Miner was raking ash at kiln hood when a piece of ash hit the end of the rake causing the rake to hit the miner on the chin. Miner sustained a laceration requiring stitches.

Miner was using a pipe wrench to remove an incline water strainer, and when the fitting broke, the wrench to slam against an I-beam pinching the right-hand pinky finger.

On 11/13/24 the miner reported pain on the right knee as a result of a workplace incident that occurred on 11/01/24. Miner was sent for medical evaluation, was placed on restricted work activities and referred for physical therapy.

While operating a Kubota RTV, the vehicle hit a pot hole and jolted the miner who sustained a back sprain.

Miner was on top of the rail car opening the lids and strained ligaments of lumbar spine.

On 08/01/2024 miner was changing a drive belt using a wrench when injury occurred to shoulder joint. On 08/06/2024 miner was issued restricted duty by medical clinic.

EE got debris on right eye while standing next to another miner who was power washing.

While walking on a flat surface, miner twisted left ankle causing a potential foot fracture.

Employee 1 was holding wrench while employee# 2 was pushing on wrench to loosen leveling nut for column. Employee #1 's finger became caught between the wrench and the concrete pad that the column sets on when the leveling nut broke loose and the column base shifted down

Miner works in bagging department and is unsure what may have caused is right forearm to hurt starting on 12/16/2020. The miner informed supervisor on 12/21/2020 and was sent for medical evaluation. Miner was diagnosed with tendinitis on right arm, given anti-inflammatory medication, and put on restricted work duty until 12/30/2020.

Injured employee was in a boom lift using a cutting torch. The worker turned the torch off and draped it over the handrail. The acetylene valve knob bumped against the mid-rail of the basket opening the valve which allowed acetylene gas to penetrate the employee's pant leg. A spark from the grinder ignited the gas causing a flash fire and burn to the worker's left leg.

Miner was descending from a ladder, lost footing and used right arm to hold the ladder to prevent a fall. Miner sustained a sprain of the right shoulder rotator cuff. Physician put the miner on restricted work activities, and was released back to work immediately.

Miner was operating a forklift moving bagged product on a pallet inside the packing warehouse and hit a structural post with the front of the pallet. The miner reported feeling back pain after the incident.

Miner was walking on the concrete sidewalk and lost their footing as they stepped on a 2" rock. The employee began to fall and caught themselves on a nearby parked piece of equipment. The employee reported pain in their knee and later sought medical treatment. The employee was placed on restricted work duties.

Occupation Illness EE went to clinic 12/23/19, diagnosis "Allergic Contact Dermatitis" due to contact with Quicklime on both hands and left wrist. Was prescribed medication. 12/26/19 EHS Manager took EE to same clinic for workers-comp medical evaluation, physician confirmed "Allergic Contact Dermatitis to Lime". EE released to work 12/26/19 without restrictions.

A voluntary health based audiometric exam provided by the company to a miner who has only worked in a job identified to be below the required action level of 85 dBa and is therefore is not enrolled in the hearing conservation program was reviewed by the MSHA Inspector who told the company to report the voluntary exam results showing a shift as a reportable hearing loss.

On 07/18/19 miner reported to supervisor that they were using a wrench to tighten packing on the pressure vessel discharge at the hydrator, and when they pulled the wrench upwards, they felt a pain on left side of back from neck to lower back. Miner refused treatment on 07/18/19 but on 07/31/19 the miner requested medical treatment.

On 07/03/19, employee was working with broken bags of hydrate lime in the packing department. At the end of EE's shift EE felt a burning irritation on the upper bilateral inner thighs. On 07/10/19, the employee reported the upper thigh injury and an additional heat rash on a small part of EE's top right foot.

Miner was reaching for the emergency stop cable on the bagger mesh conveyor which was at waist level just a few inches away, and as ee started leaning sideways at the waist ee felt a sharp pain in lower back. There was no physical contact with equipment, tools, or bagged products.

Miner was using a come-along wench to extract a probe from kiln transfer chute. The wench strap slipped and got caught on a T-pipe connector, causing the T-pipe to break-off, go airborne, and hit the miner on the left elbow.

