On 3/17/2021 EE was loosening the toggle plate tension rod's 85mm nut, rod broke. The spring violently released hitting wrench and cheater pipe, which struck EE in the palm of left hand and under chin. EE was treated for a contusion & released. On 4/16/2021 EE sought medical treatment for same injury and was placed on restrictions making this a reportable incident on 4/16/2021.

EE was replacing VSI hammers by removing the lifting chain from several worn VSI anvils. The anvils were placed in the recycle basket. One anvil had the chain under the anvil. EE was bending over the edge of the basket and reaching in when EE lifted and turned the anvil. It was at that moment that they felt a sharp pain in lower back.

EE was lifting a new rock shelf insert, weighing approx 80 lbs, from baskets. EE raised it approximately 4-5 inches above the basket when the insert began to slip from EE's grip. EE's left index finger was caught between the top of the basket and the bottom of the insert.

When moving a stand, employee's finger was pinched between the stand and a vertical support at the shop. Laceration to R middle finger, pad side, near tip required 7 stitches.

While shoveling spillage from under the tunnel conveyor on to the conveyor, the employee felt pain in the left side of his neck. He went to the doctor on 10-17-12, was diagnosed with a neck strain and placed on light duty. He was returned to full duty by same doctor on 10-19-12.

Employee was fueling excavator. The fuel nozzle was slipping out of the fuel tank so the employee reached up to grab it. While reaching for the nozzle the employee felt a sharp pain in his back. Employee decided to seek medical treatment on 6-12-2012. Diagnosis: Lumbar Strain. Light Duty.

EE and two other employees were rotating the VSI crusher lid closed. EE had his left hand in the notch on the lid for pushing leverage. He was not able to move his hand and his left hand got caught between the notch and crusher motor resulting in a fracture to the top of his left hand.

EE was dropping hammers off at our C-conationer located at Vulcan Pit in Knippa, TX. Pulled on a drill hammer to get it out of pickup and felt a pop in his back. He finished unloading the hammers and returned home. Rested that day and woke up the next day with back hurting. Tried to work on the 9th. Could only make 1/2 day before he went to doctor. No witness.

On 10/26/10 employee slipped as he was mounting a crane & strained his back & wrist. The employee was given restrictions by the doctor & was able to meet the restrictions performing his regular job duties. The employee's doctor notes were evaluated on 1/10/11 & it was determined to place the employee on limited activity to further help heal employee's injuries.

Employee was installing screens and felt sharp pain in stomach. The employee was given restrictions by the doctor and was able to meet the restrictions performing his regular job duties. Employee returned to doctor on 1/10/11 with continued restrictions and it was decided to accommodate employee and restrict regular job duties to further help heal employee's strain.

Injured was cutting old plate off the top of primary crusher jaw dies when wet material behind plate cut area popped and shot debris out. One piece found its way under his new-style cutting glasses (not goggles) and created a superficial cornea injury to his left eye.

(Late report by EE. Treated with ibuprofin until 06/13/2005--when he sought medical treatment. Saw doctor on June 17th.) Slipped off haul truck while descending, causing a shoulder strain--i.e., missed a step and tried to catch himself with his arms. EE HAD SURGERY TO REPAIR TORN ROTATOR CUFF ON 9/13/05.

Employee was standing on the vibratory feeder helping with maintenance on the jaw crusher. He turned to step back from a metal plate that was about to be hoisted. His feet slipped out from under him. As he fell his right arm hit the edge of a step down in the feeder (18" high) which dislocated his right shoulder.

He had surgery performed on his shoulder on 9-14-04. He had surgery performed on his shoulder on 9-14-04 which has resulted in time lost from his job.
