While replacing a bolt in a strap, where rock had fallen out from around it. A piece of rock fell out, measuring 24" x 12" x 10", striking the bolter operators leg while installing the replacement bolt.

Miner had EE's finger pinched between a training roller and a piece of belt structure when a chain broke that was holding a come-along.

While helping the scoop man clean the feeder.The employee bent over to move the slack shuttle car cable out of the way. The scoop operator stated scoop was sitting still when the injured yelled you got me. The operator did not realize they hit employee. When interviewing the injured employee they said the scoop swung over towards them hitting the top of their head.

Employee was cleaning off a shuttle car with back towards the scoop. Scoop man was rock dusting outby areas. EE backed up to the shuttle car never seeing the employee cleaning off the SC. EE accidentally pinned the left leg of the injured employee between the SC and scoop.

While helping to change the batteries on the section scoop in the Apple Mains section scoop charger, the mechanic grabbed the jumper cables to move it out of the way of the scoop tire, coming into contact with a cut in the cable. The cut exposed a slice in the red phase of the energized wire, causing an electric shock.

Mechanic was using a pry bar on a rusty piece of equipment. The pry bar slipped free, causing the mechanics arm to jerk upward quickly. He said that he felt something tear when this happened. The injured worker continued to work his regular duty until 02/11/2015, when he had corrective surgery.

While the roof bolter was bolting, in the #5 entry of the Hickory Mains Section, he cut his right forearm on a roof bolt plate and required stitches.

While operating the roof bolter, the operators drill steels became stuck together in the roof. He tried to separate the two drill steels when he felt pain in his mid to lower back.

While operating the continuous miner, the machine dislodged a post. While moving the post, to reset it, the operator felt a sharp pain in his Right Shoulder. He then worked the remainder of his shift. He did not miss any work until 09/02/2014.

Employee was lifting the end of a steel I-beam while building an entry canopy. He and another contractor were lifting a piece of I-beam when he felt a "pop" in his left shoulder. He worked the rest of the shift and next day before seeking medical attention. He is still awaiting results of an MRI.
