Injured employee changed from EE's grinding shield to EE's welding hood, grit and debris was in the welding hood, and as the employee placed the hood on EE's head the debirs fell into EE's left eye.

Floating feed cone came loose, feed hopper & adjustment cap to drop 3-4" onto ee right foot. EE attached overhead crane to stuck floating feed cone. He applied pressure to the assembly he did not notice he had lifted the entire assembly off floor level by 3-4". He started the 3rd cut and at the time is when the floating feed cone came loose allowing the rest of the assembly to drop

Injured employee was removing arm guard from H6800 spider, using a sledge hammer to break arm guard loose. Employee went to the doctor on 8/29/2014 and he recommended surgery to the left shoulder

Injury initially occurred on 1/10/08 when employee injured right arm while removing cheek plate bolt from jaw crusher. Did not become reportable until 5/28/08 when the employee had surgery on torn bicep muscle in right arm.

Injured employee was emptying a chip hopper and a large amount of chips spilled on the ground. The employee was picking up the chips with his hands and throwing them into the chip hopper. While lifting a pile he did not realize he was standing on the other string of chips, he cut through the gloves and cuts his middle left finger. Resulting in stitches

EE WAS REPLACING BEARINGS AND SHIM PLATES IN A 3" CRUSHER. AS EE REACHED FOR A PLATE, THE STACK SHIFTED AND WHILE TRYING TO CATCH IT, EE HAND WAS CAUGHT BETWEEN PLATE AND CRUSHER FRAME. EE RECEIVED A LACERATION TO LEFT HAND REQUIRING 10 STITCHES

THE EMPLOYEE WAS USING A PRY BAR TO LOOSEN A U-BOLT IN A 7 FT CRUSHER BOWL. THE PRY BAR SLIPPED AND CAUGHT HIS FINGER (LEFT) BETWEEN THE PRY BAR AND BOWL, RESULTING IN 5 STITCHES.
