EE was walking through the plant when EE stated that EE tripped over a pile of dirt and fell to the ground.

Employee was walking down steps at the plant, when EE stepped onto the ground from the last step EE rolled EE's left ankle. EE has a sprained ankle.

The employee has suffered a standard threshold shift in hearing compared to their baseline. The employee has been retrained on the proper use of hearing protection and has also went over the hearing conservation plan with a safety team member.

EE was pulling hand cart with a 55 gallon drum on it through a doorway. The cart became stuck on the door sill, as the employee pulled harder, employee felt pain in employee's back

Removing a PTO from a mobile piece of equipment using wrenches and screwdrivers. A piece of metal got behind the safety glasses and into EE's eyes.

While operating the dozer in reverse, the EE had head turned and struck a rock which jarred their neck.

Miner experienced a reportable threshold shift in hearing levels when compared to EE's baseline audiogram which met MSHA reporting criteria.

while helping to remove a bearing off of a screen shaft, an employee was using a hammer and struck the bearing causing a piece of metal to fly striking injured employee in the lip it was thought to be a laceration, but was later found to be imbedded in injured employee's lip

EMP'S LEFT HAND WAS INJURED WHILE DOING MAINTENANCE ON A ROCK BREAKER. THE EMP'S LEFT HAND WAS CRUSHED BETWEEN THE ROCK BREAKER AND HAND RAIL. EMP HAD 7 STITCHES.

emp was making repairs on plant and was holding onto an i beam, emp felt his right wrist pop. emp reported the claim on 9/26 as record only but on 10/1 made an appt to see dr on 10/02. emp has a broken wrist

Employee was helping repair plant when left ring finger got pinched between pry bar and metal frame.

emp twisted his left ankle while walking around the plant.

Emp was removing snow and ice from scale area when he slipped on ice and fell. Emp's right hand hit a concrete barrier on the way to the ground and he received a laceration needing 2 sutures. Emp returned to work on 1/13 with limited use of right hand. Emp followed up with doctor on 1/14 and released to work with no restrictions, normal duty.

emp. felt a sharp pain in right shoulder while trying to lift the cab protector gate on a loader. emp has missed work, pending further medical eval.

Emp. was pulling a cable and had his knee in place for support, when he began pulling, he felt a pain in his knee. Emp did not seek medical treat at the time of the accident, but later went to er for swelling and soreness. Emp missed 1 day of work, but is still on light duty.

EMP STRAINED HIS SHOULDER WHILE TRYING TO REMOVE A FALLEN TREE OFF OF THE HAUL ROAD.

Removing a piece of drill pipe that needed replaced and as employee was lifting the pipe he got a sharp pain in his left arm-bicep area.

Employee was digging out a stone chute and felt sharp pain in his left shoulder, arm and hand.

Emp hurt left shoulder while climbing ladder.

Employee was lifting parts and felt pain in back. He did not go to chiropractor until 6/29/07, where he was informed he would need more than one visit.

Emp stated he got out of bin truck and felt a pop in his leg. He did not report accident at that time, but did not show up for work on 1/29/07. He did not report injury to me until 2/2/2007. EMP ALSO STATED DID NOT FEEL PAIN UNTIL SUNDAY OR MONDAY.

EMPLOYEE WAS DOING PLANT REPAIRS WHEN HE GOT OBJECT IN EYE.

EMP WAS HELPING TO CLEAN OUT CUSHER, ROCK SLID AND HIT EMP ON THE SIDE OF KNEE CAUSING THE KNEE TO TWIST

EE WAS CLEANING OFF HIS WINDSHIELD, SLIPPED, FELL TO THE GROUND & LANDED ON HIS TAILBONE.

WHILE CLEANING OUT CRUSHER, A LARGE STONE CAME DOWN & HIT EE ON LEFT FOOT.

WHILE MAKING SCREEN REPAIRS A PIECE OF SCREEN RUBBER SLID DOWN AND STRUCK EE ON THE BACK OF HIS RIGHT ANKLE.

EE SLIPPED ON ICE, FELL AND HIT HEAD CAUSED A CONCUSSION.

DIRT IN EYE WHILE DOING PLANT INSPECTION.

EE HEAR A POP IN LEFT SHOULDER. AS HE WAS CLIMBING ONTO TRUCK TO CHECK HIS BED OUT.

EE WAS GETTING OUT OF TRUCK (BIN TRK) AND FELL HURTING HIS BACK

EE WAS USING HIS LEFT INDEX FINGER TO REMOVE A NUT OUT OF IMPACT WRENCH. HE INADVERT SQUEEN TRIGGER AND NUT WAS PUT ONTO HIS FINGER.

EMPLOYEE WAS WORKING ON ROAD VEHICLE AND STRAINED HIS BACK.

EMPLOYEE WAS WALKING AROUND SHOP & FORGOT ABOUT OPEN DITCH & FELL INTO DITCH.

WAS LIFTING/CARRYING A MAGNETIC DRILL. HERNIA DIAGNOSED ON 6/1/00.
