Employee was fueling front end loader and walked away from the equipment and when EE heard the fuel overflowing ran to stop it, stepped on a rock and employee sprained the ankle.

Mechanic was climbing out of bucket elevator and felt a pop in the right knee. Employee was still able to perform task and did not miss work until surgery date of 12/4/2025.

While making wooden storage boxes EE attempted to drive a 4 inch screw through a 2x4 using an unstable support, the screw pierced through the wood, EE's shoe, and into EE's right foot at the base of the middle toe.

While exiting a man lift the employee stepped on a impact and twisted right ankle.

A 45 ton haul truck pulled away from the dump area with the bed still partially raised, it then snagged a powerline, pulling it down. Personnel were evacuated to a safe distance, power was disconnected and the equipment removed

Employee was using a 4" Dewalt Grinder to clean up a weld. The grinding disk broke apart and a piece of the the disk lacerated the back of left hand behind the middle finger. Employee was transported via company vehcile to an urgent care facility and received sutures

Employee was in the process of putting a bolt in a steel beam. With EE's left hand holding the bolt in the top bolt hole. As the left structure piece was closing the gap between the end plate and the main beam the tip of EE's left thumb was briefly caught in the gap.

Right thumb tip was pinched between pallet jack and dunnage.

Employee was welding on the ground level, suddenly EE turned to the opposite side of work area and tripped over a piece of wood lying on the ground. EE fell placing hand on the edge of a structural component H-Bar 3" flange x 4" x 6" base, the sharp edge of the material caused a deep cut in EE's right hand. EE was wearing all the PPE and proper gloves.

EE was replacing a wheel on the pan conveyor and wheel needed to be realigned to remove. EE hit the wheel with a hammer and a metal piece fractured off the hammer and hit the EE in the chest. EE was taken to urgent care where they were able to remove the metal piece and stitch up the puncture.

The Employee was blow drying the ceiling in the air compressor, cleaning the debris

Crusher Operator was moving a wheelbarrow full of material and as it was tipping over they tried to catch it and strained a shoulder and bicep. The EE is still able to perform normal task and has not missed work. MRI results came back in November showing torn rotator cuff and EE will need surgery.

Grinding on duct, raised face shield, debris got in right eye

Employee was holding a piece of rebar vertically with left hand to drive into ground. Employee was holding a sledgehammer with their right hand, chocked up to the head. As they were hitting the rebar, they missed and smashed the right index finger between the handle and the top of the rebar.

Worker lacerated skin on four fingers using a saw cutting wooden wedges

Miner was lifting 75lb vibrators to install into vibrator brackets on side of outlet hopper on a railcar. EE placed 3 into their resected brackets but when went to lift the 4th vibrator was struck with sharp lower back pain. Railcar is 37" height and 32" from ground to bracket, and 16" deep. Area was free of debris and without any trip hazards.

Employee was working on preheater tower, completed the job and went down elevator. Elevator stopped working while miner was inside at 1805. Doors would not open and elevator would not move. An electrician was able to get the elevator to move to the second floor where they opened the elevator doors and miner was able to safely get out at 1930.

Were removing interference fitted sprocket while installing bucket elevator sprocket under pressure with porta power ram, the bolt to the puller broke while employee was in front and struck employee left outer bicep.

Was breaking up hardened material with jackhammer, jackhammer broke causing loose material to fall and EE slipped on loose material. EMTs were called and EE was released from ER that night and followed up with DR who put them on restrictions:NO heavy lifting, pushing, pulling, climbing, overhead reaching, only sedentary work. Will be reevaluated 3/17/21.

When attempting to open difficult railcar out gate, employee applying pressure to bar lost EE's balance when the gate came loose. EE fell to the side, same level, rolling EE's right ankle.

Audiologist evaluation of annual audiogram shows that a reportable hearing loss has occurred.

EE lost footing while they bent over to pick up an extension cord in front of the main office building. The EE caught balance by placing their right hand on the rim of a metal drain cover. EE sustained a laceration to the web of the thumb requiring 6 stitches. EE was released with no restrictions.

An EE was closing a silo gate, in a vertical motion, and got left pinky finger caught in between the gate and the metal frame as it closed. The EE sustained a small fracture to the small bone in the pinky finger tip.