While driving a forklift the driver hit a dip in the road and caused the forklift to jolt. This created pain in the left leg and lower back of the driver.

An employee was getting a sample of quicklime in the silos galley way. As the employee walked, fugitive area dust got into the employee's eyes underneath their safety glasses. The employee was taken to a local clinic, had residual lime removed from their eye, and was given prescription medication.

EE was raking ash when a rock hit the opposite end of the rake causing the rake handle to jump out of the EE's hand and strike the EE on the right hand thus causing a contusion to right ring finger.

Miner was removing a compressed air hose from a wall hook when the hose Chicago coupling hit the miner's upper front teeth.

Employee was dismounting a forklift, slipped and strained right leg.

An employee was climbing down from the loader when right foot slipped off the top step at the rear left side of the loader. Medical attention was not immediately sought, but was later diagnosed with a strained knee.

Employee was accessing the top of a customer trailer to close lids. EE bent over to close the lid. With left foot on the rail, the rail broke, caused employee's left knee to come into contact with a rail bracket sustaining a sprained knee.

Worker was splicing a flexible PVC pipe with a utility knife. While making the cut, the pipe moved causing the knife to deflect onto the workers left hand. Worker was seen by medical personnel and received four sutures.

Worker was descending stairs; while holding the hand rail and walking down the stairs, left hand, little finger was caught between hand rail and structure steel beam jamming and bending it.

Worker was reaching to pull a 50lb bag of gypsum when lower back began to hurt.

Employee was walking down stairs and caught boot on stair; employee was diagnosed with a sprained knee.

Employee got off of shuttle wagon and was walking on flat ground to apply hand brakes to railcars, when ankle gave out. Employee sought medical treatment for pain and was placed on restrictions for an ankle injury.

The kiln pre-heater was clogged. In the process of clearing the clog, a hot pebble bounced of the clearing rod and landed on the neck of the injured worker. The pebble caused a 1st degree burn on the workers neck.

A load out employee was loading gypsum into a customer trailer when they felt a pop in their back.

An employee came into the office noting he was itching all over, he had welts all over his body, and had weakness in his legs. The employee was taken to a local clinic and was treated for an allergic reaction.

Employee was rolling balls of aluminium foil to calibrate crusher, when the box of aluminium foil fell off the table. The employee went to catch the box, when the blade side of the box cut the employee on the hand. Employee was taken for medical care and received stitches to his hand.

Employee was sitting at the operator controls, in the control tower, and was bitten twice by a spider on the wrist.

Employee was experiencing tightness in the chest and shortness of breath during his lunch break. Ambulance was called and employee was treated for a personal health issue.

Lime dust puffed up when a sample bucket was dropped. The dust got into the eyes of the worker. This caused irritation and discomfort.

While walking up a staircase on a silo, the employee banged his left knee on the staircase railing because he got too close to it while he was walking.

The EE started work at 1900 10/3. At 0100 saturday the EE felt a pain in his low back from lifting rail car doors. The EE returned to work at 1900 10/4 with no complaints of pain and while lifting other doors he felt a pain in his upper back. On 10/6 the EE asked to see the Dr and was placed on restricted duty.

An employee was carrying a 72# shaft and bearing set from a screw conveyor up a flight of stairs. He felt a burn in his groin and was found to have a hernia.

On 06/23 the employees was working in the cooler pit below the kilns raking scale at the time he was wearing a tight fitting respirator. While raking he received minor lime burns. On 07/13 he requested medical treatment for the burns on his face that were not healing.

Miner was walking on some uneven ground and sprained his ankle.

While removing plywood from overhead in the pre-heater, a rock fell and struck the injured person on the lower back.

Two employees were executing an oxygen bottle change on the service truck then bottle got stuck. Force was applied to the cylinder to get it un-stuck. At the same time force was applied at the front of the cylinder. The injured employee stuck his hand in the back of the tank to try to help. The bottle got loose and struck his hand causing injury.

Miner using a-frame ladder to make repairs, stepped off ladder and missed bottom step. Miner' s foot landed awkwardly and caused strain to left groin muscle.

Closing doors on coal railcar hopper, miner lost balance and twisted left knee.

