Mine manager was conducting pre-shift inspection when EE noticed a roof fall in one of the rooms. EE was able to stop any miners from entering the mine and called the field office supervisor and notified. There was no injuries to miners, and no equipment damage. The fall seemingly, occurred overnight when no one was working.
While walking along a conveyor catwalk, employee tripped over EE's own feet and fell (same level), spraining EE's left knee.
Plant Mechanic sprained right knee when EE stepped down laterally 18-inches from a crusher lube platform to the ground below. EE reported feeling something move in EE's knee but had no pain or swelling. EE requested medical attention the following day; EE treated and released to full duty. The incident became reportable on 11/7/17 when EE began physical therapy.
Injured employee received a fractured lower left leg. Injured was bolting roof in the mine while in the man basket of the bolter. The connection on the machine between the man-basket boom and the equipment chassis failed causing the man basket & employee to fall to the mine floor. EMS was summoned.
Blast Crew member injured EE's right hand when EE tripped over a rock while walking and reached out to catch self from striking the mine floor.
Employee sprained his left shoulder when a wrench slipped as he was tightening a hose fitting bolt on a drill. When the wrench slipped, he extended his left arm and hand to catch himself from hitting the boom.
During the pre-trip examination of a mechanical scaler, the operator strained his lower back as he climbed from the machine's catwalk to the upper level.
Miner was standing on the drill tire & using a pry bar to move into position for repair work the cylinder of the drill boom. He lost his balance and fell to the mine floor, apx 4ft below.
Haul truck driver found to have a lumbar sprain after failing to properly lower bed in controlled manner.
A large rock unexpectedly fell from mine roof and struck the bucket of front end loader while loader was mucking out a shot. The miner operating the loader was jostled inside the loader cab and received sprain/strain to neck and back. The work area had been previously scaled.
A large rock unexpectedly fell from the mine roof and struck the cab shield of a rock truck while mucking out a face. The truck driver was jostled inside the cab and received lacerations to head and tongue, a concussion, and fractured sternum. The work area had been previously scaled.
A metal eye on a metal plate broke loose and the plate smashed employee's index finger on right hand. Employee needed stitches in finger. Returned to work same day. Employee was wearing a safety welding glove. Plate did not cut thru glove, but did cut employee's finger.
Blaster's Helper received 2 inch laceration and sutures to right foreman when his arm came in contact against broken drill steel that was loaded on truck for later disposal.
Underground mechanic sprained his left ankle when he stepped on a rock as he climbed down from scaler.
Worker received a back sprain while operating a D9L bull-dozer to back-fill a surface mine pit.
Worker suffered a pinched & minor cut to the left thumb as he was attempting to re-install a steel pin for a screen deck.
Worker was hand scaling mine roof when he was struck in the head by fallen rock.
Dozer Operator was pushing waste material from access road, when material under dozer gave way and the dozer slid over high wall.
Employee was working on air line when puff of air blew dirt into his eyes. Particle had to be removed from eye.
Operator smelled smoke and exited machine to investigate. Found fire in the battery compartment and tried to put out with extinguisher. Fire would not go out and operator initiated mine evacuation.
A roof fall occurred which propogated from an older fall. The new fall traveled from an unused part of the mine and stopped beside a haul road. Material from the fall piled up on part of the haul road.
Employee was air arc cutting when a piece of metal blew up under his shield and struck his eye.
Employee slipped and started to fall. Tried to grab a piece of steel which sliced open his left hand. Employee was taken to the hospital where he obtained stitches.
A unknown type fire started in a screen deck during maintenance which spread to 2 conveyors and 2 additional screen decks. The fire was extinguished in approximately 40 to 50 minutes by local fire departments. There were no injuries or illnesses resulting from this occurance.
Employee was taking hoist cylinder hose off loader when hot oil sprayed on his face.
Employee was measuring the opposite end of a chute while another employee was grinding at the other end. A piece of steel went into his eye.
Employee was grinding with a handheld grinder and a piece of steel went between his cheek and his safety glasses, impacting his right eye.
EE was holding the hand rail of the loader basket when the boom moved farther than expected and pinned his fingers between the basket and the rock face.
Employee was holding a piece of drill steel as a punch which was struck with a hammer to drive out a pin. A piece of the steel broke off and impacted the side of the employee's head, giving him a laceration.
EMPLOYEE WAS REMOVING THE DRIVE SHAFT ON A LOADER WITH PRY BAR. THE DRIVE SHAFT FELL AND HIT THE PRY BAR CAUSING IT TO HIT THE EMPLOYEE IN THE CHIN. STITCHES WERE NECESSARY.
Employee felt a pinching sensation in his back and his leg started to go numb.
Scaler operator experienced loss of hydraulics and then noticed smoke and fire at rear of machine. Operator exited machine and initiated mine evacuation while scaler continued to burn.
Employee wiped eye while wearing glove causing a speck of metal to become embedded in eye.
While changing screen cloth, eye started hurting and watering. A piece of the cloth must have broken and impacted the eye.
Euipment (Gradall Scaler) vibration and bouncing caused back strain
EMPLOYEE WAS CLEARING ROCKS FROM A BIN & A 6" ROCK FELL ON HIS HAND CAUSING A LACERATION. HIS TREATMENT WAS STITCHES ON THE WOUND.