Contractor was repair a coupling when the machine turned back on and caught EE's hand.

Electrical contractor fell thought ceiling tile while running electrical wiring for new motor control panels. Miner did not use the fall protection harness and misjudged the stability of the ceiling tile to hold ee's weight. Miner was trained on fall protection use. Miner sustained fractures to head and left wrist from the fall.

Employee was helping to load I-beams on a dolly. EE lifted the end about a foot and a half and set on dolly. EE felt shooting pain during lift and EE's hip to ankles felt bad. EE continued to work until end of shift at 7:00 pm. Management was made aware of the incident on December 23rd.

Employee was on the forklift transferring a cooling pipe from the front of plant to the back side where the other crew members were waiting for it. Pipe began to roll off the forklifts hooks, employee then proceeded to get off the forklift and tried to stop the pipe from rolling completely off. The pipe fell and landed on right foot.

Employee was shoveling material and as he was shoveling he felt a sharp pain in his back. Employee overstrained or twisted his back wrong and pulled a muscle.

Employee spotted a truck to be loaded and had the spout in the down position in the trailer. He gave hand signal to driver to come inside and driver began to move truck and trailer in reverse. With the spout still in the trailer, the employee banged on the window pane in front of him to attempt to get the driver to stop. The window broke and cut the employee's wrist.

Employees were closing bay doors to mobile equipment shop. One employee was inside pulling the door shut and another employee was outside pushing the door. The outside employee did not realize his finger was in the line of fire and overlapped into the inside of the door frame. Once the door reached the closed position the employee's finger was caught between the door frames.

Employee was manually cleaning a belt scraper by bouncing the handle of the scraper on the roller. A spring held tension on the scraper and when the employee raised the handle past the half way point it swung around and hit him on the left side of his nose.

Subcontractor was swinging a sledge hammer against mill shell. He slipped, lost his balance, hit his face on the mill shell and busted his lip.

Employee was cutting bolts from a bearing house when the wheel of the grinder kicked back causing his left hand to loose grip with the grinder. The left hand came in contact with the cutting wheel causing a 1" laceration to his left index finger knuckle. Injury required 3 stitches.

Employee was trying to unplug Silo 4 discharge. He opened an inspection port to see if the material was flowing and material flushed out and got into his eyes. Employee was wearing safety glasses and proper PPE.

Employee climbed up on loader bucket to clear hopper and fell approximately 5 feet off side of the loader bucket causing a sprain to his left ankle. Employee did not follow safety procedures and was not tied off.

Employee was trying to open gate on 54 feeder with a pry bar. The bar slipped when feeder gate broke loose causing his left ring finger to be caught between bar and feeder pan. This resulted in a crushing injury and fracture of left ring finger.

The employee was welding overhead when he smelled something burning. It was his shirt and reflective vest that was burning. He put out the flames and was taken to the hospital. Contributing cause was wearing the reflective vest required by the mine. The employee received 1st and second degree burns on his side and back.

Overhead hoist was being utilized to lift liners into mill. Liner became hung when entering doorway causing choker to break. Choker hit person in forehead below hard hat protective area.

Driving a split bearing with a punch and hammer and a piece came off and hit him in the stomach.

Employee was climbing ladder outside #5 Mill Bldg. He was about on 5th rung when the ladder starting slipping on icy surface. Employee landed on his back with ladder on top of him & his left foot caught between 5th rung & outside of ladder. He was taken by ambulance to Medical Center Hospital ER. He sustained fracture to left fibula & tibia above ankle joint requiring surgery.

Employee was on front ladder of haul truck to remove cab compartment. When he tried to remove cover, it stuck but suddenly loosened causing him to lose his balance, so he jumped back off ladder landing on his heels on concrete pad about 4 ft. below. This caused injury to left ankle resulting in use of ankle splint.

EE was power washing the exterior of the finished cement tube, when a foreign particle got in his right eye. Particle was probably a spec of metal or paint from the power washing. (EE was wearing Safety Glasses during that job-His Supervisor has informed the office today that Safety Goggles have been provided for the crew.)

Employee was unloading rocky gypsum when an updraft from unloading pit caught him in the face causing particles to get in his eyes. This caused a corneal abrasion to his left eye requiring use of antibiotic eye drops.

