Production Assistant working in the Kiln and Preheater area became overheated resulting in Heat Exhaustion.

When employee was getting up from Packer chair EE felt a pain in Left Knee

While dismounting forklift,Maintenance Mechanic felt pain in Right Calf Area

No curbs on the exit lane to align tractor-trailers to the overhead safety cage. Driver pulled under the safety cage at a slight angle and the safety cage would not lower all the way down onto the tanker because of the adjustment chains. Driver stepped onto the top of the tanker and lost footing and fell between the safety cage and the tanker landing on the ground below.

Maintenance Mechanic was pulling on gearbox during removal and shortly after noticed a bulge in their stomach area.

Employee states EE tripped over fire extinguisher & fell onto a pallet that was on the ground, fell onto EE's side.

Maintenance Mechanic was attempting to plug in a grinder, and tripped over support leg of adjacent press causing them to fall at same level and injuring their back.

Quarry employee was opening valve on Water truck when valve moved suddenly and strained L Thumb.

Production Utility was attempting to clear a Mill Feed chute when foreign debris entered Right eye resulting in corneal abrasion

Employee was descending a step ladder at the Secondary Crusher area and lost footing and fell approximately 2' and landed on Left side injuring Left wrist.

Employee was struck on Right Hand by piece of falling metal debris.

Employee was walking down stairway to Main Office and was stung by a wasp on the Left Index Finger, causing swelling on the back of the hand.

Employee completed closing air valve and turned to walk away and twisted EE's Left Ankle.

Maintenance Mechanic removed PPE and got debris in Right Eye resulting in corneal abrasion.

Maintenance Mechanic was attempting to loosen set screws on bearing with a ratchet wrench resulting in a Left Elbow sprain.

Attempting to separate an Elevator Bucket from Elevator chain during maintenance, resulting in muscle strain

Employee was climbing a ladder, when they felt a pop in right knee.

Maneuvering a screw on a dust pump into place fell and pinned right hand again a piece of metal crushed bone

Fork Lift operator lifted an inner drum using a cable for securement. Employee tried to shift the drum on the forks from the side when it fell off and landed on right foot.

Employee felt ill - like heart attack, called 911 for transport to ER - initial from EMT is most likely a panic/anxiety attack.

Miner entered the Kiln to perform an inspection. Made their way to the kiln inlet to take a look at the sugar scoops and was taking pictures of the brick that did not get removed. In the process of taking pictures, lifted up their head and a small clump of kiln dust fell from the bricks and landed between hard hat and safety glasses eventually moving into right eye.

Employee was holding a portable 2-way radio microphone in right hand. Another employee pulled it away and the metal bracket on the back of the microphone cut the right middle finger with a deep gash. EE was sent to ER and received 3 stitches to seal the wound.

Employee was walking from office to the parking area - one part of concrete slab is raised appx. 1 inch across . Employee tripped on the raised edge and fell to the ground causing a left small finger middle phalanx fracture

Walking on the concrete to get a loader, slick from mud and rain, employee slipped but did not fall. Strained lumbar region.

Was removing a plate from the west drag when it slipped from a pry bar that was holding it up and pinched the employees left index finger against a washer of the long bolt EE was taking out, laceration on the front side first and second sections - 4 sutures and splint.

Employee was using a grab bar to pull self up into the cab of a grader when EE felt a tug on the shoulder - after consultation EE may have a bone sour/bursa sac inflammation or an impingement - MRI may be needed to decide.

Forehead laceration

Worked Full Duty till LDW 8/5/19 - Loading a shot on uneven ground, as EE turned from one hole to the next EE lost footing on a rock, hurt right knee.

Employee plus other maintenance and electrical employees were changing out the slide on the #33 silo. The slide slipped off and landed on the employees left foot just above the steel toe protection. We immediately took the employee to a local ER for X-Rays and confirmed that foot was broke. EE was place on light duty and referred to TOC

Employee slipped and strained knee.

Production Attendant using a sledge hammer to knock in finish mill doors when the hammer bounced and struck the employee in the face. Resulting in a laceration to the eye lid requiring sutures.

Tripped over cord while performing housekeeping tasks.

Using hammer to move railcar offloader door and struck finger causing fracture and avulsion.

Employee was pushing a wheelbarrow across the floor when employee lost employee's balance and fell resulting in a dislocated right knee.

Employee was assisting in maintenance of a pump and motor and strained back while attempting to shift it.

Employee was moving a 55- gal drum into place when it shifted and caught striking EE's thumb.

Employee was using a water hose to wash debris from crusher. Injury was reported on 3/23/2017. First aid treatment was rendered. Employee requested and received follow-up treatment on 5/2/2017 due to continued pain and was diagnosed with lower lumbar strain and prescribed physical therapy. Incident became reportable on 5/2/2017 due to being prescribed PT.

