Injured employee was cutting a piece of conveyor belt with a razor knife and cut left wrist, requiring staples to treat the injury.

Injured employee was assisting another employee move a metal plate. In the process of moving the plate, the injured employee lost his grip and the plate fell and struck him on top of the left foot directly behind the safety toe protection of his boot.

While walking up to inspect maintenance activities was struck with a flying object.

Employee was cleaning around tail pulley under plant and a rock fell and struck him on his lower back. Employee was sent for medical treatment that consisted of prescription medication for bruising and was released to work to perform normal job duties.

Employee was welding a plate in the Jaw feeder when a rock fell approximately 2 feet from the side of the feeder and hit or rolled on his middle finger on his left hand. Employee injury resulted in 4 sutures to his finger.

EE WAS UNLOADING A ROLL OF WIRE FROM TRUCK WHEN HE LOST HIS FOOTING INT HE MUD AND FELL ON HIS TAILBONE.

ENERGIZED. ELECTRICAL DISCONNECT IT STARTED ARCING, EE REACHED TO DE-ENGAGE THE DISCONNECT AS IT EXPLODED BURNING EE ON FACE & LEFT ARM.

EMPLOYEE STRUCK RIGHT ELBOW AGAINST LADDER WHILE CLIMBING ONTO PIT LOADER.

WHILE CLIMBING OUT OF TRUCK CAB, DOOR CLOSED ON EMPLOYEE'S LEFT THUMB, CUTTING IT. INITIAL REPORTS INDICATED NO SUTURES. SUBSEQUENT REPORTS, BASED ON X-RAYS, INDICATE A FRACTURE TO LEFT THUM B.
