Employee was found on the floor of the #2 slurry tank. Employee had significant head injuries and was pronounced dead by the Justice of the Peace. The accident scene is currently under a Section 103(k) order and subject to MSHA investigation.

Truck driver's lid got stuck on safety cage and when he tried to pry it free, it came loose and sprung and hit him. He fell 11 feet. This was told to me by Ralph Rodriguez.

Several utility workers were loading mill rods into the raw mill. As they were unloading a rod from the forklift forks, their finger was caught in between the hand rail and bottom of rod.

Production tech was removing a slurry hose for cleaning, EE had shut off all valves as well as open the pressure relief valve to get rid of excess pressure. When removing the camlock there was still residual pressure and caused slurry to spray on EE's face, chest and right wrist.

Electrician was performing normal duties, troubleshooting a blower motor. It was hot in the area they were in and began to feel ill. They radioed that they needed help and when other employees got to them called EMS when they arrived they gave them fluids and determined they were dehydrated.

Laborer was working with the vac truck and needed to cut the old hose off and add a new section. While cutting the bad section, employee was using a utility knife, cutting towards their body when the knife slipped and cut their hand, right above the wrist.

Contractors were in the clinker passenger elevator, the car stopped abruptly halfway down, entrapping them. They contacted their manager at 11:37 to alert them that they were stuck. At 12:07, the Safety Manager called MSHA to report an entrapment. Plant electricians were called to reset the car, they were successful and were able to get them to the ground and extracted safely.

A power outage at the 491 Substation knocked out power to the kiln system, which includes the PHT elevator.  At the time of the outage, a production technician was in the elevator and became trapped, at the 30 minute mark MSHA was notified. through the hatch at the top of the elevator car. Employee was able to self-rescue when power was not restored.

Employee was using a sledgehammer to break a mill door free, had to switch tools and when EE reached out to grab the other tool felt a pop in EE's lower back.

Upon exiting the elevator, the micro switch for the inner door failed not reading that the car was on the ground, and started going back up. The elevator went up about 12 feet and stopped, not allowing the employees to exit. There were no injuries sustained.

EE was working with a jack hammer and was removing material with a wheelbarrow (with the assistance of other EEs). EE left the shift fine, woke up the next day with back soreness. Was treated initially and told to tend with self-care. On 3/22 went to see a doctor, doctor diagnosed with a potential bulging disc. Eventually was prescribed, prescription grade medication on 3/27.

An experienced carpenter of 15 years was hired to build forms for concrete. While cutting stakes the IW encountered a knot. Holding the Makita circular saw, in left hand, the saw bound up and kicked back, striking IW's right thumb. IW was taken to the ER, treated and released. On 3/16/24 IW received stitches.

At 6:48 pm the console operator dialed 911 in response to a confirmed fire on the clinker belt that feeds our clinker shed. MSHA was notified around 7 pm, and the local Fire Department arrived on-site at 7:04 pm. The Fire Department was able to extinguish and secure the area within 15-20 minutes upon arrival.

EE reached overhead to disconnect an air line from a quick connect 3/8 air hose. The EE failed to bleed the pressure off the line before disconnecting it. EE lost control of the air hose it bounced off the structure and came back and struck EE on the upper lip requiring three stitches.

EE trapped in an elevator to clinker silos. Electricians had to be called out to access the emergency switches to raise the elevator. Techs arrived on 8/26 and found a limit switched failed and need to be replaced.

Fire occurred to the High Angle Conveyor (HAC) when power was lost to the plant. A set of triple airlock gates and a knife gate separate the belt from the hot gases. The knife gate failed to shut in fail-safe mode when a valve was inadvertently left off. The HAC belt ignited and burned near a Continuous Emission Monitoring Station. Six compressed gas bottles heated and ruptured.

EE was using a wire wheel on a grinder. It did have a guard and the trigger lock is not present. EE claims that the wheel caught and torqued toward them. It made contact with their right leg. EE was taken to the hospital and given stitches. No restrictions or light duty.

EE claims an injury on 2/19/19 that EE hopped and turned midair to avoid another EE brushing against EE. None of the crew remembers the incident. EE reported on 2/20/19. Seen by Dr. on 2/22/19 and diagnosed with Scoliosis. Medical evaluation on 3/25 determined pain to be work related.

Employee was using a pulley and winch snatch block to lower thimbles into a hole. When pulling the pulley/winch snatch block back up, EE grabbed the wire and it caught EE's left index finger and cut EE's finger below the finger nail.