Employee was closing valves on line 2 baghouse, as he stepped backward his right foot rolled on its side causing ligament injury (twisted ankle).

Employee was using heavy hammer to loosen hardened material that had built up around cover/lid on top of railcar. When lid was struck with hammer, the lid rebounded and struck employee's left little finger causing dislocation.

EMPLOYEE WAS TRYING TO GET BOLT LOOSE ON NOSE CASTNG. HE HIT IT WITH A HAMMER AND A METAL SLIVER FLEW OFF THE NOSE CASTING AND CUT HIS LEFT FOREARM.

Employee was cleaning below the clinker stacker belt where buildup had accumulated. He had one knee in the floor while poking the buildup with a 3 ft steel bar. He thinks he raised the bar too high where the belt caught it and pull it toward him hitting him in the chin making about a 1/4 inch cut.

EE was working on roll crusher & found a leak in pressurized 1" nitrogen hose. As he tightened fitting to stop leak, the hose blew off striking him on the side of his head & across upper right back & shoulder, knocking him into the handrail. EE was transported to ER. He sustained bruises & abrasions to back/shoulder area. Possible hearing loss (too early to tell).

Employee was in clinker tunnel jackhammering a large clinker chunk. When the jackhammer broke through the chunk, employee lost his balance causing him to slip and fall on his right leg and twisting his right ankle.

Employee was holding up window on haul truck with left hand while closing the door with right hand. He caught his left-hand ring finger in the door resulting in a smashing injury and a laceration requiring stitches.

Employee was pouring concrete and took off safety glasses to wipe off face and got splattered with concrete in both eyes.

Employee was walking into feed-end building where GFK2 feed pump is located. The clean-up people had suck truck and hose on ground. When employee stepped over hose, he caught his left foot on hose and came down hard on ground striking left knee.

A chunk of clinker fell on and bounced off the belt in the clinker tunnel, striking EE on his lower back.

Employee was leaning over stirring paint in 5 Gal. bucket on outside platform. Clinker dust was blowing in the area due to light winds. A Particle of clinker blew into his right eye from behind. Employee was wearing safety glasses.

Employee was operating forklift in windy conditions when particles flew in behind safety glasses and lodged in right eye. Eye was flushed but still irritated. Employee was taken to eye doctor and nothing was found to be in his eye, but doctor prescribed antibiotic eye drops.

At approx 12:30 p.m. smoke was detected from duct out of EEF8, the fan for EBH8. The diverter gate to #1 Separator was closed off allowing all the heat into EBH8, resulting in ignition of bags. Odessa Fire Dept responded and no action was taken. Fire destroyed about 960 bags.

Men were sorting mill balls. The pedals on the bobcat got stuck and driver lost control of the bobcat. The mill balls spilled over the top of the hopper and hit two of the men sorting the balls.

Employee and co-worker were carrying a 7.5hp motor (approx 125 lbs) up some stairs when employee heard and felt right knee pop. Employee has continued working but knee did not improve and surgery was performed on February 9, 2007. Employee returned to work on February 12th with restrictions.

Employee was removing 4" hose from K1. When coupling was released, the hose sprang back and hose coupling struck employee just below right nostril causing a laceration. He was taken to the emergency room and received stitches.

Employee was on callout to repair GBC4. Employee was on slope of tunnel walkway when clinker rolled under left foot causing him to twist left knee. Employee was taken off work October 16, 2006 due to allergic reaction to injection that caused left leg to swell.

Employee was making repair to water pump housing on plant truck. Drill bit was too large and as he was changing bit, he accidentaly engaged drill. Employee's left thumb was cut and required stitches. Employee failed to disconnect power before changing bit.

Employee was welding on 59 screw with proper protective equipment. Wind was blowing about 30 mph and caused unknown particles to get in right eye. Eye continued to bother employee and he was seen by an eye doctor on February 17 and received antibotic eye drops.

Employee was helping install new hammer on crusher. The pin was being lowered & shackle got lodged. When employee dislodged, due to slack in hoist cable, the pin slid down against metal step. Employee's right hand was caught between pin and the step, causing broken little finger and laceration and fracture to ring finger.