Employee was assisting in manually moving a plate of steel and dropped it resulting in right, middle finger being caught under. The injury resulted in a laceration and fracture to the finger.

Employee was assisting other employees in placing a shaft into a mechanical separator. The shaft shifted and caught the employee's right thumb between the shaft and separator wall. The incident resulted in a laceration to the right thumb.

Chain fall was hooked to liner that had pad eye welded to it. The pad eye broke loose and liner fell and hit EE in the right thigh. EE received a laceration on right thigh.

Employee was walking across the maintenance shop when EE was struck by a particle ejected by a nearby task (portable grinder). Foreign object in left eye. Object was not imbedded and was flushed out. Required Rx medication for eye.

Employee swinging hammer and struck left index finger resulting in lacerations requiring sutures.

Tripped over drain line, sprained ankle. Injury occurred on 4/28 and was classified as first aid. Pain returned and was referred to physical therapy on 10/31/16 resulting in reclassifying to reportable on 10/31/16.

Injury was reported to Mine Operator that this was a work related injury on 10/12/16. Employee reports: While using a pneumatic, pistol grip, type of chisel EE injured EE's right thumb. EE requested medical attention on 10/13/16 and was seen by medical professional on 10/14/16 and diagnosed with a strain/sprain to the right thumb.

Climbing down from front end loader

While repairing a screw conveyor the employee's hand got caught under the screw. He was reaching for a loose bolt. The screw was being turned manually by a coworker. Employee received four stitches in his right thumb.

Employee was in the process of opening a F/M separator door. When he was pulling down on the wrench he felt a pull in his right shoulder.

Employee was replacing some floor grating in the water tower building. While doing this task he had to pull some of the grating up by hand. He felt a pull in his left shoulder while pulling up the grating

Employee was doing a PM of the FM separator. He noticed some build up of material that was not allowing the dampers to close properly. He then went to knock the build up off with a hammer and while doing so some material fell into his left eye. We took him to the clinic and the doctor reported a scratch on his cornea. He was given an antibiotic cream. No restrictions were given.

While in the process of cleaning the swirl chamber the miner lifted a jack hammer into position and felt a pain in his right shoulder. The miner did not see a doctor until 7/21/2014 at which time he was placed on restricted duty.

The miner was in the process of rebuilding a 300mm pump assembly. While holding a wrench in place the wrench slipped and his right pinky finger was pinched between the wrench and the flapper arm on the pump. There laceration required four stitches.

The Miner was in the process of staging tools for a job. As he removed a 3 ton com-a-long from the back of a golf cart he felt some pain in his back. He told his supervisor at the time of the accident. However it did not require medical attention until 7/14/2014

The Miner was eating lunch in the break room when he felt something like a bite or sting on his left leg. The next night the area became sore. He went to the clinic on 2/27/2014 at which time it was determined that he had a staff infection. Which has been treated with antibiotics.

The miner was using a jackhammer to clean up in the plant. He was working in a kneeling position. When he went to stand up he felt pain in his right knee. He went to the clinic the following day and returned to work with no restrictions. However he has not improved and we sent him back to the clinic on 03/12/2014 and he is now on restricted duty.

While working on a floating pump assembly the miner was struck in the back with the boom of a excavator. He has five broken ribs and a partially collapsed lung.

Employee hit his elbow on a steel flange as he descended a step ladder.

Employee was in the process of fabricating a steel elbow. While lifting a piece of steel he felt a pull in his abdomen.

While pouring chemicals for a process mixture. Employee felt some pain in his back. The containers he was pouring from are 5 gallon containers. He began to have more pain once he was at home. This became worse around 4:30 PM.

While in the process of removing a wedged bag from the packer line EE picked up the bag and felt a pop in his left upper arm. He has torn his bicep.

As ee exited the loader when he put his weight onto the ground with his right leg his knee gave out. This injury did not become reportable until 06/21/2013.

The miner was trying to position a replacement gate into place. The gate hung on a beam and the miner was pushing on the gate to free it up when he felt a pain in his shoulder. The miner did not seek medical attention until 04/29/2013

Dust Particles in the air

Employee was walking on catwalk and tripped on a piece of plywood causing his right shoulder to come in contact with a concrete block wall.

EE was removing bags from a dust collector when he felt a sharp pain in his left shoulder. He was put on restrictions on 11/27/2012.

Employee was installing a spout into a bind, then the spout slid and caught employee's tip of finger.

EE was operating a Front End loader when the cover came off of one of the air conditioning vents. When he went to put it back on he noticed a rag in the hole. He put his hand in to pull the rag out and contacted the fan blades lacerating the pad of his middle finger requiring stitches.

