The bulk loader proceeded to clear the air slide and opened the clay belly to address a clog using an air hose. Upon opening the air valve, cement was expelled in the opposite direction, resulting in cement entering THEIR left eye. Eye wash station used, but followed up with eye doctor.

Mechanic was climbing out of bucket elevator and felt a pop in the right knee. Employee was still able to perform task and did not miss work until surgery date of 12/4/2025.

A 45 ton haul truck pulled away from the dump area with the bed still partially raised, it then snagged a powerline, pulling it down. Personnel were evacuated to a safe distance, power was disconnected and the equipment removed

Employee was using a 4" Dewalt Grinder to clean up a weld. The grinding disk broke apart and a piece of the the disk lacerated the back of left hand behind the middle finger. Employee was transported via company vehcile to an urgent care facility and received sutures

Employee was in the process of putting a bolt in a steel beam. With EE's left hand holding the bolt in the top bolt hole. As the left structure piece was closing the gap between the end plate and the main beam the tip of EE's left thumb was briefly caught in the gap.

Employee was welding on the ground level, suddenly EE turned to the opposite side of work area and tripped over a piece of wood lying on the ground. EE fell placing hand on the edge of a structural component H-Bar 3" flange x 4" x 6" base, the sharp edge of the material caused a deep cut in EE's right hand. EE was wearing all the PPE and proper gloves.

EE was replacing a wheel on the pan conveyor and wheel needed to be realigned to remove. EE hit the wheel with a hammer and a metal piece fractured off the hammer and hit the EE in the chest. EE was taken to urgent care where they were able to remove the metal piece and stitch up the puncture.

Crusher Operator was moving a wheelbarrow full of material and as it was tipping over they tried to catch it and strained a shoulder and bicep. The EE is still able to perform normal task and has not missed work. MRI results came back in November showing torn rotator cuff and EE will need surgery.

Miner was lifting 75lb vibrators to install into vibrator brackets on side of outlet hopper on a railcar. EE placed 3 into their resected brackets but when went to lift the 4th vibrator was struck with sharp lower back pain. Railcar is 37" height and 32" from ground to bracket, and 16" deep. Area was free of debris and without any trip hazards.

Employee was working on preheater tower, completed the job and went down elevator. Elevator stopped working while miner was inside at 1805. Doors would not open and elevator would not move. An electrician was able to get the elevator to move to the second floor where they opened the elevator doors and miner was able to safely get out at 1930.

Was breaking up hardened material with jackhammer, jackhammer broke causing loose material to fall and EE slipped on loose material. EMTs were called and EE was released from ER that night and followed up with DR who put them on restrictions:NO heavy lifting, pushing, pulling, climbing, overhead reaching, only sedentary work. Will be reevaluated 3/17/21.

EE lost footing while they bent over to pick up an extension cord in front of the main office building. The EE caught balance by placing their right hand on the rim of a metal drain cover. EE sustained a laceration to the web of the thumb requiring 6 stitches. EE was released with no restrictions.

An EE was closing a silo gate, in a vertical motion, and got left pinky finger caught in between the gate and the metal frame as it closed. The EE sustained a small fracture to the small bone in the pinky finger tip.

Electrical contractor fell thought ceiling tile while running electrical wiring for new motor control panels. Miner did not use the fall protection harness and misjudged the stability of the ceiling tile to hold ee's weight. Miner was trained on fall protection use. Miner sustained fractures to head and left wrist from the fall.

Employee was helping to load I-beams on a dolly. EE lifted the end about a foot and a half and set on dolly. EE felt shooting pain during lift and EE's hip to ankles felt bad. EE continued to work until end of shift at 7:00 pm. Management was made aware of the incident on December 23rd.

Employee was on the forklift transferring a cooling pipe from the front of plant to the back side where the other crew members were waiting for it. Pipe began to roll off the forklifts hooks, employee then proceeded to get off the forklift and tried to stop the pipe from rolling completely off. The pipe fell and landed on right foot.

Employee spotted a truck to be loaded and had the spout in the down position in the trailer. He gave hand signal to driver to come inside and driver began to move truck and trailer in reverse. With the spout still in the trailer, the employee banged on the window pane in front of him to attempt to get the driver to stop. The window broke and cut the employee's wrist.