WHILE LIFTING A 55 LB. BOX OF EXGEL, IT SLIPPED.WHEN EE CAUGHT IT AND PUT IT INTO THE ANFO LOADER, HE FELT A PULL IN HIS RIGHT SIDE.
GETTING OFF BAILEY MACHINE COMPANY TRUCK AND RING FINGER LEFT HAND CAUGHT ON SIDEBOARD WHEN HE STEPPED OFF. SEVERED HIS LEFT RING FINGER. NOT ABLE TO REATTACH.
BOLTER DOOR SLAMMED ON EMPLOYEE FINGER.
FINISHED WELDING BETWEEN CRUSHERS AND REMOVED WELDING HOOD. WHILE REACHING FOR HIS REGULAR HARD HAT, HE CUT THE TOP OF HIS HEAD ON A BOLT.
FALL APPEARS TO HAVE BEEN THE RESULT OF HIGH LATERAL GROUND STRESS ACTING UPON A LOW ANGLE FAULT ZONE. THIS AREA HAD BEEN INACTIVE SINCE JUNE 2000 DUE TO ADVERSE GROUND CONDITIONS. THE AREA IS 165 FT LONG BY 45 FT WIDE AND APPROX 20 FT ABOVEROOFLINE. THERE WAS NO EQUIPMENT DAMAGE OR PERSONAL INJURIES.
SAND PLANT WAS SHUT DOWN DUE TO CLOGGING, EE ATTAMPTED TO LATCH ACCESS DOOR WHEN LATCH SLIPPED OUT OF GROOVE AND HIT EE ON RIGHT SIDE OF FOREHEAD.
EE WAS MARKING AN 1/8" STEEL PLATE TO DESIGNATE DRILLHOLES. THE PLATE WEDGED AGAINST THE FRAME WITH HIS FINGER IN BETWEEN, CAUSING INJURY.
ROOF FALL OCCURRED IN L/37 INTERSECTION, IMMEDIATE AREA OF FALL WAS THE SITE OF A SMALL ROCK FALL APPROX 2 YEARS AGO, THE ROOF WAS UNBOLTED, THEAREA SURROUNDING THE INTERSECTION WAS BEAMED AND DANGERED OFF. AREA OF FALL WAS APPROX 60' LONG BY 40' WIDE BY 20' HIGH.
WHILE TIGHTENING PUNCH PLATE, THE RATCHET SLIPPED AND SMASHED EMPLOYEE'S RIGHT PINKY FINGER BETWEEN THE PLATE AND RATCHET.
MINER WAS EMPLOYED AS A LOADER OPERATOR FROM 1973 TO 1991 FOR LATROBE CONSTRUCTION. FILING OF FORM 7000-1 SHOULD NOT BE CONSIDERED AS AN ADMISSION THAT HE IS SUFFERING FROM OCCUPATIONAL LUNG DISEASE.
ADVERSE GEDOLOGY IN THE MINE ROOF. FALL ENCOMPASSED AREA APPROX. 60' LONG, 45' WIDE, 15' HIGH. MSHA PITTSBURGH DISTRICT OFFICE WAS NOTIFIED AND INSPECTOR OKEY WOLFE CAME TO INVESTIGATE.
FALL OCCURRED OVER THE WEEKEND WITH NO PERSONNEL IN THE MINE. THE FALL ENCOMPASSED AN AREA 45' WIDE, 100' LONG , 8' THICK. ADVERSE GEOLOGICAL CONDITIONS, MUD, SLIPS AND FAULTS WE BELIEVE CONTR IBUTED TO THE FALL. WE STOPPED MINING IN THIS ROOM APPROX. TWO WEEKS AGO DUE TO THESE CONDITIONS.
EMPLOYEE WAS CUTTING FLASHING WITH UTILITY KNIFE--SLIPPED AND CUT HIS RIGHT LEG - 3 STITCHES.
EE WAS WALKING UNDER THE C-10 TUNNEL BELT WHEN HE SLIPPED AND FELL LANDING ON HIS LEFT HAND AND WRIST. IT WAS HEAVILY SNOWING AND SLEETING.
EE WAS ON TOP OF SAND PLANT BAGHOUSE LOOKING DOWN INTO BAGHOUSE WITH THE DOORS OPEN, A GUST OF WIND BLEW THE DOOR SHUT AND SMASHED HIS HAND.
EXCESSIVE HORIZONTAL STRESSES CAUSED ROOF TO FALL. ROOF STARTING FLAKING AT 11:00 AM. ALL PERSONNEL REMOVED FROM AREA, ROOF FAILED AT 4:00 PM. FAIL ENCOMPASSED AREA 45' WIDE 80' L 20' H.
EE WAS TIGHTENING BOLTS ON A SCREEN DECK AND DIRT FELL IN HIS EYES. THE EE WAS WEARING SAFETY GOGGLES AT THE TIME.
EE WAS WALKING DOWN STEPS OF CRUSHER, HIS HEAD AND NECK JERKED BACKWARD CAUSING PAIN AND STIFFNESS IN NECK AND BACK.
