Employee was using a porta-power jack to push the crusher motor base back to tighten v-belt drive. The jack's head kicked out and fell towards the ground, the handle end of the jack flipped up and struck employee in the mouth.

EE was helping unload jaw crusher parts from two tractor trailers with a mobile crane. A motor base was unloaded and placed on the ground. A toggle block was unloaded and placed on the motor base. A crusher shaft was then unloaded and placed on the ground with one end resting on the motor base. EE was walkign away when the toggle block slid off the motor base striking his foot.

EE WAS RE INSTALLING A RETURN ROLLER ON A CONVEYOR APPRLX. 32 INCHES FROM THE GROUND HE WAS UNDER THE CONVEYOR ON HIS HANDS AND KNEES. AFTER INSTALLING THE ROLLER. THE CONVEYOR WAS LOWERED INT O POSITION. AT THIS TIME, THE ROLLER FALL OUT OFONE OF THE BRACKETS AND STRUCK EE ON THE FINGER.

EE WAS CLIMBING OFF A FRONT END LOADER. HIS HANDSLIPPED OFF OF THE HANDRAIL CAUSING HIS BODY TO TWIST OFF TO THE SIDE. HE FELT HIS KNEE POP WHILE TWISTING.

THE EE WAS SITTING ON THE TOP OF THE CRUSHER INSTALLING A PIECE OF RUBBER AROUND THE BOTTOM OF THE CHUTE FEEDING THE CRUSHER. AS THE EE WAS BOLTING THE PIECE, HE FELT A PAIN IN HIS BACK.
