Employee was eating lunch when EE became strangled on iced tea. Employee stood up while coughing causing EE to loose EE's balance and falling & hitting EE's head on the concrete floor causing a laceration to the back of EE's head.

The employee slipped on some metal causing EE to fall forward. EE felt a slight pain in EE's shoulder but did not seek medical attention at that time. After 5 weeks, EE sought medical treatment and was diagnosed with a strained muscle. EE was placed on restricted duty as of 7/16. The injury is being reported within 10 days of receiving medical treatment.

EE WAS MOVING A HALF FULL DRUM OF ANTI-FREEZE FROM THE SHOP TO A HAUL TRUCK OUTISDE OF THE SHOP TO DRAIN ANTI-FREEZE FROM TRUCK INTO DRUM. EE WAS USING A SET OF HAND TRUCKS TO MOVE DRUM. DRUM SHIFTED TO RIGHT SIDE OF HAND TRUCK AND STARTED TO TIP OVER. EE LUNGED TO CATCH DRUM AND STRAINED LOWER BACK.

EE WAS INSTALLING A NEW DRIVE SHAFT IN THE CAT LOADER. HE WAS SLIDING THE SHAFT INTO PLACE THE SHAFT SLIPPED, EE LUNGED FORWARD TO CATCH SHAFT AND HIS KNEE GAVE.

EE WAS PULLING ON A 1 1/4" RUBBER WATER HOSE TO CLEAN UP SPILLAGE IN SURGE TUNNEL. HIS FOOT SLIPPED IN THE MUD CAUSING HIM TO FALL TO THE GROUND. IT WAS RAINING AT THE TIME OF THE ACCIDENT.

EE WAS ADDING HYDRAULIC FLUID TO THE LOADER. HE PLACED THE HOSE INTO THE TANK AND TRIED TO PUMP FLUID BUT IT DID NOT WORK. HE WENT AROUND TO THE OIL STORAGE CONTAINMENT AREA, WHICH IS 14' WIDE BY 36' LONG AND 2' HIGH. HE NOTICED THE AIRLINE WAS DISCONNECTED FROM THE SUPPLY LINE LOCATED ONT HE SIDE WALL OF THE SHOP. HE THEN STEPPED ONTO THE CONTAINMENT WALL AND WALKED OUT APPROX. 10

EE & OTHER EE'S WERE REMOVING THE FEED HOPPER FROM THE NORDBERG CRUSHER OMNI CONE 1352. EE WAS USING A PRY BAR 51" LONG AND WEIGHING 15 LBS. THE CRANE WAS ATTACHED TO THE INSIDE OF THE FEED HO PPER AND WAS APPLYING UPWARD PRESSURE. WHEN THE HOPPER WAS TO MOVE EMPLOYEES WERE CAUTIONED TO MOVE AWAY. WHEN THE HOPPER RELEASED INJURED LUNGED FOREWARD AND WAS STRUCK BY THE BAR.

EMPLOYEE WAS CLIMBING OFF A VIBRATORY SCREEN. EMPLOYEE STEPPED FROM TOP OF SCREEN ONTO THE TOP OF THE SPRING PERCH LOCATED ON THE SIDE OF THE SCREEN. EMPLOYEE LOST HIS FOOTING AND FELL APPROXI MATELY TWO FEET STRIKING HIS CHEEK ON A HANDRAIL.
