The drill operator was assisting on drill maintenance, EE was guiding the drill hammer which was suspended by the mechanic truck boom when EE tripped over a rock, that had not been removed from the work area. The operator stumbled while maintaining EEs grip on the drill hammer, between EEs grip on the hammer and EEs shifting body weight resulted in a fracture to the left scaphoid.
While drilling production blast holes to the coal basin on the Marion 8700 Dragline Bench, an unplanned release of carbon monoxide and hydrogen sulfide inundated the pit area. The Miner was overcome by the gasses and became nauseated. Employee was taken to the hospital where they tested positive for elevated levels of carbon monoxide in their blood.
Injury occurred on 5/9/2019. EE did not seek medical attention until 7/11/2019. Was tightening vice and pulled left shoulder.
Injury occurred on 10/16/16 -did not receive MRI results until 11/11/16 - working but receiving PT. Driving pickup - vehicle veered off to side of road - struck rock with under carriage - bounced arm on center arm rest resulting in stress fracture.
Injury occurred on 3/4/16, did not go to the doctor until 3/10/2016. Off work since 3/11/2016. Removing covers from generator set, twisted left knee causing sprain of ligament of knee and aggravating osteoarthritis.
Changing bushing in blade tilt cylinder - cylinder shifted pinching finger between bushing and blade arm.

Standing near heater, coveralls ignited causing burns to back legs, possible foreign material on coveralls.

Walking to dragline slipped and fell
Working on manlift to install new house crane in 8700 dragline. While setting limit switches for crane operation, check valve on manlift basket malfunctioned causing basket to tip. Wearing the proper PPE (harness) prevented him from falling, although the jarring action caused injury to left shoulder.

Twisted leg getting off dragline. Pulled arm to avoid fall. Accident occurred 7/31/12. Off work since 8/14/12.
Walking toward vehicle slipped in mud - jammed right arm against vehicle - fell back and grabbed vehicle - pulled arm. Did not seek medical attention until 11/8/12 - off since 11/8/12.
Cutting tooth wedges on drag line bucket w/ torch hot slag fell onto shoe burning hole into shoe and burn to top of left foot. Accident occurred on 6/19/12 did not seek medical attention until 7/2/12 off work starting 7/3/12
Bending under mirror banged hat on mirror forehead hit bracket bolt causing laceration to forehead requiring sutures.
Walking in dragline tripped/slipped on grading fell on right knee. Did not seek medical attention till 4/2/12 off since that date.
Climbing onto haul truck front end loader bumped truck causing movement of truck jarring EE. Treatment date 3/26/12 off since 3/27/12.
Mounting haul truck-foot slipped off step-causing body weight on right leg.
Reaching for a roll of welding wire, felt pain in back & abdomen - strain.
Slipped on bucket (Dragline) hit hand on railing.
Dropping belly pan on D-11 Dozer-Pan struck left hand causing fracture to let ring finger.
Moving screen - cut fingers on screen edge requiring stitches.
EE was laying on Dragline tub floor checking weld and twisted and felt pain in his right lower abdomen.
Moving bucket clevis - felt pain in lower back.
Non Chargable. EE was found slumped over inside the water truck which he was driving. According to reports, EE suffered a heart attack brought on by several blockages, including one blockage that was at or close to 100%. EE died on 9/11/10 after life support was disconnected.
Moving hoist chain & felt pain in right groin & side.
Climbing ladder onto haul truck - felt pain in neck. Accident occurred on 6-15-10 - did not seek medical attention till 7-16-10. Off work starting 7-19-10.
Taking bobcat out of garage - wind blew foreign material into eye.
Standing on wooden plank-slipped-fell approx. 12 to 15 inches. Landed on left wrist-causing wrist fracture. Accident occured on 11/11/09. Did not seek medical attention until 11/16/09. Off since 11/16/09.
Diagnosed with carpal tunnel syndrome, worked until 1/21/10, off since that date.
Employee collapsed, suffering from an epileptic seizure on mine property. Employee did not notify the company of this pre-existing condition prior to or during employment. The employee has yet to return to work.
Lifting hood on truck. Experienced pain in neck/shoulder & back area.

Cutting open a crack on fairlead of dragline. While cutting, hot oil & grease from inside the crack, shot out striking the employee in the facial area causing burns around his burning goggles.

Using hammer-hammer slipped pinched thumb between hammer & plate. accident occurred 3/28/08- did not seek medical attention until 4/1/08.
Experienced pain, weakness. Reports indicate he has four blocked vessels in the heart - 3 which are blocked at 100% and one blocked at 90%.
Walking from welding truck, he stepped on a patch of snow-covered ice, slipped and fell.

Aggravation of previous left knee injury from 12/19/06 - was off work from 1/24/07. Returned to work on 3/12/07. Returned for treatment on 3/28 and off work since that date.