Employee was poking preheater on A level and hit a hard spot, causing him to strain his right elbow and forearm. He was seen by doctor and placed on restrictions.

SUPERVISOR STAGING LINERS FOR THE FINISH MILL WHEN LEFT HAND MIDDLE FINGER GOT SMASHED BETWEEN LINER AND STEEL PIPE. MINER RECEIVED STITCHES. PROJECT COMPLETED 11/18/05. 11/28/05 miner returned to dr for stitch removal when dr decided to amputate tip of left hand middle finger. On 11/29/05 tip removed. No work was scheduled for miner. Miner working at a project as of 12/4/05.

Miner was affected by smoke while removing liners from Finish Mill. Miner was taken under his own power to the hospital where he was treated with fluids (I.V.)

Employee was giving signal to crane operator and operator stepped on boom pedal instead of brake causing boom to strike employee on front side of body. Employee continued to work full duty and asked to see doctor on 11/4/05 and was released with restrictions.

Replacing motor, heads, and rubber on Bleuler Mill. Process was repeated several times resulting in overexertion of left elbow. Employee sought treatment and continued to work full duty. Surgery performed March 30, 2006 and employee has returned to work with restrictions.

While sitting in passenger seat of water truck, the wind caught the open door slamming it on the left hand thumb. This caused laceration that required stitches. The work area is rather confined contributing to the injury.

Employee was walking beside water treatment building and stepped on a rock, causing foot to slip & twisted right knee. Employee was seen by doctor and continued to work at full duty. His injury did not improve and surgery was done on June 20, 2005 to repair meniscus tear.

Employee was attempting to get sand flowing out of bottom of hopper with a pry bar. He did not stop the belt or lock it out. The moving belt caught the pry bar causing it to jerk and smashing his left hand between pry bar and hopper. Cut on knuckle of middle finger required stitches.

Employee stepped out of crane onto a pile of rocks causing him to slip, twisting his left knee. Taken off work starting 1/19/05 pending surgery to repair miniscal tear.

EMPLOYEE WAS CLEANING #59 SCREW, TRYING TO UNPLUG IT ON THE SOUTH SIDE. HE WAS TAKING SCREW COVER BOLTS OFF AND HOT MATERIAL GUSHED OUT OF THE SEAL, FALLING ON TOP OF HIM. HE HAD PROPER GEAR ON BUT DID NOT HAVE HIS GLOVES INSIDE HIS JACKET SLEEVE. HOT MATERIAL GOT INTO HIS RIGHT GLOVE CAUSING SECOND AND THIRD DEGREE BURNS TO HIS RIGHT HAND AND WRIST.

EE went to clean #58 screw. He stood on top of screw (approx 4-5' off ground) and opened lid, inserted the air lance. He had not yet turned on the air and hot material puffed out and hit his face. He jumped down and landed in a pile of hot material located next to screw. he quikly got out of pile and ran to get help.

EE went to retrieve a mid kiln sample. He found the port plugged so he inserted a rod to loosen material. The rod became stuck, so he had to pull hard to remove. The rod became free and hot feed spilled from the port and entered the gap between the cuff of glove and coat sleeve. 2nd degree burns to lower arm and 1st degree on upper arm.

EE WAS WALKING DOWN STAIRS ON SOUTH SIDE OF GBC6. HIS RIGHT FOOT SLIPPED ON BUILDUP, CAUSING KNEE TO TWIST. THOUGHT TO BE A STRAIN UNTIL RESULTS OF MRI REVEALED FRACTURE OF TIBIA. MUST USE CRUTCHES. EE IS WORKING RESTRICTED DUTY AT THIS TIME.

DRIVER AND PASSENGER WERE TRAVELING EAST ON THE PLANT ROAD SOUTH OF GBH8 IN A COMPANY PICKUP. DUE TO DIRTY WINDSHIELD AND MORNING SUN, DRIVER VEERED OFF ROADWAY AND STRUCK A LARGE ROCK. DRIVER SUSTAINED BRUISES THAT DID NOT REQUIRE MEDICAL ATTENTION. PASSENGER STRUCK WINDSHIELD AND DASHBOARD CAUSING BRUISES TO FACE AND A CUT ABOVE HIS RIGHT EYE REQUIRING STITCHES.

