EE was cutting a belt and the knife slipped cutting EE's knee
EE shoveled the belt and hurt upper, left arm.
EE used a push bar to clear a jam and got right, ring finger caught between the bar and a metal plate.
EE opened a door handle that snapped and cut EE's left hand.
EE struck right knee on a piece of angle iron.
EE struck EE's left elbow on the carousel.
Picked up grating and twisted wrong and sprained left knee.
EE lifted a rock
EE picked up a rock and strained back
EE was walking and knee buckled
Pushing tube in hole and felt pain in elbow. Did not go for medical treatment until 12/28/16
air chisel tip broke and embedded in left shin
Carrying a piece of wood w/another employee. The other employee dropped wood and it hit employee in the knee twisting EE's knee.
EE lost footing on rocks and strained knee
EE jarred back when operating loader and struck a rock.
Employee twisted right knee while climbing over rock at quarry.
Employee twisted right knee while working on a truck.
Employee turned to walk back to personal vehicle and turned into front portion of rock truck injuring left shoulder.
EE was stepping away from conveyor support system when a rock rolled off of running conveyor belt and fell, striking EE on left front corner of hard hat. Rock rolled off hard hat & struck face below nose. Injury consists of split upper & lower lips (6 stitches), cracked mandible below nose and 3 cracked teeth. No concussion or loss of consiousness.
Emp was loosening bolts on crusher and while pulling breaker bar, he felt pain in groin area.
EE WAS SHOVELING STONE FROM CONVEYOR BELT, FELT TWINGE IN STOMACH. (HERNIA SURGICALLY REPAIRED ON 2/15/05.)
GOING UP LADDER OF LOADER PULLED SOMETHING IN SHOULDER.
Walking out of tunnel, rock fell from the belt above.
EMPLOYEE WAS INSTALLING TWO NEW ELECTRIC MOTOR STARTERS IN EXISTING PANEL BOX. TURNED BREAKER OFF TO WHAT HE THOUGHT WAS THE CIRCUIT HE WAS WORKING ON. IT WAS NOT THE RIGHT BREAKER. CAME IN C ONTACT WITH HOT WIRES CAUSING A FLASH.
REPAIRING CONVEYOR BELT. PUTTING NEW CLIPS IN BELT, HAND SLIPPED CUT HAND ON OLD CLIP.
WHILE OEPRATING LOADER EMPLOYEE BEGAN FEELING TINGLING AND NUMBNESS IN FEET AN DLEGS AND LOWER BACK. PAIN GOT WORSE DURING REST OF DAY.
