While hauling stripping equipment, the haul truck driver contacted a dip in the road which caused the seat to bottom out. The driver experienced back pain, and was diagnosed with back spasms.

Injured employee was putting air in the tire of the drill truck and another employee was letting the drill down so they could tell if it had enough air in it. The injured employee had their right foot under the tire and did not know the other employee was going to let it down. The foot got smashed.

Employee was changing screens and a bar dropped in 1/2 bin employee tried to retrieve. Employee fell which resulted in a broken leg.

Employee was washing out the surge tunnel, their eye started bothering them, might be debris in eye. Employee worked till 6.30pm did not report incident till 9.00pm. Next morning their eye was red irritated and swollen half way shut. Employee went to dr couldn't find anything was given a pres. for drops and returned to work.

Employee claims he was cutting a large hopper and while cutting the beam, he cut completely through. He stated he did not know he had cut through and had turned to walk around and cut the other side of the beam. He states when he turned around, the beam fell and hit the right side of his shoulder.

Emplyee was helping move a 55 gal. drum from the service truck, when the drum rolled on emplyee's foot. Employee was taken to emergency room and X-ray revealed a hairline fracture.

HIRED TRUCKER WAS IN PROCESS OF DUMPING EXCESS OF OVERLOAD OF MATERIAL. WENT TO REAR OF TRUCK AND WAS CLEANING TAILGATE AREA TO FASTEN THE TAIL GATE. LOST FOOTING ON DUMPED MATERIAL GRABBED END OF TAILGATE AS THE GATE WAS CLOSING CATCHINGFINGERS ON LEFT HAND.

MECHANIC FAILED TO DISABLEFAN WHILE GUARDS WERE OFF. WHEN PROCEEDING TO DO MAINTENANCE WORK, FANCAUGHT LEFT THUMB RESULT IN CUT THAT REQUIRED ONE STITCHE AND PRESCRIPTION FOR PAIN.
