Employee was using a empty 5 gallon bucket to dip water and pour into a pump for priming. He had filled the bucket approximately 1/2 full and when he lifted the bucket up to pour it he felt a twinge in his lower back. Employee was seen by his doctor and X-rays were taken that were negative. Employee was given prescription medication with no work restrictions applied.

Employee working on feeder at the tunnel belt was sitting up after crawling out from under pan and struck his head on a flow paddle switch causing a minor cut to his forehead. Employee received a tetanus shot and dissolvable stitches and returned to work with no restrictions.

Employee was operating the Cat Skid Steer loader when he swerved off the roadway and hit a small drainage ditch to go by an oncoming Cat 980G loader, that resulted in a lower back injury. Employee received cortisone shots, steroid packs, pain pills and was put on restricted duty pending his follow up visit by the physician.

WHILE EE WAS STEMMING HOLES, FELT A SHARP PAIN IN SIDE. CONTINUED TO WORK. NEXT MORNING IT WORSENED. IT WAS BELIEVED THAT THIS WAS A "FLARE-UP" OF A PREVIOUS INJURY THAT OCCURED IN A MOTOR VEHICLE ACCIDENT WHILE WORKING AT ANOTHER SITE. UPON INVESTIGATION OF OUR WC CARRIER IT WAS DETERMINED ON FEB 1 TO BE A NEW INJURY.