EE was replacing a spout plug in the shipping silos spout. When EE looked up to align the plug with the bolt holes dust fell into EE's eyes. EE was wearing safety glasses and hard hat. EE had lowered the spout and knocked any loose material out before beginning work.

Fire was possibly started by welders working in the area earlier in the day. MSHA was called but the length of the fire did not meet MSHA report. Hot work permit issued and followed. Coal system has been locked out. No coal was in the day tank. Belting was the only loss. Fire was reported at 6:21pm and extinguished at 6:28pm.

At 10:16 an employee reported was stuck in the elevator at the cement silos. The elevator car was about 3 to 4 feet above ground. The employee was able to get out at 11:16. The elevator over speed brake locked and stops the elevator from operating.

Employee was using a come along and choker to pull the HDR. The HDR had become stuck and would not move in either direction. When EE pulled with the come along the choker failed and the hook came back and struck the EE. EE sustained a broken pinky finger on EE's right hand.

A small fire was located in the southwest corner of the coal stockpile. The excavator was located in the quarry and had to be brought up to stockpile to dig the fire out. Fire was extinguished at 9:48 while on the phone with MSHA 1 call. High winds and spontaneous combustion are the likely cause. No worker activity is being done in this area.

A glow on top of the coal stockpile was observed. It was confirmed to be 3 small flames. Because of location it required an excavator to extinguish. The excavator was located on the other side of the plant. Because of pre-shift and travel time it took longer than 30 minutes to extinguish. The fire was extinguished at 9:51 p.m.

Service Technician was diagnosing elevator issues from the 4/6/16 incident. The technician tripped a door safety switch and the elevator stopped. The elevator system would not reset and a second technician had to be called. It took about 30 minutes for that technician to arrive and reset the elevator.

Due to build up on bottom of vessel opening and with limited access to this area men began cleaning from below bottom opening. Material buildup on vessel broke loose during operation and struck men causing bodily injury.

EE was working on an auto sampler. Air was turned off and the electrical equipment was turned off. Residual air pressure in the lines allowed the unit to cycle when the EE freed the blockage with his finger. The EE lost about 1/4 inch of his left index finger including some bone. Stitches to close the wound

During a coal mill system start up, a fire occurred in the coal mill system. The fire forced open the pressure relief doors on the lower level of the bag house. The CO2 suppression system automatically initiated, and the fire was extinguished in approximately 10 minutes.

Employee was inspecting equipment (Iron Ore Slide Gate). After inspection, the slide gate was removed and in hand and brought towards his body to secure. Employee slightly turned his left side and the left side of his nose made contact with conduit support. Nose began to bleed.

A computer glitch during new kiln commissioning caused a kiln shutdown. Approx. 1200 degree F. air was forced into a rubber duct due to a drop in positive air pressure. The hot air eventually caused the duct to catch fire from the inside out.

EE (5 Point contractor) left a cresent wrench in the housing that he went to grab and when he placed his and on the angle iron frame to reach into fan housing the set screw that is a 7/16" bolt, which has been known to fail in previous encounter, sheared and caused the fan and housing to fall from set position to the bottom side of frame.

On 04/02/2014 the EE was shoveling sand in sand tunnel & felt a sharp pain in his back/buttocks area. He reported the incident on 04/03/14 to his Supervisor & was taken for MD assessment. EE was not assigned any restrictions, and returned to full duty that day. On 04/29/2014 EE asked reported pain, taken to MD & was given a steroid shot. No restrictions, & returned to full duty.

Employee had neck tilted back looking at light that was not working on elevation above and wind blew trash containing metal shavings into his eye. The operator & 2 helpers were attaching the jib to the boom of a 90 ton hydraulic crane. The operator removed the pin on the side of the boom before he installed the 2 pins that hold the jib in place. This caused the entire jib to drop to the ground. One side of the jib struck the operator's hard hat as it fell to the concrete paving.

EE was removing a 3/4" bolt using a Tone SR71E Simple Torque gun. As the bolt became loose, he released the trigger and reached in to remove the bolt while the gun was still spinning. His left thumb became pinched between the torque reactor bar and the steel beam. His basic reaction was to pull his thumb free. As he did, the pinched condition caused his thumb to be injured.

The welds on an anchor for a hand hoist failed under tension and metal anchor and buckle snapped back and hit his elbow. X-ray does not show fractures. MRI does not show fractures. He tried to step back to catch himself but a chain that holds the tailgate caught his leg and tripped him. He tried with his right hand to catch himself but it cut his hand and he still fell. He landed on his shoulder and hit his head.