EE was operating a stand behind Skag Mower when he went to push a branch out of his way. At this point he steered the mower sharper to the left than he meant to. He thought the section he was standing on was going to move in such a way that would twist his ankle so he got off to stop mower. When he his foot contacted the ground he tore ligaments in his ankle.

EE was moving a chute on a dolly. They had to take the chute off the dolly to maneuver it in a tight spot. While moving the chute it fell over and caught his hand between the chute and a handrail. Restrictions were given.

EE was replacing the bearing on the crusher dryer elevator. When he lifted the bearing up to put in the lower bearing housing he felt a strain in his back. He went to the doctor on 3/21/12 and was assigned restrictions for a strained back.

Injured employee was assisting another EE to switch a valve on top of the overtrack silos. He put his hand on the end of the bar and helped push up. When the bar moved up he got the little finger on his right hand caught between the bar and the overhead old FK line causing a laceration requiring 4 stitches.

AMEC's subcontractor' s (BWSC) EE fell down and broke his ankle. The fall caused the injury to his ankle. He was taken to hospital where the ankle was determined broken.

EE was replacing one link on the Raw Mill Elevator chain. EE was having difficulty removing the pin from the chain and decided to use a porta power ram to press the pin out of the link. EE leaned over the porta power to see if pin was moving when the porta power kicked out striking EE in the face. EE received 13 stitches and a broken nose.

EE had been cleaning the preheat tower. EE took a break to get a drink of water. He lifted up his face shield and tilted his head back to drink out of a gallon jug. When he tilted his face back, dust that had accumulated where the face shield connects to the hard hat fell behind his safety glasses into his right eye. Prescription eye drops were given.

The miner was working in the preheat tower cleaning and water blasting. He reported that his shoulder and arm were aching. The employee was moved to operating a loader for the remainder of his shift and the next night. After that he continued with no problems. He has since complained about his shoulder hurting again and he is now on restricted duty.

EE was staging kiln brick for rebricking contractors when his pinky finger got caught between two of the bricks. The kiln bricks weigh approximately 20 pounds a piece. The tip of his pinky finger was broken and stitches were required. EE was wearing gloves.

EE had been utilizing a bench grinder and 4" grinder on guards during the day. That night his eye started bothering him. After going to the doctor 2 days later it was determined that he had metal in his eye. Eye drops were prescribed.

EE notified the company that he had carpal tunnel which was going to require surgery on June 9, 2011. EE had the carpal tunnel surgery on July 22 and returned to work with restrictions on July 25, 2011. There is no specific incident which caused the carpal tunnel.

EE was opening the outer door to the personnel elevator when his pinky finger was caught between the edge of the door and the handrail resulting in a laceration requiring 4 stitches.

EE had been installing 50 pound buckets on the additives elevator. The following morning his back/hip area were hurting. EE went to doctor who said it was a strain. Restrictions were issued.

EE was closing the door on the crusher dryer. EE placed his hands on the moving part of the door. As the door closed, EE's pinky finger became caught in a pinch point, breaking the finger.

EE had been waterblasting the preheat tower. When he opened one of the doors to water blast that area some dust got on his sleeve. When he lowered his arm the dust managed to go from his sleeve down into his welding glove and burned the back of his left hand. He received second degree burns and a prescription was used in the treatment.

EE was climbing down a ladder on a rail car to get the brake. He missed the bottom step with his right foot and landed hard on the ground. EE tore the meniscus in his left knee. EE was not given restricted duty until 5/19/2010.

EE was about to walk down a flight of stairs in the preheater tower when his face shield (which was flipped up in a position parallel to the floor) caught on something. He slipped down several stairs, caught himself by getting his arm around the handrail. He strained his bicep muscle on right arm and had restrictions for several days.

EE was closing hatch in top of personel elevator. EE was lookng up to ensure hatch was closing properly. When hatch closed, dust came down and got into the eye, behind EE's safety glasses. This cement dust caused a small burn to the eye, requiring prescription medicine for the eye.

EE was reaching down to pick up a jack hammer, he lost his balance and his hand was smashed between the jackhammer and a door frame. EE broke a bone in his right hand below the knuckle on his pointer finger.

The employee was opening a lid on a tanker truck to begin filling it with CKD and the lid blew open due to pressure build up blowing the dust into his eyes, burning them.

Employee was washing a beaker and did not know the lid had a break in it and the employee cut his right hand between the pinky and ring fingers.

Employee was disassembling a sand blaster unit hose. The employee did not completly bleed off all air pressure and upon removal of the hose, the coupling came back, striking him in the chin. The employee received stitches to his chin.

Employees were putting sheets of tin on a building. Employee was handing a sheet of tin to another employee, who was unable to grip the tin and it slipped out of his hand striking injured employee in the left arm.

The employee was going to look into a preheater door, after inspecting the door he stepped in some hot material.

The employee was going to look into a preheater door and after inspecting the door, he stepped in some hot material that remained on the steps.

