Employee injured lower back while moving a small distribution box. Not serious, has worked up until 10-12-2012. Doctor requested no work for 2-weeks and was given meds.

Whiling lifting roof rock off the miner a piece broke off striking employee on the chin, resulting in 3 stitches. Not serious, NO LOST TIME.

While traveling along belt line on a mantrip in very low roof conditions, employees knees caught the roof and rolled him backwards. Did not miss work until May 29th, 2012 after doctors visit. Not serious injury told to rest lower back area for 1-2 weeks. Should be able to return to full capacity after that time.

A roof fall occurred in #2 entry of 1st west mains return. It is located 1 crosscut inby spad #226 and continues across the intersection.

No immediate accident. Employee has an occupational illness (miners knee).He had some swelling in his L-knee. Went to Dr. and was diagnosed with bursitis from excessive and repeated crawling. Going to have surgery on 3-14-2012 to clean up his knee. May be off of work 1-2 weeks. Prognosis good

Employee hit a pothole with front tire of man trip and steering wheel spun around and hit his middle finger on R-hand. This resulted in a fracture of middle finger. NO LOST TIME ACCIDENT

A roof fall occurred in the #4 entry of 1st North West Main intake travelway. Fall begins approximately 50ft outby SS #5836 amd extends approximately 60 feet outby and is the entire width of entry.

A roof fall occurred by the #2 entry of 1st northwest mains entry. The fall starts approximately 120 feet inby SS# Z430 in #2 entry and extends approximately 60 feet inby and is the entire width of the entry.

A roof fall occurred on the 1st west main intake at break 12. The fall was at a intersection it measured approx 50'long by 35'wide approx 12'high at highest point to bottom.

Poor roof conditions the man was bolting and a piece of rock slid off of canopy and hit arm cutting it.

While employee was attempting to move a large piece of rock, it slipped out of his hand and pinched his middle finger. This resulted in a cut requiring a few stitches. No lost time.

EE was pulling a heavy piece of rock off of the miner, it slipped out of his hands and fell onto his left hand. Small laceration and bruise of left finger (middle).

While hanging cables EE caught his right shoulder/back on roof bolt plate. Resulted in small laceraton.

A roof fall occurred on the #4 beltline at x-cut 30 in the 2nd west mains headings. The fall was at an intersection. It measured approx 35 feet long x 35' wide. It was approx 11 feet high at the highest point to the bottom.

At approx 2:00PM, a roof fall occurred on the #7 beltline. The fall was 100' in length and ranged from 7-15' high. The fall was located between walls 69 and 71 and adjacent to wall 70 along the 1st Northeast Mains.

Examiner went into return of 1st West Mains on the morning of 11/5/10 and discovered a roof fall which impaired his normal route of travel.

Victim was in between 2 cribs he had built. He noticed a piece of draw slate that needed pulled down. Using a rock bar, he pulled the 3 foot by 2 foot by 3 inch piece of draw rock. Unfortunately, the rock slid down the bar, pinching his right forearm against the crib. The sharp rock cut his skin and bent back his thumb.

A roof fall approx. 60' in length was discovered in the #5 entry of the #1 butt working section approx 600' outby the face area. The fall did not impede travel or Air flow to the working section.

During a rock dust survey, a roof fall was discovered in the #5 entry of the 1st north east mains section approx. 1200 feet outby the face, adjacent survey spad #11950 which is located in the #4 entry. The fall was approx. 160 feet in length and 4 to 6 feet high.

A roof fall was discovered in the #3 entry of 2-2 butt room in abandoned section. The fall was 80' in length and 20' in width. No equipment was involved. The fall was discovered on the weekly air course run.

A roof fall was discovered in the #4 entry of the 2 mains butt, 1 x-cut inby spad #11274. The fall was 60' long, 20'wide and 42 " high. No equipment was involved. The area was cribbed, posted and dangered off. This section was abandoned and equipment pulled out before the fall.

A roof fall was discovered in the #5 entry of x-butt near spad #10,117. Cribs and gob were removed to prepare to build seals when the fall was discovered. The fall area was 4 feet in height and 40 feet in length. Affected area is being cleaned up and re-bolted.

A roof fall occurred in the #1 west main return, #2 entry 1 crosscut inby spad #2273. The fall was 50'x4'. No equipment was involved. Area was posted off.

A roof fall occurred in the #1 west A beltline inby spad #75 and outby spad #84. The fall was 60 feet in length and 4 1/2 feet in height. No equipment was involved. The area was cleaned up and re-bolted and supported with cribs and posts.

Employee pinched his left ring finger between tongue of trailer and edge of scoop bucket. No lost time, not serious.

