Employee lost balance after bending over to remove a wheel chock. Fell backwards onto back. Employee was transported to the ER via ambulance and released the same day. (Per a MSHA accident investigation, this is not a reportable accident.)

Employee was having symptoms of a heart attack prior to reporting to work. Other Employees notice and took action to get him to the hospital safely. It was later determined not to be life-threating or work related.

Injured employee exited a machine loaded on a bean trailer, had both feet on the ground, turned and the hill of his boot got caught on one of the cribbing blocks on the ground causing him to fall and land on his right hand, causing a contusion.

THE EE WAS NEAR THE SHOP USING A PRYBAR TO MOVE A PIECE OF SHEET METAL. HE LOST HIS BALANCE & WHILE TRYING TO CATCH HIS FALL, HE FRACTURED HIS WRIST.
