Miner works in bagging department and is unsure what may have caused is right forearm to hurt starting on 12/16/2020. The miner informed supervisor on 12/21/2020 and was sent for medical evaluation. Miner was diagnosed with tendinitis on right arm, given anti-inflammatory medication, and put on restricted work duty until 12/30/2020.

Injured employee was in a boom lift using a cutting torch. The worker turned the torch off and draped it over the handrail. The acetylene valve knob bumped against the mid-rail of the basket opening the valve which allowed acetylene gas to penetrate the employee's pant leg. A spark from the grinder ignited the gas causing a flash fire and burn to the worker's left leg.

Occupation Illness EE went to clinic 12/23/19, diagnosis "Allergic Contact Dermatitis" due to contact with Quicklime on both hands and left wrist. Was prescribed medication. 12/26/19 EHS Manager took EE to same clinic for workers-comp medical evaluation, physician confirmed "Allergic Contact Dermatitis to Lime". EE released to work 12/26/19 without restrictions.

A voluntary health based audiometric exam provided by the company to a miner who has only worked in a job identified to be below the required action level of 85 dBa and is therefore is not enrolled in the hearing conservation program was reviewed by the MSHA Inspector who told the company to report the voluntary exam results showing a shift as a reportable hearing loss.

On 07/18/19 miner reported to supervisor that they were using a wrench to tighten packing on the pressure vessel discharge at the hydrator, and when they pulled the wrench upwards, they felt a pain on left side of back from neck to lower back. Miner refused treatment on 07/18/19 but on 07/31/19 the miner requested medical treatment.

On 07/03/19, employee was working with broken bags of hydrate lime in the packing department. At the end of EE's shift EE felt a burning irritation on the upper bilateral inner thighs. On 07/10/19, the employee reported the upper thigh injury and an additional heat rash on a small part of EE's top right foot.

Miner was reaching for the emergency stop cable on the bagger mesh conveyor which was at waist level just a few inches away, and as ee started leaning sideways at the waist ee felt a sharp pain in lower back. There was no physical contact with equipment, tools, or bagged products.

An employee was getting a sample of quicklime in the silos galley way. As the employee walked, fugitive area dust got into the employee's eyes underneath their safety glasses. The employee was taken to a local clinic, had residual lime removed from their eye, and was given prescription medication.

The kiln pre-heater was clogged. In the process of clearing the clog, a hot pebble bounced of the clearing rod and landed on the neck of the injured worker. The pebble caused a 1st degree burn on the workers neck.

The EE started work at 1900 10/3. At 0100 saturday the EE felt a pain in his low back from lifting rail car doors. The EE returned to work at 1900 10/4 with no complaints of pain and while lifting other doors he felt a pain in his upper back. On 10/6 the EE asked to see the Dr and was placed on restricted duty.

At the same time force was applied at the front of the cylinder. The injured employee stuck his hand in the back of the tank to try to help. The bottle got loose and struck his hand causing injury.

EE was taken to a Med Center and treated. He was given prescription medicine and 2 days away from work. The back of his neck has improved in appearance and minimum pain.

Operating a Bobcat. Dumped a load of dirt, coal dust and lime, the wind gusted and blew dust in the face of the operator. The operator did not think anything of the incident. He completed his shift and went home without reporting it. It was not until the following morning that he had an issue with his eye.
