EE was exiting the shop on EE's way to the service truck when EE slipped and fell on a slippery snow/ice covered spot. EE went to a medical facility where it was determined that EE's tibia was fractured.

A coworker was picking landscaping rocks from a muck pile on a walking surface that was uneven, steep, and unstable. As the coworker stepped forward, the rock under the coworker's right foot rolled and right foot slipped off the rock wedging against a larger rock.

UG mechanic was installing a cylinder on the Gradall Scaler. With the anchor end pinned, the mechanic guided the cylinder to connect the rod end into position. As the mechanic guided the cylinder, EE's left pinky finger was pinned between the cylinder and the frame of the scaler.

The employee was mixing Nitrate and Diesel fuel in the Bulk Truck to produce Ammonia Nitrate. As the Nitrate slid down the bin, the flow of Nitrate stopped because of a crust build up. The employee used a digging bar to break the crust to resume flow. When the bar broke through the crust, the employee fell forward striking the lip of the bin.

After completing fueling process, the Fuel Truck Operator climbed back up the scaler to record the hours of operation for fuel records. With three points of contact, the operator descended the vertical ladder of the scaler. As EE's left foot hit ground level, the operator experienced discomfort in the left calf.

Three employees were in the basket of a JLG Aerial Lift bore scooping the the underground roof. As the employees traveled from one bore hole to the next, they noticed a small discolored stone on the roof. One of the employees grabbed the handheld scaler and struck the small stone. As the stone fell to the ground, a larger piece struck the employees left 4th finger.

EE was cleaning up under the primary crusher. EE slipped and fell landing on EE's hands and knees. The fall resulted in the EE experiencing back pain requiring first aid treatment. On April 27, 2022, a medical provider placed restrictions that elevated the injury status from first aid to reportable injury with days of restricted work activity

Panel operator was ascending the catwalk to screen #4 at the Tertiary Plant and felt a pop in right knee. No initial medical treatment was received. An MRI was done on October 7 which revealed a torn medical meniscus. Ortho eval on October 15 prescribes physical therapy with no restrictions.

Two employees were mounting a storage cabinet to a wall inside the tool trailer. One employee turned to pick up an impact drive on the work bench and felt discomfort in lower back and hip.

From a squatting position on the walking platform the mechanic began to remove the middle housing that contained the high pressure filter. After breaking the housing loose, the mechanic turned the housing by hand. The housing fell loose into EE's hands. The weight off the housing, filter and oil cause the employee to fall forward into the scalar from chipping a tooth

As employee was cleaning the snow and ice off the platform scales at the Bakersville scale house. The shovel hit a hard ice spot abruptly stopping the shovel and a snap noise in the employee's right wrist. Causing some discomfort.

The crew was installing a bearing on the stacker conveyor. A maintenance worker used a 6 foot pry bar to lift up the shaft to access the bearing. The pry slipped off the shaft and the worker's right shin struck the toe board below. Note: The injured employee received initial treatment for the injury on 11/16/2017.
