4,000psi burst pressure Supply hose to the Brake Valve. Heat shield was still on the turbo however where this hose came out it wasn't blocking any fluid as it sprayed towards the rear of the turbo. All other hoses intact around the area.
Employee's right hand was caught in between a come-along chain and a conveyor frame when removing a bend pulley. Cutting the right index finger.
Employee tripped over an uneven transition between gravel and the asphalt parking lot.
Employee claims a foreign material became embedded in their left eye while working.
Employee claims slipped walking down steps and injured back.
Injured employee was using a sledge hammer to tighten belts on a primary crusher when the employee felt a pinch in his right shoulder. Resulting in a strain to the right shoulder.
Injured employee was holding brace arm while other employees was putting pin through the arm and stick. Other employee started the pin, and injured employee throught the pin had started and eased off the brace. Injured employee was not expecting the extra weight of the pin added to the brace. Causing lacerations to the right index finger resulting in stitches.
Injured was doing daily maintenance work and caught his finger between the punch plate and handrail. Resulting in stitches.
Inj EE was removing tail pulley on the 7's belt. When EE started pulling on the bearing the pulley shifted & dropped & smashed his pinky finger between the pulley bearing.
Injured employee recived flash burn to right eye. Doing welding activites
