Employee stepped wrong and felt a pain in left knee while walking from truck to pump MCC.
Maintenance employee was rebuilding a pump, using an impact gun and noticed pain in EE's right wrist/hand.
Employee reported left wrist/hand pain to company Safety & Health on Feb 21st, after experiencing pain/discomfort/loss in mobility over the past months. Employee was immediately given a medical appointment and was diagnosed with mild carpal tunnel and given a corticosteroid shot in the left wrist.
Operator (OP1) driving in pickup truck, when they struck a trackhoe. CPR and AED were administered and OP1 resumed breathing with a pulse. EMS arrived and life flighted them to the hospital. Doctors believe OP1 experienced a heart attack, prior to striking trackhoe.
Employee was setting up 24-ft extension ladder at the fuel shed, to check gauge on the diesel tank. Employee was working alone, taking ladder from bed of Maint truck to lean against tank, when they felt a pop in right shoulder and pain. Employee states that they have extreme pain when they lift arm over shoulder height. Appointment with Dr was 30 Aug 22.
Employee was on ladder of railcar, reaching for a portable electric hoist (~20-25 lbs), grabbed the hoist and rotated EE's left arm (with the hoist in hand) to hand off to another operator, when EE felt a pop and pain in EE's upper left shoulder. Employee went to the emergency room that day and was placed on light duty, to include no lifting, pushing or pulling.
Employee was investigating intruders near Pond. While walking from pond (no established walkway), walked through washed out area with uneven terrain & foliage. Employee slipped, fell to the knees and felt pain in L hamstring. Employee was offered to go to dr, but refused until 21 Sept. Dr appt on 23 Sept put EE on modified duty, ordered an MRI. No diagnosis available.
Employee doing maint work on caustic pump in Flot bldg. EE leaned over the pump, a pin hole leaked on the caustic line of pump, spraying caustic soda 50% membrane under employee's face shield. Caustic hit EE's L side lips and EE's R eye. At the hospital, found with no lasting injuries, returned to duties, with no modifications, but w/ prescription eye drops for irritation.
Employee assisting to feed belt onto conveyor at head pulley. Employee bent over guard at head pulley to pull belt when they felt sudden pain in lower back from reaching across. Employee refused Workers Comp claim/appt 4, 11, 12 Feb. and agreed to claim/appt Feb 17. Appt found lumbar strain; not allowed to lift over 25 lbs, bend, stoop, or twist until follow up.
Employee was joining drawheads on railcar with sledgehammer, when they struck the drawhead, the force twisted left wrist. Employee has seen several specialists throughout months, MRI, X-ray to determine source of pain/issue. 3/18/20: doctor finally placed a hard cast on left wrist and gave cortisone shot.
Maintenance employee's leg and foot caught under personal vehicle and HDPE pipe, that Maintenance crew were dragging.
Employee was standing on level ground that was frozen when he twisted his body to return to his truck he felt a sharp pain in his right knee.
Employee was operating a boom truck when the auto kick out on cable got in a bind. When he attempted to free the auto kick out it slipped and mashed his left ring finger.
Employee was cutting seals off of doors on top of railcars when the knife slipped and cut his left arm. He had 6 stitches.
Employee was going up a slight slope and pulled a ham string on right leg.
Employee was removing some dead trees from pond spillway and got stung by a wasp. He had an allergic reaction to the sting.
Apparently the employee (unconfirmed bumped) into a piece of expanded metal on one of his legs, causing a scrap.
BEARING SLIPPED OUT OF EMPLOYEE'S HAND AND HIT HIM ON THE LOWER RIGHT LEG CAUSING A CUT THAT TOOK 13 STITCHES.
EMPLOYEE WAS ATTEMPTING TO TIGHTEN THE V-BELTS ON A CONVEYOR WHEN THE WRENCH HE WAS USING SLIPPED OFF THE BOLT AND CAUGHT HIS LEFT LITTLE FINGER BETWEEN THE WRENCH AND THE SIDE OF THE CONVEYOR.
