EE was welding under the feeder at the primary plant at Penn MD Quarry when a welding bead dropped and fell down into EE's boot, burning the top of EE's left foot
According to the EE, EE "just turned and knee gave out". EE stated that this was a "preexisting injury that EE thought was healed". EE finished EE's shift and did not seek medical attention until 3/12/2026. EE was later caught walking through the location while on EE's cell phone and EE was walking without issue.
EE struck right knee on a piece of metal while climbing out of primary screens.

While emp was descending down the stairs from the catwalk, stepped on a rock and injured left knee.
EE was changing wear plate in tunnel. rock fell from above hit ee in left hand.
EE was cleaning grizzly bars with sledge hammer. Swingning sledge missed rock, sledge hit steel bar at grizzly door. Sledge rebounded and struck ee on top of head. Was not wearing hard hat, it fell off while bending over.
