Injured employee was holding a hammer wrench on a jamb nut. The other employee struck the hammer wrench with a sledge hammer, the jamb nut broke free causing the hammer wrench to move suddenly. This caused his right index finger to be caught between the hammer wrench and the drill boom. Resulting in a laceration and broken right index finger.

EE WAS OPERATING HAUL TRUCK. WAS HAULING PRODUCTION MATERIAL FROM THE PIT DOWN TO THE PRIMARY. WHILE DRIVING DOWN THE ROAD, THE EE GOT THE TIRES IN MUD AND ROLLED THE TRUCK ON ITS SIDE.