The employee was loosening and removing bolts on a vessel door using a 12 inch pipe wrench and felt a kink in his back.

Employee was repairing an FK pump, cleaning air ports with a hammer and chisel, when the chisel struck the left ring finger, causing a laceration.

Employee was utilizing a jack hammer to break up a cement pile, when a piece broke free and landed on the top of his right foot, just behind the steel toe in his boot.

On March 20, 2009 the employee underwent surgery for a cumulative injury. EE is a production utility operator and cannot pinpoint an exact date or activity that may have caused his carpal tunnel syndrome.

The employee was cleaning out the discharge of the grind-out silo airator. The opening to this area is approximately 5 feet high. When the employee stood up after exiting he stated that he felt a "pop" in his lower back.

While checking a cut phone cable dust was dislodged and fell into the employee's left eye. His eye was flushed immediatley. He was taken to the clinic where he was given prescription eye drops.

Employee was carrying a bucket of ANFO to the holes. After loading a hole he was on the way back down on a slope and lost his footing. During the fall he caught himself and strained his left middle finger. Went back to DR 9/26 and became a medical treatment injury.

The employee was using a knife to cut insulation from lugs on a motor. The blade slipped and the employee cut his small finger on his left hand.

An extension ladder was being used to access the work area. EE was preparing to mark the holes for installation of the rubber seal. He stated that as he climbed the ladder, he stopped momentarily on the first rung before proceeding. He then climbed to the third rung. The ladder then slipped out from underneath him, causing him to fall and strike his jaw on the Crusher frame.

The employee was on top of cement storage silos. Wind was gusty (up to 24 mph). Dust was picked up and got into his eye. Employee was wearing PPE which included safety glasses.

Employee was trying to open a valve on the cement silos. The valve was stuck and the employee was pulling on it.

Determined to be work-related on 12/10/07. The employee underwent surgery for carpal tunnel syndrome on 11/19/2007.

The employee was using a water blaster to dislodge material from the Preheater. He later complained of pain in his right arm and shoulder.

It was a very hot day. Employee working in an already hot environment on the 8th Floor.

INCIDENT DID NOT BECOME REPORTABLE UNTIL 11/6/07 when the employee received medical treatment. The employee was collecting a sample. As he was walking adjacent to the belt he slipped on loose clinker.

The employee stumbled and twisted his right ankle while stepping off of a platform his was working on in the Packhouse.

EE was putting bearings on the North Standby Elevator counterweight. Skin got irritated by dust falling from above while working.

Employee was putting gearbox on Jefferies Feed Belt and felt something in his eye. He was wearing his safety glasses.

On March 5, 2007 the employee underwent surgery for a cumulative injury/illness. Employee could not recall a specific njury or event. Medical providers determined this was work related and cumulative in nature.

The employee missed a step as he was descending a set of steps. He fell 2-3 steps and struck his knee on the landing. The landing consists of metal grating.

Gunite hose clogged up. It was disconnected from the gunite machine. Crew was trying to unclog hose. Some way there was trapped pressure & when the clog broke loose, it blew out striking our supervisor who was only observing.

The employee and witness were carrying a heater. The witness was carrying the front portion of the heater and the injured employee followed behind with the other portion. As the injured employee was walking down the steps from the Preheater elevator to the Kiln floor, he slipped. While falling the employee hooked his right arm around the handrail.

Employee was driving a wedge into a chute a with hammer, and the employee who got hurt reached for the wedge and was struck by the hammer, resulting in laceration right thumb (6 stitches).

Employee was meeting Quality Control Manager to collect raw material substitute samples. As she was climbing the stairway leading to the plant, employee tripped upward onto the stairs. She was carrying a glass sample bottle in her right hand. When she fell forward, the glass broke and her hand went into the glass as she tried to catch herself.

David was clearing a bag jam at the flapper. This is at the transition of the packer discharge conveyor and the incline spring conveyor. He pulled the flapper up by grasping the lower edge and cut his left middle finger.

The employee was putting dust collector bags in a dust collector. While inserting the bag over the ventura, dust fell from the top of the dust collector into his eyes.

The employee mashed the end of his little finger of his left hand while punching out an obstruction on the #2 mill feed belt below #7 weighfeeder. The finger was caught between the punch bar and the chute housing.

Employee was using a step ladder to access the #5 Synthetic Gypsum Screw. The portion of the screw where the work was being performed is below a catwalk. The catwalk limits the worker from standing up fully. The employee was lifting and pulling on a bearing hanger.

The employee was opening the door on the discharge chute of the 54? belt. The hinges broke and the door struck the top of his foot as it fell.

