EE was operating a skid steer loader, assisting another EE with a blast. Injured EE dismounted skid steer loader, he stepped on an uneven surface/object, severely spraining his right ankle.

Employee performing maintenance work (welding) while standing on a base made of aggregate material. Lost balance, attempted to catch himself as he fell, pushing his left hand out against wall. In contacting wall, he fractured a bone in his wrist.

EE ALLEGES THAT HE WAS PRYING A ROCK OUT THAT WAS WEDGED BETWEEN THE TAIL PULLEY AND CONVEYOR FRAME. THE ROCK POPPED OUT SUDDENLY AND THE EE STATES THAT HE OVEREXTENDED HIS LEFT ELBOW WHEN THE BAR WENT DOWN QUICKLY. EE ALLEGES HE FELT A PAIN IN HIS LEFT ELBOW.

EE WAS LOADING A PRODUCTION SHOT. A 5 1/2" STICK OF BLASTING AGENT GOT HUNG IN THE HOLD APPROXIMATELY 2 FEET FROM THE TOP. HE LAID DOWN ON THE GROUND AND REACHED DOWN INTO THE HOLE TO RETRIEVE THE STICK. HE HURT A MUSCLE IN HIS SHOULDER/NECK AREA.