Employees were closing bay doors to mobile equipment shop. One employee was inside pulling the door shut and another employee was outside pushing the door. The outside employee did not realize his finger was in the line of fire and overlapped into the inside of the door frame. Once the door reached the closed position the employee's finger was caught between the door frames.

Employee was manually cleaning a belt scraper by bouncing the handle of the scraper on the roller. A spring held tension on the scraper and when the employee raised the handle past the half way point it swung around and hit him on the left side of his nose.

Employee was cutting bolts from a bearing house when the wheel of the grinder kicked back causing his left hand to loose grip with the grinder. The left hand came in contact with the cutting wheel causing a 1" laceration to his left index finger knuckle. Injury required 3 stitches.

The employee was welding overhead when he smelled something burning. It was his shirt and reflective vest that was burning. He put out the flames and was taken to the hospital. Contributing cause was wearing the reflective vest required by the mine. The employee received 1st and second degree burns on his side and back.

Employee was climbing ladder outside #5 Mill Bldg. He was about on 5th rung when the ladder starting slipping on icy surface. Employee landed on his back with ladder on top of him & his left foot caught between 5th rung & outside of ladder. He was taken by ambulance to Medical Center Hospital ER. He sustained fracture to left fibula & tibia above ankle joint requiring surgery.

Employee was on front ladder of haul truck to remove cab compartment. When he tried to remove cover, it stuck but suddenly loosened causing him to lose his balance, so he jumped back off ladder landing on his heels on concrete pad about 4 ft. below. This caused injury to left ankle resulting in use of ankle splint.

EE was power washing the exterior of the finished cement tube, when a foreign particle got in his right eye. Particle was probably a spec of metal or paint from the power washing. (EE was wearing Safety Glasses during that job-His Supervisor has informed the office today that Safety Goggles have been provided for the crew.)

Employee was cleaning below the clinker stacker belt where buildup had accumulated. He had one knee in the floor while poking the buildup with a 3 ft steel bar. He thinks he raised the bar too high where the belt caught it and pull it toward him hitting him in the chin making about a 1/4 inch cut.

EE was working on roll crusher & found a leak in pressurized 1" nitrogen hose. As he tightened fitting to stop leak, the hose blew off striking him on the side of his head & across upper right back & shoulder, knocking him into the handrail. EE was transported to ER. He sustained bruises & abrasions to back/shoulder area. Possible hearing loss (too early to tell).

At approx 12:30 p.m. smoke was detected from duct out of EEF8, the fan for EBH8. The diverter gate to #1 Separator was closed off allowing all the heat into EBH8, resulting in ignition of bags. Odessa Fire Dept responded and no action was taken. Fire destroyed about 960 bags.

Men were sorting mill balls. The pedals on the bobcat got stuck and driver lost control of the bobcat. The mill balls spilled over the top of the hopper and hit two of the men sorting the balls.

Men were sorting mill balls. The pedals on the bobcat got stuck and driver lost control of the bobcat. The mill balls spilled over the top of the hopper and hit two of the men sorting the balls.

EE & ANOTHER EE WERE MOVING THE GUARD ON 62BC-1 COAL CONVEYOR BELT WHEN EE CAUGHT HIS FINGER BETWEEN GUARD & FRAME OF 62BC-1. ANOTHER EE WAS CLEANING OUT EE'S WOUND WHEN INJ EE FAINTED. BECAUS E EE FAINTED HE WAS TAKEN TO MED CTR HOSP WHERE THEY CLEAN & BANDAGED HIS FINGER. HE DID NOT FAINT DUE TO TRAUMA OF THE INJURY

EE, OTHER, AND ANOTHER FIRST REMOVED THE BUTTERFLY VALVE BECAUSE IT WAS NOT OPENING PROPERLY. THEY COMPLETELY REMOVED THE VALVE AND ONCE THEY GOT IT TO WORK PROPERLY THEY RESINSTALLED THE VALV E. THE VALVE-ONCE IN PLACE- WOULD NOT OPEN PROPERLY.