The need arose to switch from synthetic gypsum to additives. The gate failed to open so the injured employee attempted to open the gate manually. He used a piece of angle iron approximately 3 feet long (weighing approximately 4 pounds) to pry the gate open. The angle iron slipped from the gate suddenly. The employee immediately began to feel pain in his back and right shoulder.

The employee was working on a section of drag chain. He was hooking the chain hoist up when he bent down and his left knee popped.

The employee was assisting the witness who was using a hammer and chisel to work on a bearing when a small piece of metal broke off and lodged in the employee's forearm.

The employee was leaving work for the day. As he exited the plant he felt himself growing faint. He went down onto his knees then fell backward striking his head on the concrete. The treating physician determined that what caused the employee to have the brief fainting spell was not work related.

On May 18 the employee complained of pain around his right elbow and forearm. He had been shooting an 8 gauge shotgun in the Cooler the previous day to remove a chunck of material.

Employee and co-worker were working on the #7A & 7B Syngyp Screws. As they removed the cover for the screw, EE felt a pain in his lower back. EE was pulling and lifting the cover at the upper end with his right hand. Co-worker was lifting and pushing the cover from the lower end. The cover hung so EE tugged on it to loosen it from the screw trough.

The injured employee was assisting in removing material from a plugged chute. He and two other employees were using a punch bar and sledge hammer to dislodge material. Suddenly a large piece of material fell onto the bar. The employee's hand was caught between the bar and the frame of the chute.

The employee was closing the exterior door for the Overtracks Silo Elevator when his finger was caught between the handle (on the top door) and the bottom door. The door is a bi-parting door. When the top section of the door is pulled downward the bottom section automatically moves upward until the sections meet. Both sections are equipped with rubber astragal.

On Feb. 7, 2006 the employee turned in a first aid report that he had struck his elbow when the wrench he was using slipped. On April 3, 2006 the employee was moving a roll of skirting when his left elbow "popped".

The injured employee was removing a section of plastic tubing from the Main Baghouse Inlet Pressure Tap. As the section of the pipe broke loose, the material on the pipe was disturbed. The employee felt material enter his left eye. The employee was wearing presciption safety glasses. Due to weather conditions on this day, wind gusts were common.

Employee was jack hammering concrete, jack hammer slipped and hit right foot and bent the steel toe boot down.

The injured employee was working with the vacuum truck when he noticed a damaged section of hose. The employee broke off the damaged section while the truck was in operation and his left arm was pulled into the hose.

The witness was using the vacuum truck to conduct cleanup around the VIC Tower. He needed to cut a section of hose and did not have a knife. The injured employee walked by and the witness asked for his assistance. The injured employee proceeded to cut a portion of hose with a pocket knife. His hand slipped while cutting and he cut his left forearm.

The injured employee was assisting an employee that was welding. They were working in a fairly confined area. The injured employee received a flash burn to his left eye.

The employee was helping to remove a grease bearing. As the bearing was being removed from the housing, water struck the oil and hot oil splashed on employee.

Employee was walking outside of a building going to the restroom. He walked in an area that had caution tape up and walked through some hot material (hot kiln dust). He burned areas on both feet.

The employee was entering into the passenger elevator in the Mill Building. The elevator is equipped with a steel, self-closing door and vertical safety gate. After entering the elevator the injured reached up with his left hand to close the vertical safety gate and mistakenly placed his hand in the door jam as it closed.

The employee reported that during the previous week he had strained his back while packing cement bags. As he was packing, two bags began to pile up and he reached out and grabbed one of the bags.

Employee was on a portable platform ladder lifting a plate to put on a screw opening when the ladder fell (collapsed) forward causing the employee to fall backward and to the right.

Employee was working on the Process Water Pump. A fitting had broken & water had spilled onto the floor & combined with mud & other debris that had accumulated on the floor. Employee was exiting the Cooling Tower Building to retrieve tools when he slipped & fell on the walkway ramp striking his back.

The employee was weed-eating around a fence row at the entrance to the facility. The next day the employee had developed a skin rash that was continuing to spread.

Employee was inserting tire in the new tire chute of the Preheater. He turned to put a tire in the chute and his knee started hurting. Employee previously had ortohoscopic surgery on this knee, not related to work.

The employee was changing air conditioner filters in the Coal Substation. He bent over to pick up the filter and as he returned to a standing position he felt pain in his lower back.

The employee was attempting to move a weight on the track of the Radial Stacker Belt. As the employee was moving the weight he began to feel discomfort in his lower back.

The employee was operating the 830 Sweeper Truck. The rubber break-away piece on the main brush skid-runner became partially unattached. The employee attempted to pull it the rest of the way off. His right hand slipped and struck the sharp edge of the skid.

Employee was using a wrench and extender bar to reverse the screw. Injured and the witness had their body weight pressed against the extender bar when the wrench broke. The two employees fell, their body weight crushed the tip of injured's fourth digit of his right hand between the extender bar and the Cement Cooler.