Miner was removing a guard on a belt that was locked out for maintenance, as he and another miner were placing the guard to the side, the miner's finger was smashed by the edge.

Miner cutting tubing with a utility knife, cut through tubing and into his own leg.

Miner was performing maintenance on Silo G/H dust collector. Miner was removing full dust bags from compartments with other miners and got out of sequence causing a shoulder strain.

EE was shoveling cement dust. The dust circulating in the air landed on neck and body moisture on his skin caused the area to be irritated. After a few days his neck became very red and painful. EE was taken to a Med Center and treated. He was given prescription medicine and 2 days away from work. The back of his neck has improved in appearance and minimum pain.

He was closing a slide gate with a sledge hammer. The hammer slipped and he hit his left index finger.

Contractor cutting bottom of hopper. Hopper was secured with rigging. As cut was complete, hopper shifted two-inches pinching right hand, amputating index finger distal section.

EE was coming down from loading hydrate when he stepped down from the sidewalk to the ground. He did not land on level ground and rolled over his right ankle.

Unloading spare parts and banged elbow. Elbow showed signs of swelling.

Miner was operating hi-vac truck. Miner opened sight port without turning off engine, system pulsed and caused dust to go into the face of the miner. The miner was wearing safety glasses only, and dust entered his eye.

Using pipe wrench while doing maintenance on equipment

Employee making routine maintenance rounds stepping down from ladder and felt knee "pop". Worker reported and was monitored as he finished his shift. He returned the next day with stiffness in knee and requested evaluation by medical doctor.

The injured worker removed his protective gear while still in the production area to clean off his hood. While he was attempting to clean his hood, he was exposed to lime and sustained severe burns to his eyes. He has been successfully treated and released back to work on light duty.

Object pinching finger.

Operator received material in left eye when an inlet hosed to a diaphragm pump ruptured.

Operating a Bobcat. Dumped a load of dirt, coal dust and lime, the wind gusted and blew dust in the face of the operator. The operator did not think anything of the incident. He completed his shift and went home without reporting it. It was not until the following morning that he had an issue with his eye.

Employee stepped through an uncovered 12" x 16" grated opening in an elevated catwalk resulting in a contusion to his right leg and hip

Employee was raking ash at kiln #3 door. When he finished, he put the rake down and reachd for the sample scoop. His left knee went into a door used to dump material. He did not notice the door was open. Employee sustained a right knee strain.

Employee states that he was working on bag house carrying bags during shift. Felt right shoulder pain at end of shift.

Lowered gangway to access fall protection harness when end section of the extended walkway gave way. Result was fall onto tanker top. He temporarily held onto the handrail but could not support himself. Fell to ground below.

Lime dust flew up from a front loader causing a thermal burn on neck and left hand

EE was relocating fifty pound bags (no more than 100 individual bags) from the floor up to a conveyor (3.5 vertical lift) when EE felt a strain in is lower left abdomen. Event reported 2/25/08

Bag burst blowing lime dust in the right eye of the employee.

Injured was stacking empty bags behind the #3 spout packer when lime discharged from the #1 spout getting into left eye. Injured was wearing safety glasses. Immediately flushed eyes and taken for emergency care at Northeast Methodist Hospital, released after treatment with followup appointments to make sure no permanent injury to cornea as result of chemical burn incurred.

While doing clean up around a dust elevator choke up, some lime dust got into his boots causing lime burns to both of his ankles.

Some lime slid down his boot causing a chemical burn to his ankles.

Employee was manually closing top lid of rail car.

EE was decending a trailer ladder, when he stepped onto ground, he stepped in a hole that had apparently developed from recent rains. Sprained a tendon in his left foot. Was taken for emergency treatment, fracture rule out, placed on light duty for two days.

A contractor employee was picking up a pallet of 50# bagged lime (2,500#s)w/a forklift. As he inserted fork lifts forks in pallet it fell forward on another contractor employee that was walking in the path of the falling pallet. Injured was air lifted to medical facility, released at 1:00 am after treatment and injury evaluation 8/7/07.

Employee stated that he was stepping up a stair step when he tripped, fell and hurt his left knee and lower back.

