Laboratory technician was working with a box cutter knife when they went to preform a task in the lab. The knife slipped from their hand resulting in them instinctually catching the knife and causing the knife to cut their thumb.

After using plasma cutter, maintenance personnel lifted their face shield dislodging metal dust onto their face. Some of the dust went behind their safety glasses and into their left eye. Metal debris was removed from eye and antibiotic drops prescribed.

While conducting an end-of-month inventory of explosives supplies, Miner unlocked the powder magazine in Tunnel 1. Miner opened the door using their left arm to pull the heavy steel door open. As they opened the door they felt a pop, and a sharp pain in their left forearm. MRI results received on 9/9/2025 showing injury was a tear requiring surgery.

While removing tires from a dump truck to perform maintenance, the miner felt a pop in EE's left shoulder and a slight tingling in EE's arm. Injury occurred on 05/12/25 and treated as first aid until 06/20/25. On 06/20/25 it was diagnosed as a tear requiring surgery and miner was given restrictions.

Miner was assisting to shift the width of the forks on an extenda-reach forklift. While moving the forks, the miner felt a pain in EE's right forearm. The injury was initially treated as a first aid sprain, but the extended healing process led to a need for imaging which revealed a tear requiring surgery to repair. The determination that surgery was required was made on 5/16/25.

While bagging a load of 55-lbs. bags and placing them on a pallet, felt an intense sharp pain and since then whenever moving the arm there is a grinding/popping feeling and discomfort. The injury was originally diagnosed as first aid and treated accordingly. On 10/30/2024, the pain had not lessened and the doctor prescribed medication and work restrictions.

The operator sustained a strain injury while installing a new propane canister on a forklift. During the installation the operator did not feel any significant strain or pulling, but immediately after completing the process they began to feel discomfort.

Maintenance member was replacing a belt on a mill fan when their fingers were pinched between the belt and the belt sheave. Their left index and middle finger received lacerations requiring a total of 13 stitches.

The miner was injured on 5/16/24 while moving a chair from on top of a desk. Per doctor, the injury was treated as a first aid strain until 8/5/2024 when imaging revealed a tear and surgery determined necessary.

Driver was travelling from the mine to the plant with a full load. The mine vehicle was approaching an intersection when a pickup truck crossing in front of mine vehicle failed to stop at the stop sign. The mine vehicle did not have a stop sign and did not have enough time to react to the other vehicle before impacting the rear driver side portion of the passenger vehicle.

EE attempted to pick up tire chock. Pain in right shoulder. Incident reported to supervisor at 7am. Pain decreased initially but over time EE could not lift arm over shoulder. Sought treatment on 7/6/23 and was given work restrictions with right arm.

On 8/3/2022 The Process Operator was working at Unit 3 operating the auto packaging system. At 2:40pm EE went to the bathroom at unit 3, upon leaving the bathroom EE missed the step and rolled EE's ankle. The Supervisor went and picked EE up and took EE to the minor emergency room. They did x rays and determined it was a sprained ankle with no fractures.

Employee was working on changing the belts on separator fan on mill 4. They had locked everything out and tried. As they were removing the belts the fan rotated and smashed the right thumb and tore off the fingernail on the left thumb as well as the skin. It looks like with other equipment running airflow caused the fan to move.

Miner was helping with a derailed rail car. EE reached under railcar to move support. While pulling hand out from under rail car EE's left hand contacted a bur on the metal of the car caused a laceration to the top of EE's hand near the thumb. Miner was not wearing gloves at the time.

Miner greasing the front end of the skid steer. He reposition the drum in to the secondary containment tray. He grabbed the lid of the 25 gl drum the lid slipped on the ring of the barrel and the tab came in contract with his right pinky finger. He received a small cut on his finger and had difficulty moving finger. Employee taken to emergency room. Laceration on the flexor tendon.

Employee with a pre-existing back injury futher exacerbated his injury while working on mill 5. He was in the process of replacing the journals. He stated that while swinging a hammer he hurt his back. He was sent to the company doctor and he recieved restrictions.

Performing maintenance on road grader mechanic sustained injury to his left hand. A team of two mechanics were replacing/repairing the axle on the road grader. The forklift operator was positioning the tire out of the way the tire came off the forklift on unlevel ground and kicked back pinching his hand between the tire and the equipment.

Electrician with 2 yrs mining experience was injured while laying internet cable above ground. Incident was not reported until 3/12/07 when the employee reported the injury and was put on a no work status until 3/28/07. Because employee did not report incident in a timely fashion and physicians were not free in giving medical information, investigation was delayed.

Truck driver was outside his truck walking and tripped and fell to his elbows while on loose rocks and gravel and bruised his elbows and right hand. He did not seek medical treatment at the time as he felt it was just a bruise. On 3-1-07 he still felt pain and went to doctor and X-rays revealed Navicular fracture R hand/wrist and elbow contusion. Driver now on restricted duty.