EMPLOYEE WAS ASSISTING MAINTENANCE DURING KILN OUTAAG, REPLACING BELTS ON #1 COOLER FAN. EMPLOYEE CAUGHT MIDDLE, LEFT FINGER BETWEEN BELT & SHEAVE. HE WAS SEEN BY A DR. WHO CLEANED & BANDAGED THE WOUND. AN X-RAY REVELASED A FRACTURE BETWEEN THE TIP OF HIS FINGER & HIS 1ST KNUCKLE. NO STITCHES WERE REQUIRED. EMPLOYEE WAS GIVEN A TETANUS SHOT & RETURNED TO RESTRICTED DUTY.

Employee was working on ES4 separator, while putting board into seperator his left knee buckled under. He was seen by a doctor on April 30 and placed on restrictions. Injury type is unknown at this time, pending further medical tests.

EE WAS SENT TO SODIUM FEEDER DUE TO DEVIATION ONFLOW. EE GOT DOWN ON HIS KNEES TO HIT THE CHUTE BELOW THE FEEDER WITH HIS HAMMER. HE PLACED HIS HAND ON THE SOUTH SIDE OF THE DRIVE CHAIN GUARD. HIS RIGHT FINGER SLIPPED INTO THE GAP BETWEEN THE GUARD & BACK PLATE & ENTERED THE PINCH POINTBETWEEN THE SPROCKET & CHAIN. THE TIP OF HIS RIGHT MIDDLE FINGER WAS LACERATED AT THE TIP PULLIN

EE STEPPED ON PIECE OF ROLLER BEARING ON MAINTENANCE SHOP FLOOR. THIS CAUSED HIS ANKLE TO ROLL & HE FELL TO THE FLOOR. ANKLE SWOLLEN SO DR. WAS UNABLE TO X-RAY. FOOT PLACED IN AIR SPLINT. EE ON RESTRICTED DUTY.

EMPLOYEE WAS WELDING AT CRUSHER AND RECEIVED FLASBURN TO EYES AND SLIVERS OF METAL. EMPLOYEE WAS USING PROPER PPE. HE WAS SENT TO THE EYE DOCTOR WHO GAVE HIM PRESCRIPTIONS FOR EYE DROPS.

WHILE WALKING ON 3RD FLOOR BETWEEN #2 AND #3 MILL, EE RAISED HEAD AND STRUCK HEAD ON BEAM, CAUSING NECK TO JAM. EE WAS WEARING HARD HAT. HE CONTINUED TO WORK UNTIL DR TOOK HIM OFF WORK STARTIN G JUNE 17, 03.

TWO EMPLOYEES WERE REMOVING COVER FOR COAL ROLL REMOVAL. ONE WENT TO THE MAINTENANCE SHOP TO GET A SLING FOR LIFTING THE COVER. WHILE HE WAS GONE, THE EMPLOYEE REMOVED THE BOLTS FROM THE COVER AND TURNED AROUND TO PICK UP SOMETHING. THE COVER CAME LOOSE HITTING HIM IN THE BACK OF HIS LEFT LEG JUST ABOVE THE CALF. SCRAPES & SORENESS LATER DIAGNOSED AS A TEAR BY THE TREATING DOCTOR.

EMPLOYEE WAS EXITING PREHEATER ELEVATOR THAT HAD STOPPED ABOUT 6 TO 8 INCHES ABOVE THE FLOOR LEVEL. HE STEPPED DOWN AND TWISTED HIS RIGHT KNEE.

EMPLOYEE WAS CARRYING SOME HEAVY PARTS ON AN UNEVEN SURFACE IN #2 COOLER AND SUSTAINED A STRAIN TO LOWER LEFT LEG AND KNEE. NOTIFIED SUPERVISOR ON 1/9/03.

EE WAS REMOVING BOLTS IN #2 COOLER & HAD STOPPED TO TAKE HIS SCHEDULED BREAK. HE STARTED EXPERIENCING CHEST PAIN & NUMBNESS IN HIS LEFT ARM. HE WAS TRANSPORTED TO THE HOSPITAL FOR EVALUATION. HE HAD 99% BLOCKAGE IN ONE ARTERY & A STINT WAS PUT IN PLACE.