Employee was standing on a conveyor platform trying to free a loaded pallet of bagged product. His foot slipped and his left leg became trapped between a moving conveyor and a roller conveyor. He suffered abrasions and a crush injury to his lower left leg.

Employee was standing next to another worker. The other worker struck a sledge hammer against a machine sprocket. A sliver of metal dislodged and struck the injured employee on the left cheek. There was a small laceration.

Employee was climbing a metal fixed ladder when his foot slipped. He had 3 points of contact and he caught himself with his arms/hands grip on rungs above. He noticed later that evening soreness in his left shoulder and was subsequently treated at a clinic.

Employee was walking up a set of metal stairs when her left foot slipped off a stair riser. Her right knee struck the edge of a stair riser causing a hairline fracture of her upper fibula bone. She was holding the handrail but was unable to break her fall.

Employee was holding a metal inspection cap in his hand. The cap slipped out of his hand and struck the employee's steel toed boot top. The employee's small toe on his right foot was struck and the small toe was broken.

Employee had kiln dust blow into his eyes. He subsequently asked for medical care and was given prescription eye drops by the treating physician. Diagnosis was eye irritation.

Employee was descending a set of stair steps. He stepped down to the next step which was lower than he expected. His left ankle twisted and became sprained.

Employee was lifting the door to the service elevator at the firing floor level. Some lime fell from above his head and into his eyes around his safety glasses. He washed out his eyes, but became increasingly uncomfortable and was taken to the clinic for treatment.

Employee was walking on an elevated walkway when some dumped material dust became airborne and got in his eyes around his safety glasses.

Employee was descending a ladder, his handgrip slipped and he fell about 5 feet scraping his arm, hip and receiving a small laceration on his right knee.

Employee was running a bagpacking machine and noticed later in his shift that he had recieved a lime burn to leg.

Employee was closing the top lids on railroad hopper cars. A lid he lifted & released fell back against his left leg. He lost his balance & fell. His leg was pinned against a walkway (on the top of the hopper car) & he sprained his ankle. His fall was arrested by the safety harness & lanyard he was wearing (EE ended up dangling off top of hopper car).

Employee was using a hooking tool to move a rock lodged inside of a machine. When he pulled on the tool, his right hand struck a metal guard rail smashing his thumb, breaking the tip joint of his thumb.

Employee was stepping upon a stair step and his foot slipped and his left knee struck a stair step.

Employee was trying to lift a bearing grease cover to take a temp. check. The cover did not open and he applied more force, resulting in a tendon rupture to his left biceps at the elbow.

Two employees were working with a Pressure Washer unit. After changing a tip-end of the pressure wand they tested it by sending water pressure through the wand. The recoil on the wand caused the wand to strike the injured employee across his nose.

Employee was lifting a hatch lid on a railroad car. The hatch lid was heavy and the gravity force of it caused a severe strain to the employee's right wrist ligaments causing a bone in his wrist to fracture.

EE was walking with a 5 gallon pail of steel balls. His feet slipped and he fell against a metal guard rail, bruising his left rib cage. 2nd physician diagnosed broken rib.

Employee was attempting to lift a heavy component without sufficient help or use of a proper lifting device. He stated that when he turned with the heavy load he felt pain in his lower back muscles.

EE was raking ash from lime kiln firing hood. There was a pulse of air which blew lime dust into his face shield. He stated that some material got in his eyes. He washed eyes out for several days but had irritation which required med. attention. He was treated for corneal abrasion 7-12 & returned to duty same day w/restrictions. Had followup visit on 7-15 & returned to full duty.

EE WAS HOLDING AN ELECTRONIC SENSOR IN PLACE WITH HIS RIGHT HAND WHILE PULLING A DRIVE BELT ON ANOTHER PART OF THE MACHINE WITH HIS LEFT HAND. HIS RIGHT HAND CAUGHT IN BETWEEN TWO SURFACES AND HE RECIEVED A LACERATION ON THE BACK OF HIS HAND. HE RECIEVED 6 STITCHES TO CLOSE THE LACERATION AND THERE WAS NO IMPAIRMENT RESULTING

EMPLOYEE WAS WALKING DOWN A STAIR SECTION CARRYING A PAIL OF STONES IN EACH HAND. HE STUMBLED AND FELL 3 STEPS TO THE GROUND SPRAINING HIS RIGHT KNEE

