 foot rolling stand, EE fell and stand fell onto low abdomen. has pain to tailbone.
RED LINE CASE unsure of work relatedness, investigation continues employee claims hernia, no specific date of occurrence, states happened in 2014 but did not file injury with Medical department. He did come to medical but it is documented the he wished to speak with union prior to reporting, Employee did not return to file injury report
RED LINE CASE employee states that he had injury about 8-10 years ago involving left shoulder, states that he is having increased difficulty with left shoulder, increased pain and difficulty with movements of arm no complaints of new injury or specific instance or action caused current pain.
Came to work early, was struck by a car in the parking lot.
Employee was utilizing the flex lance and he thought that he felt the flex lance build up of pressure in the tube and it pushed the flex lance upward. Water sprayed on his face shield and his face shield fogged, which caused him to temporarily be unable to see. He then felt water pressure hit his right foot.

Employee pinched finger between railing and pipework. Per email from reporting party, EE was changing out the bag on the number 3 zimpro using a pipe wrench to hold bag up, while coworker, (new to job) started loosening the fitting. Injured EE's finger was caught between handrail and pipe wrench.
Employee was tightening valve packing, wrench slipped off nut and crushed finger between valve and wrench causing a tuft fracture 4th digit left hand.

Employee was caustic cleaning with a pry bay to remove scale, goggles became fogged, took goggles off to wipe them off, was wearing safety glasses and pry bar fell into trench , and caustic splattere under safety glasses causing burn to left eye
We had to move the old valves to the other side where the crane could fly them out. During the process of moving the old valve over the new one, it got caught. Two people let go but the third person didn't. The valve shifted in direction of the heater and pinched his hand.
Employee dropped 4 lb hammer on top of left foot causing pain.

Employee was hooking up a barge at R52 loading dock, the barge shifted due to wind. He was attempting to board the barge and the barge employee pulled him onto the barge, he felt a pull in his left shoulder.
RED LINE CASE BECAME RECORDABLE ON 6/17/15 CASE UNDER REVIEW employee was opening a valve and struck 5th digit right hand with sledgehammer, unsure how it happened, 1 week later presented with increased pain and swelling after weekend off, xray revealed tuft fracture
employee was running an impact tool, the impact slipped off the nut, the socket hit the grating and bounced off grating and struck employee in mouth breaking 2 teeth and lacerating lip requiring sutures

employee was changing a valve out, he was straddling pipes and when he stepped down he bumped his left knee on pipe and his knee was caught between the pipes causing contusion to the left knee

Report of incident on 4/18/2015 employee was crossing over stair case, the staircase rocked causing employee to be unbalanced, he smashed his right thumb and bruised inside of right har when thumb and arm were contacted by the tools he was carrying
On Tuesday 4/14/15 at a tank roof a worker was gathering tools and stepped over a pipe with his left foot near a "rafter", he then stepped with his right leg. His left foot was still held in place by the rafter causing his left knee to twist causing discomfort.
RED LINE CASE: H&S Manager is a disputing work relatedness employee states was hitting on mud 13 valve liquor floor for about 10 minutes and in the process of hitting the valve he felt a pull in lower back
RED LINE CASE- Salaried employee- initial determination not believed to be restricted, later determined that restricted tasks were essential job functions Determined to be restricted 5/6/2015 Employee was walking in Digestion staging area verifying material for job, while walking , employee tripped over a valve and fell on to knees.

Employee stepped off rail car to platform and foot slipped and missed platform. Fell onto buttocks and left leg fell into open space between rail car and platform causing contusion and abrasion to knee.
Employee was rolling a pump and finger caught between shiv and belt causing nail avulsion, laceration and left third finger fracture.

The drain had stopped flowing and line was assumed to be empty. #30 Mud Floor valve was not seated properly allowing flow to fill line and exit through plug once scale was knocked loose by contractors during their task. Supervisor was checking on progress and was walking away at the time scale was unplugged. A spray of hot caustic came in contact with supervisor's back.
Tightening packing on pump, ratchet gear slipped causing pain in right wrist
At approximately 11:50AM on Friday August 29, 2014, a Turner employee was injured while disposing of scrap material in a metal recycle bin. Employee was struck on the right thumb by a troughing roller that unexpectedly shifted while being placed in the bin.

Employee received a puncture wound requiring 4 stitches and suffered a impact fracture to the bone require further doctor's visits.

In process of closing knife gate wheel valve for caustic return line to caustic tank in R50 using a wheel valve wrench when the wrench slipped and caught end of left pinky finger between valve wrench and top of R50 caustic tank on edge.
Employee's shift started at 8am. At approx 10am, the employee was injured when the wind caused a door to slam on his right middle finger. When the employee was taken to the clinic for treatment, he was given a post accident drug screen and breath alcohol test. The employee failed the breath alcohol test and was subsequently terminated as a result.

While hydroblasting the airlift on R-45 #80 precipitator tank, the employee went to top floor of the precipitator tanks to monitor progress of the #80 tank. The tank "burped" splashing the employee with effluent.
Employee was walking to control room and his knee gave out. Employee did not trip or twist his knee. He was walking and his knee gave out.

Employee was cleaning launder ring on 56-2 tank. He started to feel tired. Reported to medical department and provided with electrolyte replacement fluid. Returned to work.
Employee was swapping pumps #13 and #14 and did not take a break and became overheated.
Employee was assisting with installation of air conditioner and strained shoulder while using a come along to raise unit into place. Patient was placed on restricted duty today 6/28/2013
Employee got feeling nauseated and sick to stomach.
"Red Line Case" No Mechanism of injury Stiffness in elbow for two months prior to report date
Fell down the wooden stairway at R84O trailer. There were 3 steps that the employee fell down. He contused his left knee.
Employee was using a sledgehammer on a screenbox and became dizzy. Patient was taken to the Medical Dept and provided with electrolyte fluid and rest time in cool environment.
Employee strained shoulder while assisting other employee with adjustment to boom on mobile crane. Conservative treatment was not successful and patient placed on restricted duty on 6/21/2013.
