Driving haul truck out of bottom level when the truck started to lose traction and slip backwards. The driver lost control and the truck rolled backwards, impacting the high wall. The bed of the truck was damaged and the drivers head broke the back window. The driver required staples to close a cut on his head.

Plant operator slipped and fell against a guard after changing a screen, he was wearing a harness. The Operator fractured a rib.

Employee complained of pain in elbow and required medical treatment. Employee is unsure exactly what happened other than he thinks he hit his elbow a couple of times recently while cleaning up around the plant.
Employee was perfoming housekeeping operations around a guard shoveling material and caught his finger on the guard causing a laceration to his ring finger on his left hand.
Employee was using a cut-off saw to cut a piece of channel conveyor support. The saw kicked back and hit employee in the chin resulting in a laceration. The cut needed approximately 20 stitches

An employee stepped on a rock while walking on a catwalk next to the primary crusher. He twisted his knee and dislocated his kneecap. He was taken to the doctor for treatment.

Employee was picking up timbers (used for a crane's outrigger cribbing) and loading them onto the back of a truck. His right hand slipped while putting one of the timbers down causing him to drop the timber crushing his left little finger between the timber and the truck trailer.
