He was shoveling lime off plate, the wind blew lime dust in his eyes.

Employee was unplugging the kiln cooler discharge. The feeder would plug and he would take the cap off the side of the cooler and poke the material with a metal bar to get it flowing again. Hot dust was blowing out the openings while he was attempting to unplug it. He suffered lime burns to his wrist, which became infected and reportable on 10/22/11

Employee reported that he had been refueling the fuel truck and dropped the lid to the tank, when diesel fuel splashed on his face and under his safety glasses. He removed his safety glasses and rubbed his face with the back of his glove to remove the fuel from his face and his glove contacted his eye.

Injury was greasing bottom damper on west side of kiln with a manual grease gun. The grease fitting didn't allow grease to flow, He pushed harder, resulting in Tony straining his left shoulder.

Employee was rodding out the O2 analyzer port at the kiln discharge. The port must be opened and the opening cleaned of any debris. Injured was working from the ladder and was removing the cap that covers the port. Hot material blew hot material out onto the left side of his face. Material went under his safety glasses and got in both eyes.

Employee was turning the cap that holds the oil in the trunion bearing. The cap was stuck to the shut off valve and would not release. He continued to try and twist the cap off. When it gave way he reported feeling a strong burning pain in his groin.

Employee was lowering a spout to load a truck with Lime Kiln Dust. The spout needed to be adjusted to go into the truck hatch properly. The employee jerked the spout with the cable and felt a sharp pain in his right groin area. Doctor determined it to be a hernia.

Employee was swinging a 25 pound sledge hammer to clear a dry ball from the seasoner when he felt pain in the center of his back and his right shoulder. Employee did not loose time until February 5, 2007 when the doctor diagnosed a torn rotator cuff.

EE was moving high lift through the plant when the equipment went in a hole. This caused the EE to be thrown up in the air approximately 3 feet. When he came down he landed on his ankle, spraining it.

EE was walking up steps. When he got to the landing he turned to see if the belts were running and felt a popping and burning in his left knee. EE will have surgery for torn cartilage.

EE was trying to access the front of the scaler when he slipped on some spilled oil on the hydraulic tank and fell approximately 4 feet to the ground, landing on his left side. EE did not lose any time until 10/7/05 when he had a partial lateral menisectomy performed on his left knee. --EE complained that knee still was not feeling good and started losing time again.5/19/06

Employee was hunched down near the ground trying to get a return idler to move by hitting it with a hammer. When it started moving he fell forward and put his hands in a pile of hot lime. He was wearing gloves and long sleeves but the lime got in between the two and caused second degree burns on his right wrist.

EE was raking gators off the grate at the end of the kiln when he felt pain in his left arm, lower back and left leg. EE went to the doctor and was released to full duty till the root cause of the pain was determined. EE will be having orthoscopic surgery on April 26, 2005 on his left shoulder. This will be the first time he will have missed any work due to this injury.

Employee was replacing 2 of 5 pieces of 3"conduit that were in an 18" ditch.The ditch was extremely wet.The conduit had 3-4" gaps between each piece. While stepping on the conduit to climb out of the ditch the employee's foot slipped between conduit twisting right knee.Employee tore his miniscus and will be having surgery on 11/18/04 which will be his first day off.

Employee was tapping on a bearing race with a hammer in order to remove it from the bearing. A sliver of metal from the bearing race broke off and embedded in employee's left thigh. Employee had to go to doctor to get sliver removed.

The tailgate on the back of the Mack dump truck would not latch properly. In order to get tailgate to latch employee pulled the tailgate up with one hand and lifted the latch with the other. The tailgate slipped out of his hand and smashed the middle finger on his left hand. Employee required 6 stitches.

EE WENT TO WALK OVER TH E RAIL CAR PULLER CABLE WHEN HE SLIPPED IN SOME MUD. AS HE WAS FALLING THE CABLE TIGHTENED, COMING UP OFF THE GROUND & STRUCK EE IN THE FACE. THE EE NEEDED 4 STITCHES IN HIS LIP.

HE STEPPED ON THE BEAM WITH HIS LEFT FOOT. UPON RAISING HIS RT. FOOT OFF THE GROUND HE SLIPPED AND WENT HEAD FIRST IN THE OPENING CREATED BY THE MISSING JAW CRUSHER. EE FELL 13' TO THE CONCRETE BELOW.

EE WAS CLEANING OUT FROM UNDER THE CONE CRUSHER DISCHARGE BELT. A ROCK COME OFF THE BELT OR PLATFORM ON TOP OF THE CONE CRUSHER. THE ROCK KNOCKED HIS HAT OFF AND CUT HIM. HE HAD A 1 1/2" LACER ATION THAT WAS TREATED BY THE DR AT ST. MARY'S E.R. IT TOOK FOUR STAPLES.

THE INDIVIDUAL WAS FIXING TO OPEN A LARGE DOOR TO THE MOBILE EQUIPMENT SHOP AND NOTICED A HYDRAULIC JACK IN THE WAY. HE MOVED THE JACK AND TURNED TO OPEN THE DOOR AND FELT PAIN IN HIS ANKLE. H R THOUGHT HE HAD SPRAINED IT. HE WENT TO THE DOCTOR AND WAS TOLD AS A PRECAUTION HE SHOULD SEE AN ORTHOPEDIC DOCTOR. THERE WAS A SLIGHT FRACTURE.

INDIVIDUAL WAS STANDING ON A CONVEYER BELT SHOVELING THE STONE OFF. HE LOST HIS BALNCE OR SLIPPED AND FELL APPROX 10 FT, HITTING HIS HEAD ON A HAND RAIL BEFORE LANDING ON THE GROUND. HE SUSTAI NED A LACERATION ON HIS RIGHT FOREHEAD OF APPROXONE INCH AND A DISLOCATED RIGHT SHOULDER. THERE WERE MINOR ABRASIONS TO BOTH SHINS.