Walking on snow covered ground, slipped, twisted knee & lower back. Did not seek medical attention until 2/26/07 - Disability started 2/27/07
Descending loader ladder. Stepped on rock on ground and twisted knee.
Checking underneath truck - lifted head - banged head on truck box - required sutures.
Foreign material entered eye while welding on dragline.
Bending over to tie shoe - back pain - worked until 2/6/07 - off work starting 2/7/07.
Descending ladder - foot slipped off ladder rung, hit knee on ladder. Accident occured on 12/19/06 -did not seek medical attention until 1/24/07.
Climbing into dragline compartment, he slipped - weight of body on left arm. Did not seek medical attention until 1/4/07. Began physical therapy on 1/8/07.
Moving barrel of grease - while rolling & pulling on barrel received strain to right lower back.
Knocking drag socket wedges out of drag sockets, he slipped and twisted lower back.
Pulling on pry bar, bar slipped, employee fell to ground striking elbow on ground. Did not seek medical attention until 11-6-06. Off work since 11-7-06.
Changing brushes in generator set - foreign material entered right eye.
Driving truck. Turned air conditioner fan on. Debris blew into right eye.
Opening garage door to enter building.
Replacing generator-finger pinched between generator and generator base bolt holes.
Descending ladder, he slipped and struck his left knee on bolt.
Walking down steps, slipped and grabbed railing, pulled right arm causing torn rotator cuff to right shoulder. Also abrasion to left leg calf. Disability did not start until 2/24/06.
REPAIRING VEHICLE. LYING UNDER VEHICLE, PULLING ON WRENCH. FELT PAIN IN LOWER LEFT ABDOMEN (HERNIA). ACCIDENT OCCURRED ON 2/15/06, WENT TO DOCTOR ON 2/20/06, DISABILITY BEGAN ON 2/22/06.
Employee was welding and got debris in right eye
Employee was cutting shaft from the drag drum and inhaled chemical fumes.
Dismounting crane, foot slipped off bottom step, jarred back. Experienced back pain. Accident occurred 1-14-06. Pain persisted. Returned to doctor 2/17/06. Off work from 2/20/06 with disc problem.
USING PRY BAR TO ALIGN GUARD ON DRAG PINNION PUSHED ON BAR, FELT PAIN IN HIS NECK.
LIFTING 2" X 20"BOLT OUT OF DRAG BEARING CAP, USING HOUSE CRANE & SLING. BOLT BECAME JAMMED, EMPLOYEE GRABBED BOLT & THE SLING BROKE CAUSING THE BOLT TO FALL STRIKING RT. MIDDLE FINGER TIP. CAUSED CRUSHING INJURY TO FINGERTIP, LEADING TO PARTIAL LOSS OF DISTAL PORTION OF FINGERTIP.
WALKING IN OUTSIDE AREA OF WELDING SHOP, AFTER SNOWFALL - SLIPPED - TWISTED RIGHT LEG - FELL.
Dismounting crane, stepped into ground depression, twisted ankle. Did not seek medical attention until 10/31/05. Returned to work, no lost time & no restrictions. Re-check on 11/2/05 for continued pain. Time lost starting 11/4/05.
Reaching to open valve - cut top of hand on clamp. Sutures required.
DIAGNOSIS OF COAL WORKERS PNEUMOCONIOSIS ON 9/26/05. LAST DAY WORK 9/30/05. HAS NOT AS YET BEEN AWARDED BENEFITS.

Notified on Wed. 4-6-05 @ 3:45 am that 1450 BE Dragline located @ Jeddo Basin was on fire. Dragline was idle @ time of fire & no EEs were working at Jeddo Basin @ time of fire. Fire was extinguished by Hazle Township & Freeland Fire Companies @ app. 4:10 am, having rcvd fire call at app. 3:24 am. There was property damage to cab area & direct current control room.

Stepping onto 120% wabco haul truck deck-twisted left ankle.
EE WAS MARKING DRAG ROPES WITH SPRAY PAINT. HE STEPPED ON WALKWAY AND HIS RIGHT FOOT WAS NOT FULLY ON THE WALKWAY. THE BACK OF HIS FOOT FLEXED DOWNWARD, CAUSING A STRAIN TO HIS RIGHT ACHILLES TENDON.
Trouble shooting electrical problem, stepped into inspection opening.

SUSPECTED EXPOSURE TO NOISE. WHILE BEING TREATED FOR NON-OCCUPATIONAL MEDICAL PROBLEM - DIAGNOSED AS VIRTIGO - DURING TESTING HE WAS FOUND TO HAVE HEARING LOSS IN BOTH EARS. HE HAS APPLIED BUT HAS NOT BEEN AWARDED COMP.