The incident occurred when a large rock was dumped from the loader bucket into the bed of the truck the injured employee was operating. The rock was large enough in size to cause significant jarring of the bed of the truck. Immediately the employee began to feel pain in his neck.

EE was moving a 6" vacuum hose into position to clean up around the masonry packer when the hose became hung up on an obstruction. EE pulled on the hose to dislodge it and when it came loose, stepped back off of a step down to the palletizer motor control center, turning ankle. It is a 12" drop from the floor level to the motor control level.

The injury occurred at the intersection of the packer discharge conveyor and the incline conveyor. A flapper plate catches the bag as it comes off the discharge conveyor. It was at this point 2 bags became jammed. The employee cut one bag open to allow the material out. The second bag bacame dislodged and caused material from the first bag to blow into the employee's face.

The employee had just started all of the Quarry mobile equipment. He turned away from the D-9 dozer and began to walk back towards the garage. As he was walking he tripped on a rut approximately 3-4 inches in height. The rut was frozen due to the fact it was 22 degrees on this particular morning. The employee fell forward with his left arm tucked close to his left side.

This incident occurred on the bottom deck of the VIC screens. This area is considered a confined space and has very limited means of entry and exit. The employee was working with three other employees to replace the screens in this area. The process of moving the screens involved the pushing and pulling of them into place. The screens weigh approximately 100 pounds each.

While off from work due to a non-work related illness the employee complained to his doctor about numbing sensations in his right hand. The doctor ordered an EMG, which showed the symptoms were due to Carpal Tunnel Syndrome. The company doctor determined the injury was partially due to repetitive motion on the job.

The employee was packing T-S masonry on the masonry packer. He raised his shield to look around and check the pallets on the palletizer when a bag blew up, blowing material into his left eye. He immediately began flushing the eye with solution.

An employee was using a grinder with a wire brush to get silicone off the edge of airslide in the Maintenance Shop and the injured ee was helping him. As injured ee turned away, a foreign object was thrown from the grinder and struck him in the eye. He wears prescription safety glasses.

The employee was in the Maintenance Shop working on a gear box. He was using a hammer and a block of brass to remove the shaft from the housing. While striking the brass block the employee missed the block and struck the middle finger of his left hand. The finger was struck between the hammer and the sharp edge of the gear box.

The employee struck his head, directly above his eyebrow on a pipe.

The employee was working in the clinker elevator pit. In order to access the north elevator slide gates he had to pass under a concrete beam. The clearance from the ground to the beam is approximately 4 feet. The employee squatted and while passing under the beam felt pain in his lower back.

The employee had positioned the vacuum truck and had stretched out the hose in preparing to vacuum out the area. The hose became disconnected from the truck and fell to the ground. The employee bent down and pulled the 6 inch hose with both hands. He moved the hose 5 feet from ground level to the truck. The employee complained of a sharp pain in his right shoulder.

The employee was weed-eating when he was exposed to poison ivy.

On 6-5-04 the employee was squatting down to check a railcar when his left knee popped. No medical treatment was required. On 6-21-04 the employee squatted down to pick up trash when the same knee popped again. Ice was applied to the knee and he was sent to the company doctor.

The employee was rebagging a dust collector when dust blew into his eyes. He attempted to wash out the particles but was not successful. The employee was wearing safety glasses and a dust mask.

The employee was squatting under the back of the D-9 Dozer. He was unhooking a clevis and cable. While he was squatted the employee felt a pop in his right knee.

The EE was working to clean rock out from the belly pan of a dozer. This task required the EE to lie on a piece of cardboard and move around underneath the equipment. While working on the dozer EE began to feel pain in his right leg. The EE reported he had originally injured his leg on 3-7-04 when he struck his leg on the fender of the 990 loader.

Employee was working with another to remove the cover of the #6 Syngyp screw conveyor. The sections of the cover have handles on each end to assist in removal. Injured & co-worker had removed one section and were in the process of removing the second when injured began to feel pain in his left shoulder.

EE WAS WORKING WITH 2 OTHER EMPLOYEES TO REMOVE THE CRUSHER/DRYER ELEVATOR MOTOR FROM THE MOTOR HOUSING. A CHAIN FALL WAS BEING USED TO MOVE THE MOTOR APPROXIMATELY 2 FEET. THEY EACH TOOK TURNS PULLING ON THE CHAIN. DUE TO THE AREA WHERE THE MOTOR WAS LOCATED, FREE MOVEMENT WAS RESTRICTED SOMEWHAT.

THE EMPLOYEE WAS WORKING ON THE MAIN BAG HOUSE - WEST COLLECTING SCREW. THE SCREW WAS CHOCKED UP AND HE WAS DRIVING THE SLIDE GATES OUT TO DUMP THE SCREW. AS HE WAS STRIKING THE GATE HANDLE WI TH A HAMMER, HE MISSED AND HIT HIS RIGHT HAND ON THE HANDLE OF THE SLIDE.

