loading resin onto the fletcher twin boom bolter in the #2 entry of SW section the operator of the bolter thought that they were done and started to tram the bolter and ran over the employee's left foot resulting in 2 broken toes.

torn meniscus in the right knee from crawling. EE Never missed any work until today 12/4/25 because EE's surgery is today. EE worked on light duty until today. Again NO work missed until now.

bolting a cut a small piece of rock was falling and they deflected it with their hand. the rock cut the palm of right hand resulting in 5 stitches.

using a pry bar to assist the mechanics in changing the coal chain on the miner, the bar slipped and they struck their hand on a piece of metal resulting in a fracture of the right index finger tip. NO WORK MISSED.

when shoveling in the beltline the rib rolled down onto their left leg resulting in bruising. Employee was transported to the hospital for precautions but was released and the doctor said no work for a week to rest their leg.

When getting ready to go underground EE was opening a new pair of knee pads with a razor knife and the knife slipped and EE cut left palm/thumb resulting in 8 stitches and a follow up with a hand doctor.

EE was removing the rub rail off the joy miner when it fell and struck EE's right foot just above the steel toe resulting in 2 broken toes. EE chose to continue to work through this and not miss any work. Toes are taped together and EE is back underground. No work missed.

Employee took the first side of a new cut in the number four entry when the roof started working. EE backed the CM outby the start of the cut about 20' when a rock fell out between the T3 channels striking EE in the back/ankle resulting in broken vertebrae and ankle.

putting up a T3 channel employee hit their right thumb knuckle resulting in a couple stitches. back to work, no time missed.

EE's drill steels were stuck together and while EE was hitting them to get them apart one came back and hit EE in the face. EE received 5 stitches and they want EE off until the swelling goes down and they can check for any fractures around the eye socket.

Loading plates onto the bolter EE's shoulder popped. No work was missed until starting today 9/30/24 because EE has been running the MBC and taking it easy. On 9/30/24 EE came to work and said cannot go underground because it does not seem to be helping thus starting time off.

When reaching for a stack of plates on the roof bolter a piece of rock fell on EE's left hand resulting in a dislocation of the middle finger and slight break under the nail bed of the pointer finger. Doctor gave EE a splint and NO work missed at all. underground the next day at full duty.

Helping the face boss get a rock off the miner. EE was pushing the rock, and a piece broke off causing EE to run arm into the rock. Small laceration on right forearm resulting in 3 stitches. No work missed.

a bundle of roof plates fell on their hand while loading the bolter with supplies.

The employee was traveling down the slope into the pit when they apparently lost control going around a turn and the jeep slid into the side of the highwall. This resulted broken ribs, a punctured lung, and a lost time accident.

Loading cable bolt plates onto the bolter EE struck a belt chain with elbow which caused EE to hit left little finger resulting in 6 stitches.

Employee was roof bolting when a piece of loose roof rock fell striking EE's back. This resulted in fractured ribs.

While roof bolting a piece of rock fell from the roof striking EE on right ankle causing it to roll over. Resulting in a bruise/strain to the right ankle. Doctor said time off and rest the ankle.

working on a MBC reducer, the guard slid off and struck their right shin resulting in a hematoma and slight tibia fracture. doctor said no restrictions just be careful for a while.

When installing a roof bolt and plate. The plate spun and cut employee's right middle finger resulting in 3 stitches. Doctor said no restrictions and able to return to work.

When swinging a hammer to knock over a T3 roof channel, EE swung and glanced the hammer off the bolter canopy which caused EE to hit EE's own hand resulting in stitches in the left middle finger. No time missed and no restrictions. Back to work next shift.

using a utility knife to cut the shipping band off a lift of channels, they slipped with the knife and cut left forearm resulting in 3 stitches. NO restrictions or limitations. keep clean and return to work.

when taking drill steel out of the bolter head a piece of rock fell and struck left index finger resulting in a broken finger. The doctor said tape it up or splint if necessary and return to work with NO restrictions.

When setting up to pull belt off EE dropped the belt pulling arm on left foot just behind the steel toe. The doctor's report said nothing broke but told EE to wear a walking boot, ice it and rest it. EE has been sweeping floors and cleaning outside since that date.

tramming the bolter out of #2 entry it trammed into a hole in the bottom and swung and bumped EE's arm resulting in a cracked rib.

Knocking a block out of a wall another block fell out and struck EE on the left hand. Went to doctor and came back with NO restrictions, small fracture on left hand pointer finger, wearing a splint for a couple days. No work missed.

Normal working section, EE was cleaning some rock off the miner. A piece of rock EE was taking off the miner flipped off and struck EE's left thumb and caused a laceration which resulted in 6 stitches. NO work missed and NO restrictions from the doctor. Back to work at full duty.

The operator was backing out the first side of the cut when they were struck by a piece of coal rib that fell out behind the curtain striking them in the lower back. This pushed them into the miner where they struck their face. The incident resulted in a fractured L-2, broken nose and 6 sutures above their nose.

pulling a T3 strap off the bolter EE cut EE's finger and had to get stitches. Came back with NO restriction from the doctor. full duty.

When traveling outside as a passenger on a 2 man personnel cart, the driver hit a bump and bottomed out. Employee was holding onto the side of the cart with fingers under the cart. When the cart bottomed out it got the fingers resulting in 5 stitches in little finger and broke the tip of the little finger. No work missed, employee is back 2/4/2022.

When installing a cable bolt into the roof, the head of the bolt and the plate was still on the ground. EE stumbled and went down on left knee and struck the plate just above knee pad resulting in a cut that received 3 stitches.

Employee was installing a roof bolt when unknowingly EE's glove got snagged and when EE started the rotation it fractured EE's right wrist.

Normal working section, while throwing crib blocks to be set EE got left thumb pinched between 2 cribs resulting in 7 stitches.

miss handled a drill steel while moving it from EE's tray, and it struck EE in the mouth resulting in a chipped bottom tooth.

While the operator was drilling in wet conditions, EE lost suction. At this time EE dropped the drill head to the bottom and placed EE's hand over the chuck to check the suction pressure. When doing so the drill steels fell from the hole striking EE on the top of EE's right hand causing a break.
