Removing drive pulley off of Jet Mill #01 classifier, using hydraulic puller to remove pulley from classifier off of JM#1. While coworker was applying more pressure to hydraulic puller; puller stripped/kicked off to the side, catching employee's right hand.

Operator was bagging pallets on Screen 5, picked up pallet to place under new sack and felt pain in left shoulder. Continued to work through shift. Did not report to foreman until pain persisted through to new shift next day that started at 7am. Went to foreman at 11am due to pain not subsiding and affecting work.

Employee was climbing up ladder outside screen #5 to check bin for fullness with headlamp, when climbing down ladder, employee felt sharp pan in left shoulder. Ambulance was called at 9 pm and was taken to emergency room to be evaluated.

Was doing a screen change on Screen #1 and bottom screen needed stretch out and employee left middle finger popped.

Employer was notified that there were 3 msha reportable audiogram shifts during annual medical review bus. Employee will be informed of shift/results and determine next test to proceed.

Received notification of three reportable MSHA audio shifts and must be reported within 10 working days. Will notify all three employees and those will be re-tested for determination and acquire more information and any steps proceeding.

Employer was advised that there were three MSHA reportable audiogram shifts per annual med bus testing. Employee will be informed and any future testing may be advised.

Tripped on beam lying beside tank outside building by ball mills and hurt knee and then back hurt the day after

was performing work in the forklift and knee buckled.

Employee was working around the sack hanger when they missed a step coming down the stairs causing them to fall and land on their bottom.

Was getting off forklift at roller mill 1 when right knee popped. Fell onto right knee after it popped.

Went to step up on forklift and right foot slipped off of forklift. Grabbed handle to keep from falling, twisted, and felt a pop in the right side of back.

Was removing a pipe nipple from Jet Mill 2 cooler system with a pipe wrench using my legs to hold me in place.

Doing screen change on Screen 5, EE got right pinkie caught under deck while letting it down.

Going down vertical ladder. Slipped going down and jammed right foot.

Slipped on piece of wood on stair well and fell

Employee was helping to position a gear box onto a screw conveyor when the gear box fell from the chain fall it was being lifted by contacting the employee on the left thigh.

By compactor, employee was pulling a box full of junk, box ripped and employee fell backwards. When Employee fell backwards employee jammed left wrist. Employee was given a velcro splint to wear for two weeks.

Locker room at Asbury; felt something in right eye. Used eye wash station and also the squeezable tubes of saline solution to try and wash out right eye. Couldn't seem to get out was was in eye.

Employee was walking to reset 10 scale in packing room when EE tripped onto a pallet of cardboard. The employee fell onto the cardboard on EE's right side and arm.

Employee was helping the loader operator make a mix when EE picked up a pallet and experienced pain in EE's back.

Employee was bagging onto a pallet when EE experienced pain in back and neck.

Working on trying to remove pulley from JM1 either dirt of piece of metal got in eye.

Employee was working at Ball mill #1 tightening the door on the top. When EE went to turn the bar, EE "felt a pop and a tear in the groin area". EE was taken by ambulance to the ER. released "off work" until seen by a surgeon. EE's first appointment is on 4-12-19.

Driving down to clean railroad tracks. On way back up to main road passing rail car hit frozen piled up material and hit head on plastic heater column. Seat belt held me down head and neck hit heater column.

Coming down spiral staircase thought I was down but missed last step and fell on floor my radio was on R front pocket.

Was lifting blank screen, it started to bend, when it did something happened to lower back and right shoulder.

Experiencing numbness, tingling, and loss of strength in right hand while bagging on Jet Mill 3.

After annual audiometric testing one employee was found to have a reportable change in hearing.

Employee was cleaning bathroom and was stung by a bee. Taken to hospital on 7/21 and released on 7/21. Returned to work 7/24 (Monday).

Employee was removing strap and shackle, bar or strap moved, pushing left hand into the sheet metal side of the cooler. Causing laceration to 4th finger on L hand .Employee received medial treatment, stitches and x-ray, on 7/20.17 immediately after injury occurred.

Driving forklift into aisle 3, hit the brakes and slid into I beam. Then before hitting I beam, I put my hand out and it cut my middle finger.

Employee was lifting 50lb. bags onto pallet in which last two rows exceeded above head; hurt lower back, right leg.

Cutting shot coke sacks into loader guys bucket with co-worker; co-worker was on L side and I was on Right; trying to get sack into bucket; was steadying left hand on loader bucket below sack & then we were both cutting sides & it fell down - weight of sack appr. 2700 lbs. material fell on left hand & couldn't pull it out. Called co-worker over & dug material & yanked my hand out.

right elbow pain caused by employee was drilling holes in a cement wall, with a hammer drill and it got caught on something and it twisted his right arm. injury was on 7/22/15 first dr. apt. 8/28; x-rays on 8/28; MRI on 9/9; put on restrictive duty on 9/10/15.

Was changing screen on screen 1, when hook slipped off upper deck, fell catching finger between screen and tub.

he was working on the center packer when he need a part from the north end packer so he unplugged the 120 from the rec and opened the control box to right number down and felt a sharp pain in the right hand like he got a shock.

When I stepped off the sweeper, I had a sharp pain come from my upper left leg area up to my left hip area.

OPERATOR WAS TAKING A SACK OFF OF JET MILL 1 WHEN CAP CAME OFF CONVERSION PIECE. WHEN OPERATOR LOOKED UP, MATERIAL FELL FROM ABOVE INTO HIS FACE.