The employee spent the morning cleaning around the tail pulley at the rock reclaim. After lunch he began shoveling at the Crusher/Dryer around the guards and air-lancing the rotors. At 2:00 P.M. he reported to Joe Roark that he had some soreness around his right elbow.

THE EE WAS WALKING TO HIS CAR FROM THE BATH HOUSE WHEN HE FELL ON A WALKWAY THAT HAD SNOW & ICE ON IT. HE HIT HIS RT. SHOULDER. HE COMPLAINED OF PAIN IN HIS SHOULDER & NECK. THE EE DID NOT SEE K MEDICAL ATTN. UNTIL 12/29/03 AT WHICH TIME HE WAS PLACED ON RESTRICTED DUTY.

THE EE IS UNSURE OF WHEN THE INJURY OCCURRED. THE ONLY THING HE CAN THINK OF IS WHEN HE LIFTED A FEW CEMENT BAGS (94 LBS) OFF OF THE GAP PLATE TO THE CATWALK.

THE WIND WAS BLOWING AT SPEEDS CLOSE TO 30 MPH THIS PARTICULAR DAY. CEMENT DUST BLEW UNDERNEATH EE'S SAFETY GLASSES & INTO IS EYES.

EE WAS LIFTING A MASONRY BAG (70 LBS) OFF THE INCLINE CONVEYOR THAT WAS TURNED SIDEWAYS WHEN HE FELT A POP IN HIS RIGHT SHOULDER.

EE HAD BEEN WORKING IN THE RAW MILL SEPARATOR. THE JOB INVOLVED MOVING SEVERAL LARGE PIECES THAT COMPOSE THE UPPER TABLE. EACH PIECE APPX. 250 LBS. PIECES HAD TO BE MOVED ON ELEVATOR THEN OFF ON 7TH FLOOR. CO-WORKER WAS ASSISTING. EE ALSO DID OTHER WORK. AS HE WAS CLIMBING OUT OF SEPARATOR, HE FELT PAIN IN SHOULDER. NEXT WEEK HE WORKED IN LAB. OCTOBER 20TH HE WAS PLACED ON RESTRICT

THE EE WAS CLEANING UP ON THE PREHEATER. HE WAS PUTTING MATERIAL INTO THE OFFICE DOOR. DUST WAS DISTRIBUTED EITHER BY THE PREHEATER PUFFING OR BY CLEANUP ACTIVITY. THE DUST UNDER THE EE'S FACE SHIELD AND SAFETY GLASSES INTO LEFT EYE.

THE EMPLOYEE WAS WELDING ABOVE THE COAL MILL AT THE COAL INLET DUCT. HE WAS BENT OVER IN AN AWKWARD POSITION. AS HE TRIED TO STAND UP STRAIGHT HE FELL ONTO HIS KNEES IN PAIN.

THE EE WAS CHANGING FILTERS THROUGHOUT THE PLANT. WHEN HE EXITED THE 8TH FLOOR SUBSTATION, HE TRIPPED ON AN AIR HOSE IN THE DOORWAY. AS A RESULT, HE SUFFERED A STRETCHED ACL INT HE LEFT KNEE.

THE EE WAS IN THE PROCESS OF INSPECTING THE SWIRL CHAMBER DISCHARGE. HE OPENED A 6 INCH PORT ON THE WEST SIDE OF THE SWIRL CHAMBER DISCHARGE. AS HE OPENED THE PORT COVER, AN AIR CANNON FIRED AND HOT KILN DUST BLEW INTO HIS FACE AND UNDER HIS SAFETY GLASSES.

ON ABOUT MAY 28, 2003, THE EMPLOYEE SAID HE WAS REMOVING A PACKED BAG THAT HAD FALLEN OFF A PALLET WHILE ON THE PALLETIZER PALLET LIFT. HE FELT A PULL IN HIS BACK WHEN REACHING OVER TO PICK UP THE BAG. HE REPORTED THE INCIDENT ON JUNE 12 BECAUSE HIS BACK STILL BOTHERED HIM. FALLEN BAGS WILL NO LONGER BE REMOVED BUT CUT OPEN & CONTENTS SPILLED FOR CLEANUP. DEC. 8 WAS FIRST LOST DAY.

EMPLOYEE WAS TORCHING A PIECE OF STEEL AND A PIECE OF HOT SLAG WENT INTO LEFT EAR.

THE EE WAS REPLACING THE GUARD ON THE TAIL PULLEY OF THE FINENESS CONVEYOR BELT. THE TAIL PULLEY IS PLACED CLOSELY TO A HANDRAIL AND SUPPORT BEAM. THE EE STRUCK HIS ELBOW ON EITHER THE HANDRAI L OR SUPPORT BEAM WHILE TRYING TO POSITION THE GUARD.

