EE WAS CLIMBING THE LADDER TAKING A WATER HOSE TO THE TOP OF THE TWINSCREW TO START TO WASH DOWN THE LADDER SHIFTED POSITIONS HOSE PULLED HIM BACK CAUSING A FALL.

WHILE DUMPING ON WASTE PILE, LEFT REAR WHEELS HIT BEAM. TRUCK SLIPPED OVER THE EDGE. TRUCK SLID AND ROLLED APPROX 50-60' AND ENDED IN UPRIGHT POSITION. DRIVER CRAWLED OUT AND WALKED AWAY FROM TRUCK. DRIVER WAS WEARING SEAT BELT.