EE WAS WORKING ON #1 COAL MILL & GOT FOREIGN OBJECT IN HIS EYE. HE THOUGHT HE WAS ABLE TO RINSE THE OBJECT OUT, BUT A FEW DAYS LATER HIS EYE STILL HURT. HE WAS SENT TO THE EYE DOCTOR WHO REMOV ED THE FOREIGN OBJECT THAT WAS EMBEDDED.

EE WAS CALLED OUT OT WORK AT APPROX.10 PM TO REPAIR EAS4 AIRSLIDE. THIS REPAIR WORK REQUIRED HIM TO BE ON AND OFF HIS KNEES. THIS REPAIR WORK TOOK APPROX.12 HRS TO COMPLETE. HIS KNEE HAD START ED HURTING HIM ABOUT 5AM. WHEN THE JOB WAS COMPLETE HE WENT HOME AND ICED HIS LEFT KNEE, THINKING HE HAD JUST OVERWORKED IT. HE NOTICED HIS SUPERVISOR ON 8-14 THAT HIS KNEE STILL HURT AND A D

EE AND CO-WORKER WERE SEALING A HOLE ON THE 3RD STAGE EXPANSION JOINT ON THE 2ND FLOOR OF THE PREHEATER TOWER. CO-WORKER WAS CROUCHED ON ONE SIDE OF THE PIPE SCRAPING OFF THE OLD PATCH. EE WAS CROUCHED ON THE OTHER SIDE OF THE PIPING IN FRONT OF THE OPEN HOLE. HOT MATERIAL SPEWED FROM THECRACK IN THE PIPE AND STRUCK EE AROUND THE RT. EAR, FACE AND ARM. SMALL 2ND DEG. BURN ON EAR.

EMPLOYEE WAS WALKING ON CATWALK BY KILN, AND TRIPPED OVER HIS OWN FEET. HE PUT OUT HIS HAND TO BREAK HIS FALL AND BROKE BONES IN RIGHT HAND AND SCRAPED FACE. EMPLOYEE WAS RELEASED TO RETURN TO WORK WITH RESTRICTIONS.

EE USED CONDUIT TO PULL HIMSELF UP TO THE TOP OFSCREW CONVEYOR RATHER THAN USING A LADDER. AT THE TIME, IT WAS BELIEVED TO BE A STRAINED SHOULDER. HE HAD SURGERY ON 6-3-02. HE HAD CALCIFIC TEN DITITIS AND A TENDON TEAR. HIS SHOULDER IS TO REMAIN IN A SLING FOR 6 WEEKS. THE DR. ANTICIPATESTHAT IT WILL BE SIX MONTHS BEFORE HE CAN DO UNRESTRICTED HEAVY ACTIVITIES.

EMPLOYEE WAS ON 4TH FLOOR OF FINISH MILL BUILD- ING USING A CUTTING TORCH. THE FLAME IGNITED FUEL FROM A LEAK IN THE HOSE, CAUSING A BURN TO THE LEFT HAND BETWEEN THE THUMB AND FOREFINGER. THE EMPLOYEE WAS WEARING GLOVES AT THE TIME. THERE WAS NO DAMAGE TO EQUIPMENT.

EMPLOYEE RESPONDING TO LOW SODIUM FAULT ALARM. THIS USUALLY MEANS THE ROTARY FEEDER HAS STOPPED. WHEN HE ARRIVED AT ROTARY- IT WAS STOPPED. HE STUCK HIS HAND IN TO CLEAR IT OUT, AS HE WAS CLEARING FEEDER OUT, THE FEEDER ENGAGED AND CAUGHT HIS RIGHT HAND (3 FINGERS). CALLED FOR HELP ON RADIO- POWER WAS TURNED OFF AND HE WAS FREED FROM FEEDER BY 2 EMPLOYEES. FRACTURE OF 1F

EMPLOYEE WAS TOSSING BROKEN SHANK INTO BED OF PICKUP AND CAUGHT RIGHT INDEX FINGER ON PART OF SHANK. HE WAS NOT WEARING PROTECTIVE GLOVES. HIS FINGER WAS LACERATED REQUIRING STITCHES.

