EE was cutting with torch and hit head on bottom of screen deck

EE rolled left ankle and fell onto right side. EE did not go for treatment until 4/4/19 and was diagnosed w/a fractured right rib.

EE was cutting angle iron and the blade broke off cutting right leg.

Employee pinched hand between gear box and conveyor frame.

EE was running loader and the machine swayed and EE strained right shoulder

Climbing into truck body and strained back

Employee slipped and sprained left ankle.

Slipped on ice injuring left knee/right shoulder

Using razor blade slipped and lacerated left hand thumb and index finger

Leaning forward and when employee stood up pain in lower back.

Employee developed back pain from performing work duties and operating equipment.

Struck on left thumb by rock which had jammed in crusher; sustained fracture of left thumb.

Employee was climbing into water truck when boots were wet; Employee slipped off step and sustained left calf strain.

When pin came out of backhoe at the jaw it fell and corner hit right elbow spraining it.

Employee was tightening bolt and tool slipped off while tightening and smacked hand cutting it on metal

Employee reached for the brake on the backhoe and felt sharp pain in right elbow, forearm and hand. Was lifting pipe prior and thinks this could have contributed as well.

Employee was told to clean snow off belt. Instead of cleaning belt front catwalk, employee decided to step up onto belt causing him to slip and fall.

EMPLOYEE WAS CLEANING OUT ROCK JAM AT 500 PLANT. WHILE SHOVELING & PICKING OUT ROCK, TWISTED KNEE.

EE WAS UNJAMMING SCREEN. ROCK FELL AND HIT RIGHTKNEE.

EMPLOYEE WAS DOING MAINTENANCE ON CONVEYER TIGHTENING ROLLER SLIPPED IN MUD FELL BACK AGAINST LOADER BUCKET, HIT HEAD.

EMPLOYEE WORKING ON JAW CRUSHER, USING PRY BAR TO MOVE SHAFT.

EE WAS OPERATING BACKHOE IN GUARD. BUCKET PIN FELL OUT. STEPPED OFF MACHINE TWISTED RIGHT ANKLE.
