A scale clerk arrived at the office to find a goat on the property. When EE and other employees attempted to enter, it head-butted them. The plant manager attempted to lead the goat off the property. It turned & started back towards the office, EE grabbed one horn and the goat jerked its head, causing a puncture wound to the right hand resulting in an injury requiring 4 stitches.
The injured employee along with a co-worker were making repairs to the sand screw. Both employees were welding within close proximity to one another. Although the injured employee was wearing the correct PPE, he suffered welding flash burns to his eyes.
Dismounting loader, had one foot on the ground and one foot on the 980G Cat Loader when the operator strained his back.
THE OPERATOR OF A VOLVO A35C HAUL TRUCK WAS NOT WEARING HIS SEAT BELT. AS A RESULT OF THAT, IT IS MY OPINION THAT HE HIT HIS HEAD ON A METAL POST INSIDE OF CAB, CAUSING HIM TO LOSE CONTROL OF THE TRUCK.
Was bringing pumphead out of hole being dewatered. Pumphead swung and hit EE's small finger on right hand and broke it.
EE HAD JUST CUT THE BOLTS OFF HOLDING A METAL STRUCTURE TO THE OLD STARTER BUILDING. EE PULLED ON THE STRUCTURE TO FREE IT WHEN SLAG & RUST FLEW INTO HIS FACE. PIECES ROLLED DOWN HIS FACE & LO DGED INTO THE CORNER OF HIS EYE.
EMPLOYEE WAS MOVING 3/4" (154 LBS) WIRE SCREEN CLOTH FROM THE SHIPPING PALLET TO THE STORAGE RACKS INSIDE THE SCREEN HOUSE. EE AND CO-WORKER WERE CARRYING THE SECOND OR THIRD SCREEN WHEN HE FE LT A PAIN IN HIS BACK.
EE WAS ATTEMPTING TO MOVE A PIECE OF STEEL 1/2"X2D'X6" OFF OF A 1/4" PIECE OF STEEL PLATE. THE EE SLIPPED WHILE ATTEMPTING TO THROW THE PIECE A WAY FROM HIM.
EMPLOYEE WAS STEPPING FROM A HAUL TRUCK TO A CONCRETE WALL WHERE THE BIN GATE HANDLES ARE LOCATED. THE EMPLOYEE'S FOOT SLIPPED AND HE FELL TO THE GROUND.
EE WAS INSTALLING A NEW ELECTRICAL SYSTEM. AS HEWAS PULLING ELECTRICAL WIRES, HE FELT A POP IN HIS SHOULDER. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 8/31/01.
THE EE WAS ADJUSTING A CRUSHER IN THE MAIN PLANT. AS HE STEPPED DOWN FROM A PLATFORM MOUNTED @ THE TOP OF THE CRUSHED, HE FELT A PAIN IN HIS BACK.
EE WAS CUTTING A STEP FROM AN OLD PLATFORM. HE WAS HOLDING THE STEP WITH HIS LEFT HAND. WHEN THE STEP CAME FREE, IT FELL AND CAUGHT HIS HAND ON ANOTHER STEP. THE EE DID NOT SEEK MEDICAL ATTENT ION UNTIL 3/8/01.