EMPLOYEE WAS WALKING THROUGH THE HYDRATE OPERATION ON THE BOTTOM FLOOR. HE HAPPENED TO LOOK UP TOWARD THE FLOOR ABOVE. SOME HYDRATED LIME MATERIAL FELL FROM THE FLOOR ABOVE AND STRUCK EMPLOYEE IN HIS LEFT EYE FALLING PAST HIS SAFETY GLASSES

Ee was using a metal pry bar to move rocks that were bridged in a secondary cone crusher. He jerked to pry a rock loose and strained his right shoulder. after monitoring his condition for a few days, he decided to receive treatment at a clinic. he was placed on modified duty and returned to work without loosing time from work.

Employee was driving a pickup truck through the plant site. A gust of wind picked up some lime dust and the dust was blown into his left eye.

EE WAS USING A PIPE WRENCH TO RELEASE A FITTING FROM A MACHINE. THE FITTING RELEASED AND THE WRENCH HANDLE STRUCK EE OTHER HAND AND MASHED IT AGAINST THE EDGE OF A FLANGE. EE RIGHT RING FINGER TIP WAS SEVERELY MASHED CAUSING FRACTURE WITH A CONTUSION, SPLINT AND THREE SOUCHERS NEEDED EE WAS RETURNED TO FULL DUTY ON THE SAME SHIFT

EE states he was using portable grinder to dress a piece of metal. In course of operation a piece of hot metal flew over his safety glasses & got into rt eye. Several days later EE began having discomfort in rt eye. Company took him to clinic which diagnosed small 2nd degree burn to upper part of rt eyeball. Burn had already started healing. Medicated eyedrops prescribed.

Employee was dragging a water hose up an inclined walkway. He stumbled and felt a pain in his lower back.

Employee was stepping off of a ladder and twisted his ankle when his foot slipped as his foot touched the ground.

EMPLOYEE WAS WALKING DOWN A SET OF CONCRETE STAIR STEPS. HE LOST BALANCE AND FELL TO THE GROUND INJURING HIS KNEE (LEFT).

EMPLOYEE WAS WALKING UP A RAMP WITH WOODEN BOARDS FASTENED TO THE RAMP WALKWAY PROVIDING A SAFE FOOTING. HIS FOOT SLIPPED ON ONE OF THE FOOT BOARDS, STRAINING A MUSCLE IN HIS GROIN.

EMPLOYEE CAME IN CONTACT WITH POWDERED LIME AND RECEIVED BURNS TO HIS FOREARMS.

INJURED EMPLOYEE WAS HANDLING A HEAVY METAL PLATE, CARRYING IT WITH ANOTHER CO-WORKER. INJURED EMPLOYEE'S FOOT SLIPPED AND THE LOAD HE WAS CARRYING SHIFTED. AS HE MOVED TO KEEP FROM FALLING HE RECEIVED A SEVERE MUSCLE STRAIN TO HIS LOWER BACK.

EE CAME IN CONTACT WITH POWDERED LIME AND RECEIVED ALKALINE BURNS TO HIS FOREARMS.

EE WAS BEGINNING AN ELECTRICAL MAINTENANCE JOB, WHEN OTHER WORKER BEGAN USING AN AIR TANK CAUSING LIME DUST & GRIT TO BECOME AIRBORNE DUSTING EMPLOYEE IN HIS FACE & BOTH EYES. EE RECEIVED IMMEDIATE MEDICAL TREATMENT & WAS DIAGNOSED WITH CHEMICAL CONJUNCTIVITUS, CORNEAL EDERNA, & CORNEAL FOREIGH BODY. EE RECEIVED A PRESCRIPTION & RETURNED TO WORK WITHOUT RESTRICTIONS.

EMPLOYEES HAD REMOVED 3 DOOR SECTIONS FROM THE TOP OF THE BAGHOUSE IN PREPARATION TO PERFORM MAINTENANCE. ONE DOOR SECTION WAS PLACED VERTICALLY AGAINST A HANDRAIL AND NOT TIED DOWN. A WIND GUST BLEW AGAINST THE DOOR CAUSING IT TO FALL, INJURING THE EMPLOYEE. MEDICAL CARE WAS GIVEN AND DIAGNOSIS WAS CERVICAL STRAIN TO NECK. EMPLOYEE RETURNED TO MODIFIED DUTY STATUS ON 4/14.