Employee tried to unlatch a loaded dumpster with all of the weight on the latch and pulled left shoulder.

was bagging at screen 4 packer and stacking bags onto pallets. felt sharp pain in right hand.

coming down steps from hanging a sack on screen 3, the heel of his boot got caught on a metal piece on second step & he fell. landing on right knee, hand and shoulder.

He was taking a sack off of roller mill 2 and SC2 airlock started shooting out material and blacked out his area and he tripped and fell trying to find his way out of all of the dust. He tripped over a pallet and fell into a beam and the impact jolted his back.

Employee was driving forklift. Fork caught in bridge deck. Sudden stop forced employees ribs into steering wheel.

Employee states he was getting down from loader with radio and water bottle in hand. He fell from platform approximately five (5) feet onto his back.

The employee forgot to engage the parking brake on the forklift. He got off to untie a sack, and the lift started to roll towards him. The lift hit a beam which stopped it, but a sack of material fell on his right ankle.

THE EMPLOYEE WAS LIFTING A BLANK SCREEN TO PUT IN THE SCREEN #4 DECK AND HEARD A POP IN HIS KNEE. REFUSED MEDICAL ATTENTION AT THE TIME, SAYING HE WOULD GO TO HIS FAMILY DOCTOR. THE EMPLOYEE WAS SEEN BY PANEL DOCTOR ON 7-31-09.

The employee was working on the dryer motor. While removing the counter weight, it slipped and caught his finger.

THE EMPLOYEE WAS BAGGING MATERIAL AT THE PACKER, WHEN THE BAG BLEW APART, BLOWING MATERIAL UNDER HIS SAFETY GLASSES AND INTO HIS EYE.

While checking the prader screen on the second story balcony, the employee slipped and landed on his right hand trying to break his fall.

The employee was bagging material at the auger packer. He placed a 55# bag on a pallet and when he stood up, he felt pain in the inner thigh, leg and lower stomach.

The employee was lifting bags of cement for the blending of material when he twisted his back.

HEARING LOSS NOTICED IN ANNUAL PHYSICALS DONE ON SITE BY PROFESSION HEALTH TESTING.

The employee was on a ladder removing a two inch pipe. The pipe fell sooner than he expected and struck the employee on the shoulder.

The employee stepped down off a cement wall onto wet ground (due to weather) and tripped and sprained his left ankle.

While helping the screen #3 Operator hang a sack at the floor dump, the forklift Operator lowered the forks to quickly and the employee's finger got caught between the hooks of the forklift and the sack of material.

The employee was getting dressed for work. When he put on his shirt, he hit a built-in wooden shelf in his locker and got a splinter under his thumbnail. He left early and went to his family doctor, who removed the foreign object. When I received the doctor's report, it stated that it was a metallic splinter.

THE EMPLOYEE WAS PULLING WIRE AT A JUNCTION BOX THAT RUNS FROM THE AC MILLS, HE HELD UP SOME LOOPED WIRES TO MOVE ANOTHER WIRE, WHEN HE RECEIVED A SHOCK. DURING THE INSTALLATION, THE INSULLATION ON THE WIRES MUST HAVE BEEN DAMAGED, LEAVING A KNICK IN THE WIRE.

EMPLOYEE SLIPPED WHILE GETTING OFF A FORKLIFT.

Two employee went upstairs to do a screen change. One felt weak and had trouble breathing. He laid down on the floor and sent his co-worker to get their foreman, who then called 911. The temperature has been really hot and humid for the past week.

EE was moving drums at the blender when he noticed a sharp pain in his right shoulder. When he tried to finish bagging at the magnetic, the pain go worse. Became reportable on 11/27/07 after surgery.

While attempting to open a thief sampler, the employee's pinky finger slipped into the slot on the sampler. While sliding it apart, it stuck and when the employee pulled harder, the tool sliced off the tip (1 centimeter) of his pinky finger

The employee was driving the sweeper by the jet mill room. The forklift came out of the jet mill room and the two collided. The injured employee banged his head off the frame guard of the sweeper.

Employee was driving the sweeper, when the steering wheel spung around and caught the ee's thumb.

EMPLOYEE WAS CARRYING A BUCKET OF HAMMERS DOWN THE STAIRS FROM SCREEN 4. HE SLIPPED AND FELL AND LANDED ON HIS BACK.

WHILE DUMPING SACKS OF RAW MATERIAL OUTSIDE, THE WIND WAS BLOWING AND SOMETHING FLEW INTO THE EMPLOYEE'S EYE.

Cleaning out the jet mill lines, when excessive material flew out of the line and into employee's eye.

EHILE EMPLOYEE WAS CHECKING FOR LEAKS IN SOME PIPES, SOMETHING FOREIGN GOT PAST HIS SAFETY GLASSES AND INTO HIS LEFT EYE, PIECES WERE REMOVED AND CORNEAL ABRASIONS WERE DIAGNOSED.

EE SLIPPED ON STAIRS FROM THE CRUSHER BALCONY AND CAUGHT HIMSELF, PULLING HIS SHOULDER OUT OF JOINT. THE STAIRS TO THE CRUSHER BALCONY HAVE GRATING FOR THE TREAD. HIS SHOE CAUGHT ON THE GRATIN G AND TRIPPED HIM.

EMPLOYEE BENT OVER TO STRAIGHTEN A BAG ON A PALLET. WHILE STRAIGHTENING THE BAG, HE FELT SOMETHING POP IN HIS LOWER BACK.
