He was lying on the belt under the Jaw crusher to tighten the toggle springs. He set the tools down to roll over to the other side. He noticed a pain in his back as he rolled over. He finished the shift. He reported to work at our asphalt plant on 5-21, 5-22, and 5-24. On 5-25-10 He said that he was experiencing back pain and was unable to perform his regular work.

While cleaning frozen dirt from some bolts to be taken out, he brushed some dirt down in front of him & the wind blew something into his eye.

While trimming metal tail pulley safety guard with a torch, a small piece of molten metal fell into employee's right ear. He had appt. w/ear doctor on 6/26/06 and is scheduled for ear surgery on 7/18/06 for perforated ear drum. He went back to the doctor on 7/12/06 because of irritation & received antibiotic on 7/12/2006.