EE WAS CLEANING UP AROUND FEEDER IN CLINKER TUNNEL AND GOT DUST IN EYES. HIS EYES WERE FLUSHED AT THE PLANT BUT IRRITATION CONTINUED. EE WAS SEEN BY AN EYE DR. AND RECEIVED A PRESCRIPTION FOR EYE DROPS.

MINER PRESSED THE OPEN BUTTON ON AN OVERHEAD DOOR. AS HE STOOD THERE WAITING FOR THE DOOR TO RISE, THE SPROCKET ON THE DRIVE MECHANISM CAME LOOSE, FELL AND STRUCK THE MINER ON HIS HARD HAT KNO CKING HIM UNCONSCIOUS AND LACERATING HIS SCALP.

EE WAS CLEANING ANALYZER INLET PRIOR TO SLIDING PROBE IN. HOT DUST FROM BACK OF #2 KILN ANALYZER PROBE PUFFED OUT ONTO FACE AND IN RIGHT EYE. EE WAS WEARING SAFETY GLASSES, BUT FROM NOW ON A F ACE SHIELD WILL BE REQUIRED WHEN WORKING ON THISITEM.

#44 GYP FEEDER WAS PLUGGED AND COULD NOT UNPLUG FROM BELOW. EE WENT ON TOP WHERE FLOOR WAS SLIPPERY AND LIGHT WAS LOW. HE WAS UNAWARE OF WHERE CHUTE WAS AND WHEN HE STEPPED ON PLUGGED MATERIAL , HIS WEIGHT CAUSED MATERIAL TO FLOW AND LEFT LEG WENT DOWN CHUTE AND RIGHT LEG REMAINED ON TOP.

EMPLOYEE WAS CLEANING INSIDE SILO #3 AND CEMENT PARTICLES FELL OFF WALL GETTING IN HIS EYE. EYES WERE RINSED AND PLANT AND HE WAS SENT TOT HE DOCTOR, WHO PRESCRIBED EYE DROPS.

EE WAS SHUTTING TAILGATE ON DUMP TRUCK & CAUGHT HIS 3RD & 4TH FINGERS ON HIS RIGHT HAND BETWEEN THE TAILGATE & TRUCK. HE WAS TAKEN TO EMERGENCY,X-RAYED & FINGERS SPLINTED. HE RETURNED TO WORK.

EE & ANOTHER EE WERE MOVING THE GUARD ON 62BC-1 COAL CONVEYOR BELT WHEN EE CAUGHT HIS FINGER BETWEEN GUARD & FRAME OF 62BC-1. ANOTHER EE WAS CLEANING OUT EE'S WOUND WHEN INJ EE FAINTED. BECAUS E EE FAINTED HE WAS TAKEN TO MED CTR HOSP WHERE THEY CLEAN & BANDAGED HIS FINGER. HE DID NOT FAINT DUE TO TRAUMA OF THE INJURY

EE WAS WORKING IN ES2 & GOT CEMENT PARTICLES IN HIS RIGHT EYE. HE USED EYE WASH AT THE PLATN & WAS UNABLE TO DISLODGE DEBRIS. THE EYE DR. REMOVED THE PARTICLE UNDER HIS RIGHT EYELID & RX ANTIB IOTIC DROPS.

EE WAS WORKING 13 HR SHIFT DURING KILN OUTAGE. EE WAS COMING DOWN LADDER ON BURNER FLOOR & PULLED A MUSCLE BEHIND RIGHT KNEE

EE, OTHER, AND ANOTHER FIRST REMOVED THE BUTTERFLY VALVE BECAUSE IT WAS NOT OPENING PROPERLY. THEY COMPLETELY REMOVED THE VALVE AND ONCE THEY GOT IT TO WORK PROPERLY THEY RESINSTALLED THE VALV E. THE VALVE-ONCE IN PLACE- WOULD NOT OPEN PROPERLY.

EE WAS USING METAL BAR TO OPEN AND CLOSE RAIL CAR DOORS. BAR SLIPPED STRIKING EE ABOVE LEFT EYEBROW. CUT REQUIRED 3 TO 4 STITCHES. EE RETURNED TO WORK.

EE WAS WORKING ON RAW MILL. WHEN HE WENT TO STEP ON SCAFFOLDING, HE SLIPPED AND CAME DOWN ON LEFT KNEE.