THE EE WAS WATER BLASTING WITH A 12 FT LANCE THE WEST SIDE ORIFICE TO THE PREHEATER. WHILE DOING SO HE EXPERIENCED SORENESS INT HE CHEST AREA.

EE WAS PACKING MASONRY BAGS. PROBLEMS AROSE WITYH THE PALLETIZER. THE EE TURNED OFF IMPELLOR DRIVE ON PACKER AND RAISED FACE SHIELD TO LOOK DOWN THE LINE. THE PACKING SPOUT RELEASED MATERIAL, THE MATERIAL WAS DEFLECTED OFF SEAT AND INTO EE'S LEFT EYE.

THE EE WERE PERFORMING REPAIRS ON THE CURSHER ROTARY. BOTH EE WERE WITH THEIR BACKS TO ONE ANOTHER. THE INJURED EE WAS WEARING ALL REQUIRED PERSONAL PROTECTIVE EQUIPMENT (SAFETY GLASSES, HOOD) BUT STILL RECEIVED FLASH BURNS TO THE EYES

THE EE WAS WALKING UP THE STEPS TO THE PACKER FLOOR. HE TRIPPED ON THE TOP STEP HITTING HIS LEFT KNEE ON THE FLOOR GRATING.

THE EMPLOYEE WAS CHANGING THE SCREEN ON THE VIC. AS HE HANDLED THE SCREEN OFF TO ANOTHER EMPLOYEE INSIDE THE UNIT, THE INJURED EMPLOYEE FELT HIS ARM SNAP AND POP. THERE WAS A LOT OF BUILDUP IN THE AREA MAKING IT DIFFICULT TO MAINTAIN A FIRMBALANCE. ALSO, A LADDER WAS PLACED AGAINST THE SCREEN HOUSING THAT SHOULD HAVE BEEN MOVED OUT OF THE WAY.

THE ACCIDENT TOOK PLACE ON THE 7TH FLOOR MILL ROOM. EE AND OTHER WERE ATTEMPTING TO CHANGE THE OIL IN THE ELEVATOR HEAD WHEEL-RAW MILL. OTHER LIFTED THE 6 GALLON BUCKE OF OIL APPROX. 40 LBX. H EE WHO WAS ON THE PLATFORM. HE PULLED UP THE BUCKET THROUGHT THE LADDER AND JERKED IT UP TO KEEP FROM DROPPING IT.

EMPLOYEE WAS TRYING TO UNPLUG WEIGHFEEDER. HE TRIED TO PUNCH IT OUT, HOWEVER, NOTHING CAME OUT. HE THEN TURNED THE VIBRATOR ON. APPROX FIVE MINUTES LATER WHILE CLEANING DEBRIS OFF GROUND OFF GROUND, THE PIN IN THE WEIGHFEEDER CAME LOOSE AND STRUCK EMPLOYEE ON TOP OF HAND.

EE WAS WEEDEATING AND GOT INTO POISON IVY.

EE WASCHANGING LINKS IN THE CRUSHER FEED ELEVATOR. HE WAS USING A TORCH. THE CHAIN WAS SWAYING BACK AND FORTH, AND EE WAS ALSO MOVING WITH THE TORCH. WHEN THE CHAIN STOPPED, THE EE FAILED TO S TOP AND INADVERTENTLY BURNED HIS WRIST.

EMPLOYEE WAS ATTEMPTING TO LIGHT THE TORCH THAT WAS IN THE PRIMARY KNOCKOUT CHAMBER FOR THE TERTIARY AIR DUCT. WHEN HE PLACED IT IN THE DUCT, THE FLAME FLASHED BACK ONTO EMPLOYEE'S ARM.

EE WAS GRINDING KILN SHELL WHEN GRINDER JUMPED BACK & LACERATED EE'S THIGH.

EMPLOYEE WAS CHECKING THE FLOW ON #1 BLENDING SILO DISCHARGE. HE STEPPED ACROSS DISCHARGE CHUTE TO CHECK THE AIRLINES. WHEN HE STEPPED BACK ACROSS, HE TRIPPED AND FELL DOWN TWISTING HIS ANKLE. HE IS UNCERTAIN AS TO WHAT CAUSED HIM TO TRIP OR WHAT HAPPENED.

EMPLOYEE WAS REPLACING A BOLT ON THE MILL DOOR. THE DOOR WAS PLACED ON SAWHORSES. AS HE AND CO-WORKERS WERE LOADING THE DOOR ONTO THE FORKLIFT, DOOR SLIPPED AND CAUGHT HIS FINGER BETWEEN METAL SAWHORSE AND DOOR.
