Employee called from home complaining of leg pain possibly from repetetive motion.
Unhooking binder and chain off a water truck, the binder pulled suddenly toward him causing him to strike himself in the jaw/mouth area while holding onto the binder.

Employer notified on 6/16/2005 While getting into the truck, EE bumped his head jamming his neck.
EMPLOYEE TOOK THE AIR TANK OUT OF HIS TRUCK DUE TO FROZEN WATER. WHILE PLACING THE TANK BACK INTO THE TRUCK, EE SLIPPED ON THE LADDER CAUSING HIS LEFT SHOULDER TO HIT THE TRUCK CAUSING THE INJ URY. THE TANK WEIGHED APP 25 LBS, AND THE HEIGHTTO CLIMB ON THE LADDER WAS APP 6 FT.