The victim was loading supplies on a scoop bucket when a piece of draw slate fell from between the roof bolts and struck him on the left hand breaking his little finger. His hand was resting on the edge of the scoop bucket when the 1'x1'x2" thick piece of slate fell. He was not operating scoop at the time.

a piece of draw slate fell from a height of 9 1/2', striking the continous miner operator ont he right hand causing injury to the index, and middle finger. The piece of rock was razor sharp on one end and approx. 1 1/2" on the other, measuring 18"-20" across. Broke 1-2 fingers.

A roof fall occurred in the #2 return entry of the 1st north west main return at spad # 2568. The fall was 20 foot in length and 9 feet in width not blocking the entire entry. There was no equipment involved. The area involved was posted and cribbed.

While riding out of mine (end of shift), a piece of roof rock fell out and hit emoloyee above right eyebrow, casuing a laceration.

Employee was moving a large piece of slate rock. The rock broke in half and a sharp edge cut his right wrist. No lost time medical only. Returned to work after treatment.

Employee was hitting conveyor structure with crib block and caught his little finger between crib and structure. Smashed kin-2 stitches no fracture no lost time.

Employee was capping a 5 gal bucket of grease and tripped over a crib block. Slight fracture outside ankle bone. May be off 2-4 weeks.

No specific accident occured but employee needs his right knee operated on to clean up injuries from years of crawling in low coal.

During weekly exam a roof fall was discovered in a crosscut between #4 and #5 entry of 2nd NW Mains. The location is 1 cross cut inby spad #9260. The fall did not impede normal travel or air movement.

While walking bent over in low coal, EE caught his (R) ear on a roof bolt plate. This required 4 stitches, returned to work same day. No lost time, medical only.

While scaling roof rock down, EE accidently stuck himself in the (R) thigh with pinted end of rock bar. No lost time, medical only, 4 stitches.

A roof fall was discovered in the #5 entry of the R Butt section. It did not impede the normal route of travel to and from the section. Timber was set in various locations in the #5 entry.

Roof rock fell out and hit drill steel. This caused the drill steel to bend. The bent drill steel hit employee's left hand on the back side. Returned to work 11-28-07.

During normal mining operations, miner cut into an 8" borehole that wasn't on any mine maps. Water was coming from borehole. No CH4.

During normal weekly examination of return, two roof falls were discovered. Neither of them impeded travel or air movement. They were at the following locations: 1 crosscut outby spad #5525 in K Butt return and in the intersection of spad #8525 in the O Butt return.

Over a period of time, crawling in low coal, EE has developed Miner's Knee. Occupational Ilness. Left knee has to have cartlidge removed/torn medial meniscus.

Employee was removing a high voltage coupler when high voltage wrench slipped off and hit him in mouth. This resulted in 3-stitches inside upper lip. NO LOST TIME not serious.

Empoyee lifted a heavy battery out of mantrip with poor lifting posture. This resulted in a hernia to lower abodmon.

1st North West main beltline caved, 2 x-cuts inby spad# 2527. Fall was 40'in length & over 40' through the x-cuts & 11'high at the highest point. Area was cleaned up & supported well, resumed normal operations.

Employee pinched left hand between roof canopy and drill steel. Employee tripped and accidently pushed hyd lever causing drill head to move pinching hand. (May miss 1 week of work)?

Victim was pinched between coal rib and battery powered jeep. The jeep was backing towards rib when the tram controls stuck and operator was unable to stop the vehicle.

While employee was traveling intake travelway to outside at end of shift a piece of roof rock fell out and struck him above left eyebrow, resulting in a laceration of the eyebrow area and a small fracture of orbital bone.

2 to 1 old room set-up caved in at an intersection. Area 50' long, 18' wide. About 5' lot set-up of butt on left side.

E-butt rooms on left side; fall occurred at face area of an old set-up. Area was abandoned 9 months. 3 xcuts long, 20' wide in 3-room set of butt.

Employee backed up his mantirp and front tire caught tire of other mantrip. this caused steering wheel to spin aroudn and steer knob hit employee on l back of hand.

Ee postioned himself along L rib, where came in contact with moving tail of continous miner. Miner tail hit employee causing blunt force trauma.

EMPLOYEE HIT HIS BACK ON A ROOF BOLT PLATE WHILE MOVING ABOUT THE SECTION. THIS CAUSED A LACERATION REQUIRING STITCHES, NO LOST TIME, MEDICAL ONLY.

