The continuous miner made contact with old mine works from an unknown origin. Water came from the old mine to Cabin Hollow mine. All persons were safely evacuated from the mine to the surface. The water has been pumped and equipment removed from the mine.

On the weekly examination of the bleeder system a roof fall was discovered in the number 1 entry between spad 227 and 244. The fall was approximately 20 feet long x 5 feet high x 160 feet long. The fall did not affect miners, ventilation nor equipment.

A roof fall was discovered in the #2 entry between #6 and #7 crosscut just outby spad 76. The fall is apx. 20ft long x 6ft high x 18 ft wide. No persons nor equipment were affected by the fall and it has been dangered off to possibly clean up at some point in the future.

A rock fall was discovered in the 1st south east mains in a mined out area during a weekly examination. The fall is one crosscut inby spad 556 and 564 across the old #3 and 4 entries. The fall is apx. 140 feet long x 20 feet wide x 7 feet high. No persons, equipment nor ventilation were affected. The area has been dangered off.

EE stated that he was installing a man door, when a piece of draw rock, appx. 1"x36'x36" fell and struck him on the back. EE continued to work until 8-8-08, when he consulted a Dr. that took him off work and started physical therapy.

Employee stated that as he was positioning the roof bolter the boom canopy struck a piece of rock causing it to fall striking him in the leg. The rock was appx. 15 inches x 15 inches x 4 inches thick. He continued to work until 5-10-08 when he was diagnosed with a blood clot.

EE dropped to his knees to use a hydraulic jack, when his left knee popped and hurt. EE continued to work regular duty until he had surgery on 6/4/08. He is currently off work.

Employee stated that he was roof bolting when the roof strap spun around and struck him in the nose. He was seen at the ER where he received 9 stitches to his nose. He returned to work on his next shift.

A rock fall was discovered in the pillar inby spad 1041 in the #3 left entry outby the working section. The fall is approximately 5' high X 15' long X 20' wide. The fall was discovered at 1:40 pm. The area has been dangered and cribbed off. The fall did not have any affect on persons, ventilation nor equipment.

The 002 section had pillared 5 rows without an initial fall. The crew noticed that a fall was impending and moved and timbered the equipment to a safe location. The fall rode through the breaker timbers 1crosscut inby spad 1291 appx. 70' x 20'w x 10' high. MSHA was present at the time of the fall.

THE FALL AREA IS APPROXIMATELY 20'X20'X5'HIGH. THE AREA HAS BEEN CLEANED UP AND RESUPPORTED. NO PERSONS, EQUIPMENT NOR VENTILATION WERE AFFECTED.

Employee was operating a rail runner that was connected by chain to another rail runner when the chain broke and came back and struck him in the forehead. He was transported to the ER where he received 15 stitches. Employee returned to work on the next scheduled shift.

EE WAS OPERATING THE RAIL RUNNER, WHEN A SHOVEL THAT HE HAD PLACED ON THE TOP OF THE RAIL RUNNER CAME INTO CONTACT WITH A SPILL BOARD AT A CONVEYOR BELT, CAUSING THE SHOVEL TO STRIKE HIM IN THE BACK OF THE HEAD. EE RECIEVED 5 STITCHES AND RETURNED TO WORK ON THE NEXT SCHEDULED SHIFT.

During the pre-shift examination prior to 2nd shift at 2:30 p.m. a rock fall was discovered in the #5 entry at spad 1040. The rock fall was approximately 20' X 19'x 5' high. This fall was reported immediately to MSHA supervisor and since the location was in the primary escapeway it was rerouted and all miners informed.

Employee was roof bolting when a piece of rock approximately 12"x12"x2" fell from the roof and struck his right little finger mashing it against the head of the roof bolter. He was transported to the hospital and received 20 stitches.

The fall extended from spad 982 thru 980, 981 and from the right break in #3 across to #5 right in the belt line in #4. The area had been abandoned and did not have any affect on miners, ventilation nor equipment. The area has been dangered and cribbed off.

Found driver slumped over wheel, attempt to wake him up, apparently heart attack. Emt/ems onsite attempted care and transported to local hospital where he was pronounced dead. MSHA was notified immediately when ems, an investigation was completed by MSHA on truck and site, no violations were found.

Employee stated that he was loading rock dust onto the scoop when he turned with a bag of dust in his arms and hurt his left knee. He continued to work his regular job and consulted a Dr. who performed surgery on 3-28-07 to repair cartlidge in his knee.
