Employee was on knees working on a water pump at the wash-out. When they stood up, the right knee popped. Employee continued working thru the day but the next day it was swollen. The doctor said the ligaments were OK and gave employee a Cortisone injection.

Employee was on ground when a hoist began lifting a screen cloth up to the screen. The cloth became detached from the lifting hook and bounced around when it struck the ground. It hit the employee's hand. It barely made a mark and the employee worked the rest of the week. Over the weekend EE went to the ER and was told EE had a broken bone in left hand.

After servicing a skid steer, the mechanic shut the engine lid. A cloud of dust was thrown by the fan into his face. He rubbed his eyes, but later his left eye was bothering him. He rinsed it but that didn't help the irritation. He went to the doctor who discovered a piece of metal embedded in his eye which he removed using tweezers and a brush.

EMPLOYEE WAS OPERATING A HAND GRINDER WHEN THE CUTTING WHEEL SHATTERED. THE GRINDER WAS KICKED OUT OF HIS HAND, CUTTING THE 4TH FINGER OF HIS LEFT HAND. THREE SUTURES WERE REQUIRED TO CLOSE LACERATION.

EMPLOYEE WAS USING A KNIFE TO CUT ZIP TIES ON A TAIL PULLEY GUARD SO IT COULD BE REMOVED TO INSTALL NEW ROLLER AND SKIRT RUBBER. THE KNIFE BLADE SLIPPED AND HE CUT HIS RIGHT INDEX FINGER DEEP ENOUGH TO REQUIRE SUTURES.

While installing belt rollers, the employee was pulling hard on a wrench to tighten the bolts and strained his right shoulder. At a later date, it was determined he had torn his rotator cuff.

While installing teeth on loader bucket, employee used hammer to adjust tooth position. A sliver of metal came off the tooth and struck his left arm. It penetrated the skin and was lodged there.

Employee was pounding a pin into position on a trackhoe and a fleck of metal flew up under his safety glasses, striking him in the left eye. He thought it just bounced off and continued working. It was a few days later that his eye became irritated and he discovered that the fleck was lodged up under his eyelid.

mechanic started acetylene/oxygen blow torch, heard loud popping sound, turned it off, it "blew up" near hose. his left palm, side and back of hand was burnt. He performed first aid on himself ,we sent him to Tyrone Hospital where they prescribed him pain relievers, antibiotics and wrapped his hand. torch was rebuilt approx. 2 years ago by National Valley Gases.

EE went to see a physician from our Workmens Comp panelists who referred him to an Orthopedic specialist who recommends surgery scheduled for June 22nd 2004.

WHEN IT STARTED ROLLING FORWARD EE REACHED OUT TO STOP IT AND GOT HIS LEFT INDEX FINGER CAUGHT IN BETWEEN THE HITCH & THE TRUCK. RECEIVED 6-7 STITCHES, ANTIBIO

EUCLID DRIVER WAS INSTRUCTED BY MECHANIC TO CHECK ANTI-FREEZE TEMPERATURE OF HIS EUC E-16 WITH A HAND-HELD HEAT GUN. EE UNFAMILIAR WITH USING A HEAT GUN LOOSENED CAP AND WAITED FOR HISSING TO STOP AND RESUMED LOOSENING CAP, PRESSURE BLEW OFF CAP AND BLEW ANTI-FREEZE ALL OVER HIM.