EMPLOYEE WAS TREATED FOR AN INFECTION UNDER HIS FINGERNAIL DUE TO A MINE RELATED INJURY. ON 4/13/05 HE HIT HIS FINGER WITH A HAMMER, SMASHING HIS FINGERNAIL.

While roof bolting employee's right glove got caught between head of roof bolt and drill chuck. This resulted in his right hand-wrist and thumb to spin around causing a sprain.

EMPLOYEE WAS PULLING A PIECE OF CONVEYOR STRUCTURE (ADJUSTING) TO TRAIN BELT. STRUCTURE GOT LODGED AND HIS HAND WAS CUT ON A SHARP EDGE.

EE HAD JUST TRAMMED BOLTER INTO POSITION AND WAS SETTING TRS SUPPORT, WHEN A LARGE PIECE OF ROOF ROCK FELL OUT OF ROOF. ROCK STRUCK TRS BEAM, THEN FELL ONTO OPERATOR'S CANOPY, THEN FELL ONTO EE, STRIKING HIM ON THE BACK. ROCK SETTLED DOWN ON RIB AND CANOPY. EE MAY MISS 2-3 WEEKS WORK.

While installing a line curtain with a pogo stick, the pogo stick kicked out and the sharp end cut ee's right thigh requiring stitches.

WHILE PULLING WIRE ROPES, ROPE GOT CAUGHT ON STOPPING THEN LET LOOSE AND HIT EE ON LEFT UPPER ARM CAUSING LACERATION AND BRUISE. MEDICAL ONLY, NO LOST TIME.

WHILE HELPING ROOF BOLTER A PIECE OF SLATE TYPE ROCK FELL FROM ROOF STRIKING EE'S FOREARM. ROCK WAS SMALL BUT SHARP EDGE CUT 1ST LAYER OF SKIN REQUIRING TETANUS SHOT AND STITCHES.

WHILE LOADING ROCK DUST INTO BUCKET DUSTER EMPLOYEE LIFTED AND TURNED, CAUSING BACK INJURY. MAY BE OFF 1-2 WEEK - REST LOWER BACK.

WHILE CUTTING CABLE EMPLOYEE SLIPPED WITH RAZOR KNIFE AND CUT HIS THIGH. MEDICAL ONLY, NO LOST TIME.

WHILE OPERATING STRUCTURE (MANTRIP) A PIECE OF STRUCTURE HIT THE RIB AND SLID OVER PINCHING FINGER ON STEER WHEEL. NO LOST TIME, STITCHES, RETURN TO WORK THE SAME DAY.

DRILL STEEL FELL OUT OF DRILL HOLE AND HIT EE ABOVE EYEBROW. HE WAS LOOKING UP AT DRILL STEEL TO SEE WHY IT WAS STUCK. NEVER MISSED ANY WORK.

WHILE OPERATING MBC EMPLOYEE'S FOOT GOT CAUGHT BETWEEN OPERATOR'S COMPARTMENT AND RIB. THIS CAUSED CRUSHED FOOT / TOES. POSSIBLE BROKEN TOE, MAY BE OFF 4-6 WEEKS?

EE BENT ARM BACKWARDS WHILE INSTALLING A 6' ROOF BOLT. BOLT TWISTED BENDING LEFT ARM. THIS HYPER-EXTENDED TENDON. CONTINUED TO WORK UNTIL MRI WAS TAKEN ON 6-12-03. FIRST DAY RESTRICTED DUTY 6- 13-03.

EMPLOYEE PINCHED FINGERS ON LEFT HAND WHILE TRAMMING BOLTER. HE HAD FINGER HANGING OVER SIDE OF OPERATOR'S COMPARTMENT.

BAD ROOF IN THE CUT THAT HAD BEEN TAKEN. PIECE OF ROCK DROPPED OUT ALONG THE RIB LINE AS THE ROOF STARTED WORKING IN THE FACE AREA. AS MEN RETREATED FROM FACE, ROCK HIT ROOF BOLTER OPERATOR AS HE RETREATED BACK ALONG MACHINE. MAY BE OFF 3-4 MONTHS.

EE WAS PULLING ON A 3/8 CHAIN WITH HOOK. CHAIN WAS STUCK AND LET LOOSE HITTING EE IN MOUNT CHIPPING FRONT TOOTH. NO LOST TIME, MEDICAL ONLY.

WHILE PULLING LONG BOLTER WRENCH OFF OF BOLTER, EMPLOYEE PULLED HAND AND HIT HIMSELF IN MOUTH CHIPPING FRONT TOOTH.

WHILE JACKING UP LOW TAIL DOLLY, JACK SLIPPED OUT AND HIT EE ON SIDE OF HEAD NEAR JAW/EAR.