Employee was walking down stairs holding a bearing housing (150 lbs.) not wearing work gloves. The bearing housing slipped from his hands and fell a short distance, striking his left foot.

EE WAS SUPPORTING A HEAVY COUPLING WHILE ANOTHER EE ASSISTED. THE OTHER EE LOST GRIP OF THE CONE HE WAS HOLDING AND IT HIT THE FIRST EE S HAND CRUSHING HIS RIGHT HAND RESULTING IN BROKEN LITTL E FINGER. FINGER REQUIRED SURGERY AND A PIN TO REPAIR.

WHILE USING AN INDUSTRIAL VACUUM TRUCK, HIS LEFT ARM WAS PULLED INTO THE VACUUM HOSE CAUSING INJURY TO LEFT ARM. INJURED WAS ON REGULAR DUTY UP UNTIL 12/18/03 WHEN HE WAS PLACED ON MODIFIED DU TY DUE TO CONTINUING PAIN & DISCOMFORT RESULTING IN INJURY BEING MOVED TO REPORTABLE STATUS.

TIME OF INJURY IS UNKNOWN. SOMETIME DURING HIS SHIFT APPARENTLY GOT LIME IN HIS BOOT WHICH CAUSED TWO DIME SIZED BURNS THAT WERE DIAGNOSED AS THIRD DEGREE BY A DR VISIT ON 8/20/03. THE INJURY WAS REPORTED ON 8/18/03. THE INJURY WAS REPORTED ON 8/18/03. WHEN THE BURN DID NOT HEAL PROPERLY, THE EE WAS TAKEN TO THE DR. EE WAS RELEASED TO MODIFIED DUTY.

A "SPIDER" BUSHING WAS BEING REMOVED FROM THE SECONDARY CRUSHER. A COME-A-LONG WAS BEING USED & WAS UNDER TENSION. THE COME-A-LONG SNAPPED AND HIT THE INJURED AS HE WAS WALKING BY. THE SAFETY LATCHES ON THE COME-A-LONG WERE IN ORDER. TAKEN FOR MEDICAL TREATMENT, REMOVED FROM DUTY, CAST TO BE APPLIED ON 6/26/03 AND RETURN TO DUTY.

EMPLOYEE SLIPPED IN THE MUD, SEVERELY SPRAINING HIS RIGHT ANKLE. HE WAS RELEASED FOR MODIFIED DUTY THE DAY OF THE ACCIDENT BUT WAS UNABLE TO DRIVE HIS COMMERCIAL VEHICLE WITHOUT SEVERE PAIN TO THE INJURED ANKLE.

WHILE DISMOUNTING THE LINK BELT CRANE, THE INJURED LOST HIS BALANCE AND LANDED HARD AND AWKWARDLY WITH HIS RIGHT FOOT. THE IMPACT CAUSED A BIMALLEOLAR CLOSED ANKLE FRACTURE. INJURED WAS RETURN ED TO WORK AFTER TREATMENT ON A LIMITED DUTY BASIS. INJURED UNDERWENT SURGERY AND WAS OFF WORK AS OF 6/10/03.

INJURED WAS WORKING REMOVING A SHEAVE ON THE PRIMARY SCREEN OF THE SECONDARY CRUSHER. WHILE REMOVING THE SHEAVE, THE SHEAVE FELL ON HIS RIGHT INDEX FINGER TIP CAUSING AN OPEN WOUND THAT REQUIR ED SEVEN SUTURES TO CLOSE. INJURED WAS RETURNED TO REGULAR DUTY AFTER TREATMENT.

INJURED WORKER STRAINED LOWER BACK WHILE PULLINGIDLERS OUT OF THICK MUD ON UNEVEN TERRAIN.

