No curbs on the exit lane to align tractor-trailers to the overhead safety cage. Driver pulled under the safety cage at a slight angle and the safety cage would not lower all the way down onto the tanker because of the adjustment chains. Driver stepped onto the top of the tanker and lost footing and fell between the safety cage and the tanker landing on the ground below.

Miner entered the Kiln to perform an inspection. Made their way to the kiln inlet to take a look at the sugar scoops and was taking pictures of the brick that did not get removed. In the process of taking pictures, lifted up their head and a small clump of kiln dust fell from the bricks and landed between hard hat and safety glasses eventually moving into right eye.

Injury was reported to Mine Operator that this was a work related injury on 10/12/16. Employee reports: While using a pneumatic, pistol grip, type of chisel EE injured EE's right thumb. EE requested medical attention on 10/13/16 and was seen by medical professional on 10/14/16 and diagnosed with a strain/sprain to the right thumb.

Employee was doing a PM of the FM separator. He noticed some build up of material that was not allowing the dampers to close properly. He then went to knock the build up off with a hammer and while doing so some material fell into his left eye. We took him to the clinic and the doctor reported a scratch on his cornea. He was given an antibiotic cream. No restrictions were given.

While in the process of cleaning the swirl chamber the miner lifted a jack hammer into position and felt a pain in his right shoulder. The miner did not see a doctor until 7/21/2014 at which time he was placed on restricted duty.

The miner was in the process of rebuilding a 300mm pump assembly. While holding a wrench in place the wrench slipped and his right pinky finger was pinched between the wrench and the flapper arm on the pump. There laceration required four stitches.

The Miner was in the process of staging tools for a job. As he removed a 3 ton com-a-long from the back of a golf cart he felt some pain in his back. He told his supervisor at the time of the accident. However it did not require medical attention until 7/14/2014

The Miner was eating lunch in the break room when he felt something like a bite or sting on his left leg. The next night the area became sore. He went to the clinic on 2/27/2014 at which time it was determined that he had a staff infection. Which has been treated with antibiotics.

The miner was using a jackhammer to clean up in the plant. He was working in a kneeling position. When he went to stand up he felt pain in his right knee. He went to the clinic the following day and returned to work with no restrictions. However he has not improved and we sent him back to the clinic on 03/12/2014 and he is now on restricted duty.

EE was operating a Front End loader when the cover came off of one of the air conditioning vents. When he went to put it back on he noticed a rag in the hole. He put his hand in to pull the rag out and contacted the fan blades lacerating the pad of his middle finger requiring stitches.

When he his foot contacted the ground he tore ligaments in his ankle.

When the top section of the door is pulled downward the bottom section automatically moves upward until the sections meet. Both sections are equipped with rubber astragal.

THE EMPLOYEE WAS CHANGING THE SCREEN ON THE VIC. AS HE HANDLED THE SCREEN OFF TO ANOTHER EMPLOYEE INSIDE THE UNIT, THE INJURED EMPLOYEE FELT HIS ARM SNAP AND POP. THERE WAS A LOT OF BUILDUP IN THE AREA MAKING IT DIFFICULT TO MAINTAIN A FIRMBALANCE. ALSO, A LADDER WAS PLACED AGAINST THE SCREEN HOUSING THAT SHOULD HAVE BEEN MOVED OUT OF THE WAY.

THE ACCIDENT TOOK PLACE ON THE 7TH FLOOR MILL ROOM. EE AND OTHER WERE ATTEMPTING TO CHANGE THE OIL IN THE ELEVATOR HEAD WHEEL-RAW MILL. OTHER LIFTED THE 6 GALLON BUCKE OF OIL APPROX. 40 LBX. H EE WHO WAS ON THE PLATFORM. HE PULLED UP THE BUCKET THROUGHT THE LADDER AND JERKED IT UP TO KEEP FROM DROPPING IT.

EMPLOYEE WAS TRYING TO UNPLUG WEIGHFEEDER. HE TRIED TO PUNCH IT OUT, HOWEVER, NOTHING CAME OUT. HE THEN TURNED THE VIBRATOR ON. APPROX FIVE MINUTES LATER WHILE CLEANING DEBRIS OFF GROUND OFF GROUND, THE PIN IN THE WEIGHFEEDER CAME LOOSE AND STRUCK EMPLOYEE ON TOP OF HAND.

EE WASCHANGING LINKS IN THE CRUSHER FEED ELEVATOR. HE WAS USING A TORCH. THE CHAIN WAS SWAYING BACK AND FORTH, AND EE WAS ALSO MOVING WITH THE TORCH. WHEN THE CHAIN STOPPED, THE EE FAILED TO S TOP AND INADVERTENTLY BURNED HIS WRIST.
