Loader operator got out of equipment and tried to pick up a 300 lb rock by hand.
Employee claims EE was jolted in seat while operating loader. Refused any medical attention until 4/22/19. Diagnosed with back strain.

CLEARING ROCK JAM, ROCK FELL FROM ABOVE HIT LEFT HAND.
EE was unjamming surge box, product broke loose. Material fell from upper cleanout port thru a opening near the clean out port. Rock fell approx 10' and landed on left hand and also hitting him on lower left leg. Index finger amputated.
STARTING LOADER, FELL ON ICE.
