Two employees were trying to open a hinged door on the mill for the purpose of flipping the hammer. During the procedure of opening the door a 5/16" chain being used became unlatched at the center of gravity break over. Neither of the two employees realized the chain had come unlatched

REMOVING PIPE ON HIGHWALL EE WAS WORKING TOO CLOSE TO THE MACHINE TO BE DIRECTING THE OPERATOR. THE EE WAS ON THE INSIDE OF THE BERM WITH THE MACHINE WHICH COULD BE A RESTRICTED WORK AREA.