LEFT FOOT GOT STUCK IN MUD AND INJURED WORKER FELT POP IN KNEE WHILE ATTEMPTING TO GET FOOT UNSTUCK. HE THEN REMOVED FOOT FROM MUD AND LOST BALANCE CAUSING HIM TO FALL AND STRIKE KNEE AGAINST ROCKS.

EMPLOYEE HAD CLIMBED ON TOP OF A GUARD THAT IS ELEVATED ABOUT 3 - 4 FEET FROM FLOOR LEVEL. WHILE TAKING DOWN A COME-A-LONG, EE SLIPPED OFF THE GUARD. HE LANDED ON HIS FEET, THE COME-A-LONG WAS IN HIS LEFT HAND. THE COME-A-LONG WEIGHT THREW HIS HAND AGAINST THE FRAME OF A MOTOR CATCHING THE HAND BETWEEN THE MOTOR & THE COME-A-LONG.

EE ALLEGEDLY STRAINED HIS BACK WHILE LOADING A SHOT. HE WAS WALKING DOWN A DECLINE WHEN HIS FOOT SLIPPED ON THE WET SURFACE. HE CAUGHT HIMSELF & DID NOT FALL TO THE GROUND. HE STATED HE FELT A SHARP PAIN & POP IN HIS BACK. TWO WITNESSES SAWHIM SLIP BUT STATED THAT THE SLIP DID NOT SEEM THAT BIG OF AN ISSUE UNTIL HE SAID HIS BACK WAS IN PAIN.

EMPLOYEE WAS TIGHTENING A BOLT BACK IN PLACE WHEN ANOTHER EMPLOYEE RELEASED A COME-A-LONG THAT WAS HOLDING THE FRAMEWORK WHERE THE BOLT WAS LOCATED. THE COME-A-LONG SNAPPED AND STRUCK THE TIP OF THE FINGER INFLICTING A LACERATION THAT TOOK 4 SUTURES TO CLOSE. THE EMPLOYEE WAS RELEASED TO WORK AFTER TREATMENT.

SOMETIME DURING THE SHIFT (2400-0800) FELT "SOMETHING" IN HIS RIGHT EYE. THINKS IT HAPPENED WHILE HE WAS DOING CLEANUP. A MICROSCOPIC FOREIGN BODY WAS REMOVED FROM HIS RIGHT EYE ON 6-8-01. RET URNED TO REGULAR DUTY.

WAS PUNCHING A HOLE IN A PIECE OF PLATE. DIE BROKEAND A PIECE WENT INTO RIGHT ARM.

WHILE LOWERING A 1"X8" X4' STEEL PLANT ONTO ANOTHER STEEL PLANT, THE STEEL PLATE IT WAS BEEING LOWERED ONTO "BOUNCED UP" CATCHING THE INJURED'S FINGER BETWEEN THE TWO STEEL PLATES CAUSING LACE RATION THAT NEEDED FOUR SUTURES. RETURNED TO REGULAR DUTY BY CO. DR. AFTER TREATMENT.

EE WAS STANDING WITH HANDS IN POCKET WITH HIS BACK TO WOODEN PALLET. WITHOUT LOOKING WHERE HE WAS STEPPING HE STEPPED BACKWARD & TRIPPED OVER THE PALLET FALLING BACK LANDING WITH WEIGHT ON RIG HT SHOULDER. ON 1/2/01 INJ REPORTED CONT PAIN. WAS TAKEN TO DR MRI WAS POSITIVE FOR A TEAR IN RIGHT SHOULDER TENDON. RETD TO REG DUTY SURGERY IS UNDER CONSIDERATION BY INJ. MRI REPORT RECD ON

EMPLOYEE WAS USING A HAMMER WITH HIS RIGHT HAND TO UNLOG A PIPE BLOWLINE AND FAILED TO SEE WHEREHE HAD HIS LEFT HAND POSITIONED. HE STRUCK HIS LEFT HAND THUMB WITH HAMMER, SUSTAINING A CONTU- SION AND FRACTURE TO VERY TIP OF LEFT THUMB; THEWOUND WAS CLEANED; FINGER GUARD APPLIED; FOLLOW-ING DAY TAKEN TO CLINIC TO HAVE PRESSURE UNDER NAIL RELIEVED; X-RAYS SHOW FRACTURED THUMB TIP.