HE WAS GETTING OFF A JEEP AND TWISTED KNEE.

PULLING ROCK OFF MINER HURT ARM/SHOULDER TORN ROTATOR CUFF INJURED JUNE 2002 WORKED UNTIL AUG 21ST.

WHILE SCOOPING RUNWAYS, EE PICKED UP WOOD PLANKS IN SCOOP BUCKET TO HAUL AWAY. WHILE TRAMMING WITH PLANKS IN BUCKET, ONE CAUGHT RIB OR BOTTOM & SWUNG AROUND & HIT THE LEFT SIDE OF HIS FACE.

WHILE MOVING OUT OF THE WAY OF A SCOOP, EMPLOYEE TURNED ABRUPTLY TWISTING LEFT KNEE. HE DID NOT MISS ANY NORMAL WORK DUTIES UNTIL 3/20/02.

CONTINUAL USE OF A PREVIOUSLY BROKEN LEFT WRIST DUE TO A MINE ACCIDENT HAS CAUSED NUMBNESS & PAIN FOR EMPLOYEE. HE CLAIMS HE IS UNABLE TO WORK. HE IS CURRENTLY OFF UNDER THE CARE OF DOCTOR.

WHILE SHOVELING HEAVY FROZEN COAL UNDER TAILPULLEY OF CONVEYOR, EE LIFTED AND TWISTED CAUSING LOWER BACK INJURY. EE OFF 3 DAYS RETURNED TO WORK 01-10-2002 FULL DUTY.

WHILE LOADING SCOOP BUCKET WITH CABLE OPERATOR ACCIDENTALY SET SCOOP BUCKET ON INJURED EMPLOYEE'S FOOT. OFF 4-6 WEEKS?

PULLING CABLE IN DEEP MUD. STRAINED LT. KNEE. (OFF 1-2 WEEKS?)

EMPLOYEE BROKE DRILL STEEL OFF IN ROOF. WHILE LEANING FORWARD TO EXAMINE BROKEN PIECE IN DRILL CHUCK, HE STRUCK HIS HEAD ON PIECE OF DRILL STEEL IN ROOF. THIS CAUSED HEAD TO JAM - NECK. (OFF A PPROX 1 WEEK.)

EXCESSIVE CRAWLING CAUSED SWELLING OF RIGHT KNEE(PREPATELLAR BURSITIS). OCCUPATIONAL ILLNESS.

EE EXPERIENCED SORENESS AND SWELLING IN RIGHT KNEE DUE TO CRAWLING.

WHILE VICTIM WAS HELPING ROOF BOLTER INSTALL BOLTS, A PIECE OF DRAW ROCK MEASURING 30" BY 18" BY 2 1/2" THICK FELL FROM RIB 3 RAILS OUT BY PERMANENTLY SUPPORTED ROOF, STRIKING HIM ON LEFT AN KLE, CAUSING A SPRAIN OF ANKLE.

WHILE VICTIM WAS INSTALLING LAST ROW OF BOLTS WITHIN 5' OF THE FACE, A SMALL UNDETECTABLE PIECE OF DRAW ROCK MEASURING 18" X 3 1/2" THICK STRUCK HIM ON THE HEAD CAUSING HIM TO SPRAIN HIS NECK.

WHILE JACXKING OUT LOW TAIL EE LET HANDLE GO AND LATCH WAS NOT IN CATCH CAUSING HANDLE TO RELEASE AND HIT EE ON LEFT SHOULDER (OFF 6-8 WEEKS?0

WHILE PRYING ROCK WITH PRYBAR EE STRAINED LOWER BACK. WILL MISS APPROX 1-2 WEEKS OF WORK.

WHILE CRAWLING IN LOW COAL EMPLOYEE HAD WRIST BENT BACK, CAUSING SPRAIN WITH LIGAMENT AND TENDONDAMAGE. ORIGINALLY HE DID NOT MISS ANY WORK. AFTER FURTHER EXAMINATION OF WRIST, HE WILL BE OFF FOR AN UNDETERMINED AMOUNT OF TIME.

WHILE LOADING/ UNLOADING ROOF BOLTS EMPLOYEE TWISTED RIGHT KNEE. WILL BE OFF APPROX 4-6 WEEKS FOR SURGERY.

WHILE INSTALLING BELT HANGER WITH ROOF BOLTER, HANGER CAUGHT EE S GLOVE. THIS CAUSED HAND TO WRAP AROUND HANGER PLATE AND BOLT PINCHING IFNGER AGAINST ROOF.

WALKED INTO VENT TUBE EDGE, CUT EYEBROW. DID NOT MISS ANY TIME.