WHEN EMPOYEE WAS GETTING OUT OF MAINTENANCE TRUCK HE TURNED HIS LEFT ANKLE WHEN HE STEPPED ONTO THE GROUND.
employee was taking a sheave off of pump and it slipped out of his hand and he droped it on his left middle finger. it required 7 stitches in finger.
Employee was removing a filter on AC-501 and droped the metal filter on his left leg. Causing a cut that needed 3 stiches.
Employee was setting a electric motor on a plattet to be moved. The motor started to tip over and the employee tried to catck the motor and got his left small finger caught between hoist cable and motor.
EMPLOYEE PICKED UP A STEEL DOOR TO AN ELECTRICAL SWITCH BOX AND STRAINED THE BACK OF HIS RIGHT HAND.
EMPLOYEE WAS DRILLING HOLES IN THE FRAME OF A MAINTENANCE TRUCK WITH A MAGNETIC DRILL PRESS. THE DRILL SLIPPED CAUSING THE EMPLOYEE TO HIT HIS CHEEK, CAUSING A CUT.
EE WAS PUTTING A A/C IN SUPERVISOR OFFICE. WHEN HE LIFTED THE A/C HE TRIPPED OVER A CHAIR TWISTING HIS LEFT KNEE AND HIT HIS BACK ON THE EDGE OF A DESK.
EE WAS CLIMBING OFF BL-302 & STRUCK METAL CORNER OF H0-301 GATE SKIRTING.
EMPLOYEE WAS CHANGINGV-BELTS ON BC-508 WHEN HE GOT TOO HOT.
EMPLOYEE WAS DRILLING A HOLE OVER HEAD AND SOME METAL SHAVING GOT IN HIS EYE.
EMPLOYEE WAS USING A BAR TO OPEN THE DOORS ON BOTTOM OF RAILCARS. THE BAR SLIPPED OUT OF THE SLOT IN THE DOOR & CAUGHT HIS RIGHT THUMB BETWEEN THE RAIL & THE BAR.
EMPLOYEE WAS SHOVELING SAND FROM UNDER TAIL PULLEY WHEN HE STRAINED HIS LOWER BACK.
EE WAS LIFTING A PART OF WATER PUMP AND STRAINED HIS BACK.
EE WAS WALKING ACROSS A CONCRETE FLOOR & SLIPPED ON SOME ICE & HE FELL ON HIS LEFT WRIST.
EE BROKE HIS LEFT WRIST. AS A RESULT HE DEVELOPED WHAT THE DOCTOR CALLED TRIGGER FINGER ON HIS RIGHT THUMB, CAUSED BY USING HIS RIGHT HAND TOO MUCH WHILE THE LEFT WRIST WAS HEALING.
EE WAS WORKING OFF A 6" STEP LADDER HE SLIPPED OFF THE 2ND STEP LANDING ON HIS LEFT WRIST
THE FLAT VALUE ON SU-107 WAS STOCK IN CLOSED POSITION. WHEN EE ATTEMPTED TO FREE THE FLAT HE GOT HIS ARM BETWEEN FLAT ARM AND CATWALK.
EE WAS OPENING THE LIDS ON RAILCARS HE OPENED ONE LID & IT HUNG WHEN HE WAS OPENING IT & IT FELLDOWN HITTING EE ON THE BACK OF HIS RIGHT ANKLE
2 EMPLOYEES WERE CHANGING OUT AN ELECTRIC MOTOR ON A PUMP. THEY WERE PUTTING A 12.0 SHEAVE (WEIGHING APPROX 75 LBS) BACK ON THE MOTOR WHEN EMPLOYEE STRAINED HIS BACK.
EE WAS GETTING ON LOADER WHEN HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE LOADER. AS RESULT HE FELL TO THE GROUND. WHEN HE TRIED TO BREAK HIS FALL HE LANDED ON HIS WRIST.
